1 Booking Form April 2018 It is important that you complete and return this form as soon as possible to secure a place. This can be done either via post to ASGMA, 1114 Chester Road, Stretford M32 0HL, in person or by to admin@asgma.org.uk. completing this form Member/Carer Phone Number Please note: Once you have booked a child onto an activity, you will be charged for this place unless we receive notice of cancellation at least 2 weeks prior to the date of the activity. This can be done either by calling or ing admin@asgma.org.uk. Once you have booked onto an activity, we will only contact you if the activity is oversubscribed and we cannot offer you a place.
