3 Dr. Tabassum Ali,DC,CCSP FV member Sick Scapula Shoulder pain can be attributed to a variety of conditions; one that is overlooked is SICK scapula. - Scapular malposition - Inferior medial border prominence - Coracoid pain and malposition - dyskinesis of scapular motion Anatomically the scapula, commonly known as the shoulder blade, is found on the posterior side of the rib cage and the only osseous structure it is attached to is the clavicle, the collar bone, at the acromioclavicular joint. On the most lateral part of the scapula there is a socket that allows the humerus to sit in and creates the glenohumeral joint, shoulder joint. The rotator cuff muscles and a few other muscles are the associated soft tissue involved with stabilization and motion of the shoulder. The shoulder muscles include: supraspinatus (predominate shoulder stabilizer and adductor), infraspinatus (external rotation), teres minor (external rotation), and subscapularis (abduction), trapezius (retraction, elevation, and depression), rhomboids (scapula retraction), levator scapula (elevation of scapula), serratus anterior (protraction and scapula stabilization), and pectoralis major (adduction of humerus). The scapula and the rotator cuff muscles are what make the shoulder a functional joint- they must all work in conjunction biomechanically. Patients who have difficulty moving their shoulder through range of motion or have pain upon motion can possibly have a restricted shoulder joint, but the main cause is due to poor biomechanics from a previous overuse injury and fatiguing of the soft tissue. Although the supraspinatus is the first muscle susceptible to a strain within the shoulder and can be painful, a disorganization in motion of the serratus anterior, trapezius, and levator scapula can also be painful and restrictive in motion. Once the scapula is malpositioned, the pectoralis major becomes shortened and tight in efforts of preventing dysfunctional movement. Shoulder strains are usually caused by intense, repetitive movements applying too much pressure to the muscles and joints. Softball, baseball, swimming, and tennis are all sports with repetitive overhead motions of the arm and can lead to injury. Symptoms of a SICK scapula: Pain ranging from slight to severe Restricted motion Scapular protrustion Dropped shoulder on affected side When observing the shoulder for motion, the patient may experience pain through the ranges of motion, which is a good indicator of a SICK scapula and there is some sort of restriction in the soft tissue or in the joint. To fully diagnose dyskinesis of a SICK scapula a functional assessment should be conducted. If trauma was indicated, imaging should also be considered in diagnosis to rule out any osseous fracture, joint degeneration, impingement from an osseous structure, or arthritis in the joint. If a strain is concluded another source of imaging would be a musculoskeletal ultrasound to further evaluate the scar tissue in the muscles preventing full range of motion in addition to pain upon exertion. Possible treatments for a SICK scapula: Active Release Technique, Graston, Eccentric rehabilitative exercises, Stretching, RICE (rest, ice, compress, elevate), Surgery based on the severity of the tear to reattach the muscle tendon, Steroid injections, Non- Steroidal Anti- Inflammatory medications

4 Addressing the scar tissue and rehabilitation of a SICK scapula is very important. Skipping out on stretching can lead to further advanced conditions. A misconception that many athletes have is, a tight muscle indicates a strong muscle- this is incorrect! Although a tight muscle can be strong, it is more common that the muscle is weak and spasming in hopes of remaining stable, but probably on the verge of an injury. Warm up of the shoulder muscles is important before beginning any strengthening protocol and can be done with stretching. An example of a shoulder pectoralis major stretch would be placing the forearm flat on a wall or doorway with elbow bent and stepping forward, inducing a stretch on the pectoralis major. Eccentric exercises to strengthen the muscles associated with a SICK scapula and stimulate healing within the first 6 weeks of injury will produce the best prognosis. Approximately 8-12 weeks of treatment will be needed, depending on the severity of the injury, to decrease the pain and strengthen the muscles. At SoCal Sport and Spine, we approach injuries in a conservative manner that will give the patient the most weighted benefits. Our providers are board certified chiropractors and certified in full body Active Release Technique. We examine the patient and determine the most efficient and quickest treatment plan for them. Our goal is not only to get the athlete back out on the field doing what they love to do, but to also enhance their athletic performance. We address the scar tissue that is built up from micro tears in the tissue due to overuse or previous injuries and develop a program to strengthen and train the muscles to be used functionally. Most all of our patients find results within a few treatments with decreased pain, and increased range of motion, strength, and balance. We then move into the second phase of treatment with strength and conditioning which is the most important aspect of the treatment, to prevent future injuries. 6 Run Free - The True Story of Caballo Blanco will premiere in Brea, CA, on Tuesday, September 1 at 7 p.m. at the Curtis Theatre at the Brea Civic and Cultural Center, 1 Civic Center Circle, in Brea. Grab some friends, carpool and make a movie night of it! The feature-length documentary is about ultra-running legend Micah True. Better known as Caballo Blanco the White Horse True, was the focal character of Christopher McDougall s 2009 best-selling book Born to Run: A Hidden Tribe, Superathletes and the Greatest Race the World Has Never Seen about the Tarahumara Indians of northern Mexico. The one-night-only event is sponsored by A Snail's Pace running store and club. Tickets are $12 in advance at or $15 at the door the night of the show. ***TICKETS WILL NOT BE SOLD AT THE STORES BUT AT THE WEBSITE PROVIDED ABOVE..*** A percentage of the film s profits, including from DVD sales, will go to benefit Norawas de Rarámuri (Friends of the Running People), the non-profit agency founded by Micah True to preserve traditional Tarahumara culture. Norawas de Rarámuri works to provide maize, non-gmo seed corn, and cash awards for participating Tarahumara runners, both men and women alike. On this way, the organization offers Tarahumara families nutrition during drought and support for a tradition of small farms necessary to both physical and cultural survival.

