2 Invitation To Attend Dear Colleagues, I would like to invite you to attend our meeting of phlebology friends and colleagues at The Peaks Resort and Golden Spa, Telluride, Colorado Attendee numbers will be limited to keep the meeting relatively small (40-50 practitioners), although we will not restrict or prevent others from attending if they request - this is not a selected group of invitation only attendees. This is a meeting of friends and colleagues in a particularly pleasant location, to discuss informally any topic of interest relative to the phlebology specialty. 3 The Phlebology Colorado meeting in Telluride was the most dynamic and interactive phlebology meeting I have ever attended. The format of the meeting encouraged discussion and I came away with numerous ways to make my phlebology practice better. I will definitely be back next year. M. 4 General Information PROGRAM OVERVIEW Format is completely informal, no topics for presentation other than those proposed by the sponsors for general discussion including case presentations. Each participant is encouraged to render an opinion and advice; attendees will be polled and the microphone passed around the room. Each participant is encouraged to bring a case for presentation and discussion ACCREDITATION/CME CREDIT This activity is currently being evaluated for accreditation by the University of Alabama at Birmingham (UAB) School of Medicine Division of Continuing Medical Education (CME) which is fully accredited by the American Accreditation Council for Continuing Medical Education (ACCME). The academic convenors of Phlebology Colorado meeting take responsibility for the content, quality, and integrity of the CME activity. Mountain Village is connected to Telluride by a free gondola ride. Located at 9,500 feet above sea level, Mountain Village overlooks some of Colorado s most magnificent peaks. The year round alpine resort was founded in 1987 and hosts the ski resort headquarters, a golf course, restaurants, lodging, shops and other amenities of a thriving community. COMMERCIAL FUNDING AND FINANCIAL SUPPORT The Phlebology Meeting at Telluride is being funded in part by unrestricted educational grants from various commercial organizations. A complete list of commercial supporters and contributors will be published in the final program. CANCELLATION POLICY Cancellations will be accepted until October 29, Cancellations MUST be in writing. All cancellations will be processed within four weeks after the meeting. No refund will be given to no-shows. In the unlikely event that the course is cancelled, the organisers will give a full refund of the fees paid less a $50 processing fee.
