1 ORGANIZATIONAL STATEMENT OPEN POLISH SWIMMING MASTERS CHAMPIONSHIP SZCZECIN, FLOATING ARENA, ORGANIZER: Miejski Klub Pływacki (City Swimming Club) 16 Wąska Street, Szczecin and Serbin-Racing s.c. (Partnership) 90 Łukasińskiego Street, Szczecin. COMPETITION DATES: COMPETITION VENUE: Olympic Swimming Pool of Sport, Recreation and Rehabilitation Center "Floating Arena" 16 Wąska Street, Szczecin. TECHNICAL CONDITIONS: swimming pool- 50 m, 10 lanes, water temperature: 27 degrees, electronic measurement, warm-up pool- 25 m. PARTICIPATION: The competition is open to players 20 years of age and older (born in 1995 and older), competing in two runs in each block + one relay-race. Players shall submit a written declaration that they are healthy and participate at their own risk. ENTRY FEE: The entry fee for foreign participants is 30 Euro. This amount must be paid no later than 05/06/2015 (Friday) to the bank account: MAŁGORZATA SERBIN. ŁUKASIŃSKIEGO 90 SZCZECIN , ACCOUNT NO.: PL , SWIFT: INGBPLPW, with a note: ENTRY FEE, NAME SURNAME, CLUB. APPLICATIONS: Applications containing the following information: NAME, FIRST NAME, CLUB, DATE OF BIRTH, AGE CATEGORY, runs AND TIMES (annex) should be sent by SPLASH system to the organizer s address or by e- mail: no later than 05/06/2015 (Friday). Persons wishing to receive VAT invoice are requested to report it at the latest by 05/06/2015. TECHNICAL REGULATIONS: The competition will be held in time series. No result on an entry card will result in putting the contestant in the weakest of the series. The event will be held according to the provisions of Polish Swimming Association. AWARDS:

2 1. Each participant will receive an attractive gift within the value of the entry fee places will be awarded with medals and diplomas. 3. Furthermore, there will be the prize awarded to the most valuable long-distance swimmer in the category of women and men. 4. The most valuable result according to the table Masters on the distance 1,500 m freestyle - women and 1,500 meters freestyle - men prizes will be awarded. 5. The organizer is planning to give additional prizes donated by numerous sponsors. for its receiving Participation in all of the reported competitions is obligatory. 6. A person who misses the earlier reported competition, does not receive any additional prizes, unless his indisposition is confirmed by a doctor of the competition. ALIMENTATION: The SDS hotel has a catering bar with the full board. The prices are as follows : breakfast 16 zł, dinner 20 zł, supper 18 zł. The organizer reserves the right to join women and men in all long distance races (400, 800, 1,500 m freestyle, 400 m medley stroke and 200m butterfly stroke). The start of each competition must be confirmed (by schedule) by a volunteer who will be in the office of the competition. 2. In case of a large number of applications over distances in 800 and 1,500 m freestyle organizer reserves the right to adjust the start with two runners on a lane. 3. Please, get familiar with a map to move around the site, to avoid misunderstandings with volunteers. Please follow established rules. 4. We invite you to familiarize yourself with a map showing the way from the railway station, bus station and the hotels of the pool. 5. During the event two lanes in the pool 25 m, located in the same complex, will be available in order to warmup.6. Awards ceremony in individual events will start according to the scheme. 7. Medals, diplomas and prizes will be handed by personalities from the world of sport and politics of Szczecin. Therefore players are asked for the appropriate clothing. VERY IMPORTANT: 1. A player who fails to appear at any competition to which he is reported, will be not taken into account when handing out additional prizes donated by sponsors (also applies to foreign competitors). 2. Start at 800 and 1,500 meters freestyle men and women must be confirmed no later than the end of the warmup on Friday (14:40). After that date the confirmation will not be accepted. 3. Start at the 400 m freestyle men and women must be confirmed no later than the end of the warm-up on Saturday (14:50). After that date the confirmation will not be accepted. 4. Start at the 400 m medley women and men must be confirmed no later than the end of the warm-up on Sunday (08:50). After that date the confirmation will not be accepted. COMPETITION PROGRAM
