1 The Senior PGA of Taiwan Admission Application Fact Sheet Eligibility Males 50 years or older with upstanding personality and in good health condition are welcome to enter with the recommendation from a professional player member. (Please attach a copy of passport on the application form.) 2. Entry Fee The entry fee for Admission Application is NT$ 36,000 (includes 3 days of green and caddy fee). Please send 1). the completed application form & Club model form along with 2). the receipt (entry fee is only accepted from bank wiring (the bank information is listed below) and 3). a copy of Passport by certificated mail to TSPGA. 2 5. Test (1) Golf Seminar All participants should attend golf seminar on 15 th August 2016 at Golf Course (conference room). Those who did not attend the seminar are not allowed to participate in the technical test. Participants who had been attended Golf Seminar from 16 th August 2014 to 15 th August 2016 do not need to attend the Seminar. Please inform TSPGA when applying for this test. *1400~1450pm: Etiquette & The Constitution of TSPGA *1510~1600pm: Rules of Golf (2) Test The test will be a stroke play of 54 holes over 3 rounds with 18 holes each in accordance with the rules of golf as approved by the Royal and Ancient Golf Club (R&A), and the local rules approved by TSPGA 2016 Tournament. The teeing ground area will be designated by TSPGA. The total score for passing the test should be 234 strokes with 18 above par. In the event of natural disaster or uncompromising weather, the test shall be canceled and scheduled for another date if the first round is not completed. If the first round is completed, but not the second, then a future date shall be selected to complete the remaining portion at the same location. Practice day 15 th August 2016 (please booking in advance) Test day 16 th ~18 th August 2016(Tuesday ~ Thursday, three days) *Local rules and Entry list of first round will be published in Golf Course starter and TSPGA website. *Participants who practice at Golf Course have discount on caddie fee (NT$1995 on Monday through Friday; NT$2295 on holiday) and can also use the blue tee when they provide their Admission Application form. 2

3 *Please use (a) the standard driver and irons; (b) the ball with same brand and model; if not, the applicant will be subject to losing his eligibility. Please visit on USPGA website ( for detail. Participants should fill out Club Model form along with application form to TSPGA. 6. Notice All participants will be expected to follow an honors system and to abide by all applicable rules and regulation. Should any individual cheat or break any rules, the participant and the proctor may be subjected to the following: I. Individuals guilty of cheating will lose of the eligibility to participate in this program for an indefinite amount of time. Proctors who knowingly facilitate or turn a blind eye to acts of cheating shall be banned from competitions in the country for a year as well as banned from proctorship in the future. More serious situation may result in expulsion from the organization. II. III. IV. Participants who fail to show up without reason will lose eligibility for a year and will not receive a refund. Participants should display proper appearance. Poor sportsmanship, character, or vandalism will result in stripping of eligibility. The dress code forbids no-sleeve tops, jeans and shorts. Use of betel nuts, alcohol beverages and mobile devices can result in loss of eligibility without refund. 7. Others Members should pay the annual dues of the year that they pass the test. Members have the rights to participate in monthly tournament, open tournament and championship held by TSPGA. The eligibility for each tournament should follow the rules of TSPGA. Foreign members can participate in above tournaments in the first year of being TSPGA members, doesn't need to attend pre-qualifying. The second year eligibility should be based on the first year ranking. Members shall be granted with the rights of election or being elected to be the Directors and Supervisors of the Board of Directors & Supervisors. 3