7 Bob Morris- Run Club Director/MV Assistant Manager Where are you from originally and when did you move to So Cal? Born in Pontiac, MI, but raised in So Cal. from second grade on. After college worked in Japan, Las Vegas & New York, then back to OC in What got you started on running, when, where, etc.? I started running in jr. high school in La Puente, then moved to Big Bear where I held the school mile record till Ryan Hall obliterated it. Received a track/xc scholarship at Azusa Pacific. What has been your favorite race and why? I ve run ten marathons but prefer the shorter races. Two favorites I have fond memories of are: 1) HB 4 th of July 5k, in 2000 where I won the residents race (16:43) at 48 years old. 2) World Masters Athletics 1500 meters on a track in Porto Alegre, Brazil, Was seeded 14 th out of 37 but made it to the finals where I finished 6 th in 4:51 which was the top U.S. mark for the age group that year. What is the coolest place you have gone for a run? On a trail outside of Auckland, NZ where my sister Pam lives. Lush vegetation. What are you training for now and how is it going? USATF Masters National 8k Championships At the Brea 8k next February. Trying to re- establish my distance base right now. Do you have any races that interest you? I would like to do more Masters track meets and runcation marathons to exciting locals with my wife. What are your current running goals? To improve upon my & 1500 meter times and get back into the low 19 s in the 5k. What is your best running tip? Don t get in a rut, don t underestimate yourself, and keepit fun. Why do you run? It s something I ve always enjoyed doing.

8 Any other hobbies or interests? Movies, travel, and salsa dancing. What do you like most about working at A Snail s Pace? The best co- workers I ve ever worked with. Also enjoy the camaraderie within the Club too. 11 The Ridgecrest 50k Ultra Marathon trail race had become an obsession. It was touted by fellow Brea Snails as a cakewalk, the easiest 50k in California, and perfect venue from which to start a burgeoning trail running career. The contoursof the course were gradual, well-placed aid stations would be equipped with food and other goodies to fuel my motor, and low temps would enable my body to stay cool and well hydrated. Besides, trail races were supposed to be more casual and easy going than street races, which hopefully meant more fun and less work if stories that I had heard were true. Moreover, I figured that the bucolic grandeur of the high desert would be an ideal backdrop for my first trail race. What could possibly go wrong in such an ideal spot? However, I was the intrepid runner who would soon suffer the imposed hardships of running a race unprepared and with unrealistic expectations. I was more than ready on the morning of the race, filled with rosy expectations and naïve bravado that would come crashing down on me sooner or later. The race was set to start at 7 a.m. at the Cerro Coso Community College gym above the town of Ridgecrest. It was a chilly 28 degrees as a little over 250 runners stood poised and ready for the starting gun to go off. While everyone waited in anxious anticipation, a stunning sunrise blanketed the mighty Sierra Nevada Mountains to the distant west with a surrealistic pink, orange and purple veneer. My expectations were high that this race would be very doable. What could possibly go wrong? The race started, and by mile five I was already lagging behind the main cohort. By mile fifteen I was exhausted and thinking about calling it quits. Nobody told me that there would be a downside to running an ultra trail race. Nobody prepared me for the suffering and hardship. Moreover, poor traction on the sandy trails made advancing forward frustrating. My strategy of walking up the hills and run down them was still too difficult. After mile twenty I was alone and praying for the experience to finally be over but over eleven arduous miles remained to traverse, and I was slowing down as my legs got heavier after each mile. Moreover, my running mojo was depleted along with any hope of finishing my running ordeal within a reasonable time. Lack of training and scant knowledge of trail running had done me in. I was never given the unvarnished truth about the tribulations of trail running and I had come inexperienced and unprepared to face the hardships. My expectation of a cakewalk was rapidly morphing into an insufferable learning experience. As the tiny silhouettes of fellow racers faded into the landscape ahead, I was suddenly cognizant of my vulnerability and inexperience. My plan was to mitigate the pain by focusing my mind on the surrounding panorama of giant rock outcroppings, cacti, and layers upon layers of mountains and hills stretching for hundreds of miles in all directions. This run might have been a Zen experience