1 Zero Harm Information Brief An Information brief for all Downer Technical & Asset Services personnel. This brief to be discussed in toolbox meetings using DA-ZH-FM013.1 to record the meeting contents and attendees. Please ensure this report is discussed and referred to in your minutes. Lost Time Injuries Total Recordable Injuries Medical Treatments First Aids Restricted Work injuries Environment/Plant and Equipment Damage Lag Indicators: Shows us how many unwanted events occurred Procedural Breaches SWMS Verifications No Harm Reports Zero Harm Summary Hazards/Near Miss Identified Safety Observations Motor Vehicle Incidents Audits / Inspections Lead indicators: Shows us how good we are at identifying and fixing safety areas The Technical & Asset Services Division continues to maintain a strong safety performance with no LTI recorded since February 2011 (3.156 million man hours without a lost time injury). Recordable injuries inclusive of MTI & RWI also remains low, with the current FR sitting at 1.59, a 76% reduction from the previous 12 month figures. 5 x First Aid Cases were reported during the month of September, all of which required either localised first aid treatment or precautionary review by a health care professional, with full clearance to pre injury duties provided at time of consultation. Three high potential incidents were reported for the month, one of which was a breach in Cardinal Rule No# 9 Never operate a vehicle while using a handheld mobile phone nor without wearing a seat belt where fitted. The breach involved a Downer contractor, and correspondence highlighting contractual obligations to Downer was delivered. A full ICAM investigation was conducted as required. The business continues to produce high levels of lead indicator performance through ongoing safety observations & inspections conducted by Field Supervisors, HSE teams & Management. In total, 479 Inspections/ Observations/ Risk Assessments & Audit reports were recorded via Pronto forms application. A Federal Safety Commission surveillance audit was completed on NBN in WA, results yet to be released. The Total Environmental Incident Frequency Rate (TEIFR) remains at a low level due to the current nature of work (relatively low environmental risk). The TEIFR, currently at 2.3, is Page 1 of 7
2 expected to increase with the commencement of NBN Co civil construction project and efforts to improve the environmental reporting culture. Achievements & Key Initiatives Service Strikes: NZ compared to AU NBN controls draft report indicates learning on both sides. AU can utilise documented hazard controls not yet fully implemented on NBN such as non metallic crow bars, detailed Hazard Control Plans showing distance from existing service rules etc. Combined workshops will be held between NZ and AU after final report released to close current gaps on both sides. Asbestos: NBN - Business as usual on NBN contract now that RTO training is completed by removers and supervisors and systems certified by an occupational hygienist. Foxtel - No ACM pits to be broken into on the Foxtel contract Telstra Although we currently meet legislative requirements, there is lots of work here to meet Telstra s requirements that are well above legislative requirements. These include all contractor companies obtaining class B licences, health surveillance, systems verified by an occupational hygienist, pre inspections and approvals of every individual by Telstra. Work at Heights: Ongoing trials for photographic evidence by techs for every instance when working at heights Now doing Tech of the month newsletters sent to all techs each month showing positive photos, text etc for techs completing quality work safely Measuring bench marks of where we are today using unannounced inspections to get a real picture of our compliance Important Alerts and Bulletins Released and Communicated ZHA Lacerated Chin ZHA Technician Bitten by a Dog ZHA-S-002 Toxic Liquids in Drink Bottles or Food Containers ZHB13-40 Snake Found in Roof Space ZHB13-42 Breach of Cardinal Rule 9 ZHB13-43 Dark Work Spaces NBN Safety Alert 28 Underground Asset Strikes SA13-09-Seat-belt-saves-forklift-driver Page 2 of 7
3 September Incidents Significant Incidents (SIR) this month. 02-Sep Incident Injury NBN-MDU - VIC FAC - Technician while using a hand trowel, ran trowel through conduit and hit wire, causing arm to be thrown back from electric shock. ECG conducted - fit to return PID next shift. ICAM investigation in progress. 04-Sep Incident No Harm NBN-Z5 - NSW NHI - CARDINAL RULE #9 BREACH - Technician was witnessed using his mobile phone device whilst driving vehicle. No injuries sustained. A Just Culture teleconference was conducted with various outcomes. In addition, a letter was sent to Kordia detailing the breach of contractual agreement. A Zero Harm Bulletin was also distributed to create awareness. No Lost time Injuries (LTI) this month. No Recordable Injuries (TRI) this month: 52 x Hazard (HAZ), No Harm Incident (NHI), Near Miss, Environment, Equipment/Plant/Property and First Aid (FAC) incidents this month: 03-Sep Hazard NBN-MDU - NSW HAZ - Technician while trenching at the front of property, dug up asbestos. Work stopped immediately. An alternative trench location was explored and NBNCo to notify Body Corporate of asbestos presence. 05-Sep Incident Equipment/Plant/Property NBN-Z5 - WA EQUIP/PLANT - Technician while conducting directional drilling, hit a 25m gas service line. No injuries sustained. Repairs made immediately. ICAM investigation completed. A toolbox was conducted to discuss the findings of recent service strikes investigations. 05-Sep Incident No Harm TELCO - NSW NHI - Subcontractors left asbestos containing materials on side walk, after cleaning out the asbestos pit in preparation for cable cutovers. Subcontractors will be re-inducted and retrained in Asbestos Awareness and SWMS. 17-Sep Incident No Harm NBN-Z5 - WA N.MISS - Operator while using a forklift to lift fibre optic cable drum, drum slipped off forklift and rolled to the back of the delivery truck. No injuries sustained. No person was within the vicinity (either side of the truck). ZH Advisor informed operator not to lift similar size drum until further notice. 24-Sep Incident Injury Foxtel - QLD FAC - Technician while completing FOXTEL Installation, was bitten by a dog at the side of the stomach. Resumed normal duties after GP consultation. Technician completed risk assessment which identified dog onsite and asked customer to restrain dog which customer did. However dog managed to escape. 26-Sep Incident Equipment/Plant/Property NBN-MDU - NSW EQUIP/PLANT - Technician while drilling through stairwell slab for a cable hole, hit main power. No injuries sustained. Investigation in progress. 30-Sep Hazard Warehouse - SA HAZ - State Operations Manager noticed workers within the 4m exclusion zone of a forklift in operation. Forklift operator did not wear seatbelt and no pre-start was filled in. No injuries sustained. Investigation in progress. Page 3 of 7
4 Performance Graphs The graphs below show the current trend on Injuries. Our goal is Zero Harm. This means we want you to get home safely every day, the same or better than when you came to work that day. The graph above shows the amount of Lost Time Injuries (LTI) we have had in the past 12 months. LTIFR - The Lost Time Injury Frequency Rate is a calculation of how many lost time injuries we have vs the hours we work. The number indicates (on past performance) how many lost time injuries will occur in every 1 million hours we work if we don t change anything (e.g. training, new processes, life preserving rules etc). As we only complete around 100,000 hours of work each month and it takes 1,000,000 hours to make 1 LTI into 0.5 LTI s, it takes a long time to reduce our LTIFR. As we use a rolling 12 months calculation. LTISR Lost Time Injury Severity Rate is how many days (on past history) someone will have off work if they are injured in our business. The current LTISR is 0 for. Last year the LTISR was 0. The graph above shows: TRI Total Recordable Injuries (Medical Treatment, Restricted Work, Lost Time or worse, does not include first aid) we have had in that month. Page 4 of 7
