Parent/Guardian Signature Date Each adult family member is required to serve actively on one or more of the following committees, space is limited and committees will be filled on a first come first service basis. The success of our swim club depends on parent participation. Your child will not be allowed to swim if you do not fulfill your obligations. There are a variety of jobs that can accommodate everyone s life circumstances including working parents, assignments that do not require physical labor or time away from youngsters. If you do not select a committee, you will be assigned to one. Please feel free to sign up for more than one. Swim Meet Operations Meet Director: Gene Reynolds - Coordinate officials, statistician, scorers, timing console operators, announcer, pre-meet timing system set up/post-meet take down. Timing Committee - Chairperson: Christy Gold - Help to coordinate schedule of timers for invitationals, home and away dual meets, and sending out notifications indicating timing assignments. Car Wash Fundraiser Chairperson: Christa Cicchetti - Date TBD- Help organize event (buy supplies, supervise event, etc.) Packanack Day Chairperson: Char-Len Gorski Coordinate and assist in selling snacks and beverages during Packanack Day, bracelets and ices in the evening. Pasta dinner Chairperson(s): Lori Derstine and Chrissy Sternberg- Help organize event (organize cooks, supplies, shopping for event, etc.) PAGE 1 v3.2

2 Awards Dinner Chairperson: Meghan Glenn- Help organize event (order food, supplies, order end of the year gifts, etc.) Swim Team DVD Chairperson: Manny Liwanag - Photograph and video swimmers during events. Create a DVD keepsake to be distributed during the Awards Dinner. Snack Stand - Chairperson(s): Tara Houser and Aimee Bongiovanni- Purchase food for snack stand, set up stack stand for home meets, sell snacks during meets Ad Book/Program for Packanack Odd Age Invitational Chairperson: Vacant- Collect advertising sponsor forms from each swimmer, solicit ads from local businesses, coordinate the lay-out of the ad book and get copied for the meet. Facilities Chairperson(s): Doug Jackson and John Farina- Pre-season set up/end-of--season take down, setup/cleanup of dual meets and home invitational, general maintenance and repair. Annual Packanack Lake Clean Up Day Chairperson: Rose Farina Assist with cleanup on May 5 9:00 am of the swim team area on the West beach. Coffee and doughnuts will be available at the Clubhouse. Rock The Beach Chairperson: Barb Corbett Assist with set up (June 2 nd ) and/or cleanup (June 3 rd ) at the East Beach 2018 Fund Raising Obligations Each family is expected to buy or sell a total of $50 in sponsored ads, $50 in boosters or a combination totaling $50 for the Packanack Odd Age Invitational Ad Book/Program. Each family is expected to volunteer at the Packanack Odd Age Invitational on July 14th. This is our biggest event of the season. Each family is also asked to provide dessert for both the Packanack Odd Age Invitational and Awards Dinner Timing Obligations As a member of the Packanack Swim Team, each family that is not performing another job at our swim meets is expected to time for at least 3 Dual Meets. If your child is participating in an Invitational, which is different from a dual meet, you will need to time a portion of an event. The Swim Dual Meets and Invitationals cannot function without sufficient timers, so all hands-on deck is needed. A Dual Meet schedule and sign-up sheet will be at Registration to help us formulate a timing schedule, so please sign up based upon your availability. PAGE 2 v3.2