if I were in better shape and better prepared. Moreover, my surroundings were void of any overt signs of animal life for the first 4 hours of the run with only a peaceful, albeit, eerie stillness with no wind or distracting sounds of nature. Just complete silence juxtaposed against the sounds of my own heavy breathing and rapid heart rate. Little did I know that the peaceful serenity of nature would come to an abrupt end. The trail was poorly marked, and my own nearsightedness made some of the well-marked sections difficult to read. Several of the chalk directional arrows drawn across the trails had been obscured or obliterated by foot traffic from runners ahead of me. Being both tired and lost is an eerie feeling if you re in the desert alone. At one junction I was forced to ponder which fork in the trail to take, confident of a solid fifty percent chance of getting it right. My confidence gradually faded as I fell further and further behindthe madding crowd.

12 Suddenly, I glanced at a foreboding shadow hovering directly above. Was it an airplane? The same shadow crossed overhead a second, and then a third time as if it were tracking my progress, or lack thereof. After a minute or two I gazed up into the sky in an attempt to identify the apparition that had taken a liking to me. I saw the outline of a disgustingly ugly turkey vulture with a wingspan of at least five feet. It hovered directly overhead for over an hour with a continual and purposeful vigilance. Was it a harbinger of disaster or just a curious bird with nothing better to do? What was it thinking? Or was I just too sensitive and overly concerned about something as trivial as a hungry vulture awaiting my demise? Whatever the motive, it was dead set on not leaving my side anytime soon. It finally departed upon my arrival at the next aid station. But for over an hour it had become my spirit animal or companion to keep me company when everything else had seemingly forsaken me. I did choose the right fork in the road, however, and survived to finish the race and write this story. At the finish line the lactic acid in my body went crazy, causing symptoms of rigamortis to permeate almost every muscle of my body. My movements became so contorted that they mimicked those of Dr. Frankenstein s infamous monster. I hobbled down the gym hallway to the hot shower and change of clothing awaiting me. I would never again overestimate my abilities or underestimate a race Peters Canyon Petition Finding a Better Path for the Bikeway Extension Below, you will find an organized and sensible alternative to what is being proposed for Peter s Canyon. I ask you to read the information and then sign the petition to stop the nonsense of putting an asphalt bikeway through our canyon. Happy DIRT trails, Bobby Porter One of the groups that FCA sponsors, Friends of Peters Canyon, has asked for our support on an important issue they are working on. Peters Canyon Regional Park is a tremendous natural resource for all of us, a little piece of wilderness here in the middle of Orange County. The County is currently engaged in creating a General Development Plan for the Park, and FPC was formed as a way for Park users to work together to have a voice in this effort. FPC members see the priorities as being the restoration, preservation, and conservation of the Park. They treasure the natural environment, and the wildlife. Maybe you are already a Park visitor. If so, you have a lot of company. On Saturdays, Sundays, and holidays over 800 people a day come to the Park to walk, hike, ride mountain bikes, and ride horses. The County s proposed General Development Plan includes a paved, Class 1 Bikeway, which would be at least 12 wide, running from the top of the Park to the bottom. Friends of Peters Canyon supports an extension of the current Bikeway, but they believe the proposed routing is a mistake, and one that would seriously degrade the Park. What they want is a better path -- re-routing the Bikeway extension along side Jamboree (off street) where part of the extension already exists. Your help, please... And here s where you come in. The pro-bike lobby has put up a petition in favor of going ahead with the Bikeway through Peters Canyon. FPC has mounted a counter-petition, looking to preserve the natural environment of the Park while building a Bikeway extension that would still connect the two ends along side Jamboree. They believe that s a win-win solution for everyone, and it may be no more costly than building the Bikeway through the Park. OC Parks agrees that the alternative routing could be a good solution, and they re laying out the details of that approach now. We would like you to review FPC s petition, and sign it if you agree that it makes sense. Even if you re not currently a Park user, preserving a resource like this is everyone s business.
