4 Dr. Tabassum Ali, DC,CCSP FV member Flat Feet A flat foot, also known as pes planus, is found on the medial aspect of the foot when the arch fails to develop and touches the floor. The condition normally presents in infants and toddlers; it only becomes an issue in adulthood if the arch does not eventually develop. Most adults with flat feet will not have any complaints, but to experience pain or achy feet is not unusual. Flat feet is associated with the following symptoms: Pain on the medial aspect of the foot, in the arch Achy and tired feet at the end of a day or after long periods of standing on feet Tight calves (Achilles Tendon) Numbness from compression of the tibial nerve Arthritis of the lateral ankle from compression Associated knee pain The pain with flat feet can be attributed to weakness in the tendons leading to failure of arch formation. When palpating the foot, you may also notice swelling on the medial ankle due to the constant compression. Flat feet are often differentially diagnosed with tarsal tunnel syndrome and plantar fasciitis, or heel spurs, because of similar symptoms. To confirm that the flat feet are not flexible, have the patient stand on their toes and observe for the formation of an arch. If an arch is formed, that is indicative that the flat feet are flexible and there is no major underlying condition. If the arch does not appear when standing on the toes, further imaging may be necessary to confirm there is no other underlying condition causing the deformation of the foot such as tarsal coalition. With plantar fasciitis, the patient may be able to pin point the sharp pain on the heel of the foot and will be exacerbated by putting any weight on the foot or excessive duration of exercise. A good way to diagnose tarsal tunnel syndrome is, Tinel s test; you can tap the tarsal tunnel where the nerve passes through, and if the symptoms are reproduced then it is highly probable. Treatment of flat feet can vary depending on how chronic the condition is. In rare cases surgery to fuse the tarsals can be performed to decrease the pressure on the arch and vasculature. If you're considering foot surgery, discuss the benefits and risks with your doctor. Other invasive options include surgery of the tendons to repair the area. Conservative treatment is preferred, and can have lasting results if managed properly. Icing the area for no more than 20 minutes every 2 hours can help with the inflammation. Some sources recommend an orthotic to provide support for the medial arch and to relieve pressure on the nerve. The most effective form of treatment would be to address the scar tissue that has built up over time from overuse and adjust any subluxations of the foot and ankle. Once the symptoms have subsided and the scar tissue has healed, the next phase of treatment would be to keep stretching the muscles of the plantar fascia and medial arch of the foot and to strengthen them to prevent future injury. Warm up of the muscles is important and can be done with simple arch stretching and continued on with the strengthening program. With flat feet, it is common to find tight calves. A good stretch for the calves is downward facing dog yoga pose. This pose serves dual purpose for flat feet, in that it stretches the tight calves and is exercising the tendons with weight on the balls of the feet. Exercises to strengthen the tendons associated with flat feet will include proprioceptive activity with one-legged stances and diagonal reaches. Approximately 6-8 weeks of treatment will be needed to decrease the pain and strengthen the muscles.

Our goal is not only to get the athlete back out on the field doing what they love to do, but to also enhance their athletic performance. We address the scar tissue that is built up from micro tears in the tissue due to overuse or previous injuries and develop a program to strengthen and train the muscles to be used functionally. Most all of our patients find results within a few treatments with decreased pain, and increased range of motion, strength, and balance. We then move into the second phase of treatment with strength and training which is the most important aspect of the treatment, to prevent future injuries. What got you started on running, when, where, etc.? A stray dog chased me when I was a kid and I thought it was fun. What has been your favorite race and why? Trying to race from the couch to the microwave before it beeps. What is the coolest place you have gone for a run? With the penguins. At the zoo. Running from Security. What are you training for now and how is it going? I m training for Badwater by only drinking bad water. It s not going well. Do you have any races that interest you? 24 hour death races involving chopping wood What are your current running goals? 2 hour and 30 minute ascent and descent of the Matterhorn. What is your best running tip? Left foot, then right foot, then repeat Why do you run? To further develop my animal magnetism. Any other hobbies or interests? Whittling. While I run. Also Knitting. 9 MY REVEL CANYON MILE JOURNEY BY JIMMY AN DRES If you know me, you know that my famous last words afterfinishing a marathon are I ll never run anothermarathon everagain. I ve said those words many, many times and here Iam just having finished my 27th marathon to date, so you can t blame when no one believes me anymore when Isay those words- even when I say it with great conviction. Well, I thought I was reallydone running marathons, then came the Revel Canyon City Marathon, promoted as the scenic and downhill race from above 5000 feet of elevation. Can you imagine the thought running in my mind? Yes! Running downhill! Iwill definitely PRbecause downhill is my strength! I trained veryhard forthis race. Idid more running than my usualtraining, Iran the course twice with lengths of twenty and twenty-two miles with the help of ourrunning friends(michael, Gilbert, Lucy and Oscar- thanks again!). I was ready and prepared to conquerthe RevelCanyon. It was 3:30am when we all agreed to meet at the Snail s Pace back parking lot. Iwas there at 3:20am and thinking Iwould be the first one there but as soon as Ipulled up in the parking lot, Isaw two guys standing in the middle of the parking lot talking, and they seem to be much more excited than me. Kevin and Jay were all smiles when they greeted me at the parking lot. Then Catherine and Michaelshowed up and finally Mina who volunteered to drive all of us to the race. We arrived at the race parking lot around 4:00am and we immediately made a bee-line to the shuttle and off we were to the start line. The drive uphill on Hwy 39 was pretty rough- some passengers started to feelnauseated. Then we arrived at the starting line at about 5am. We had two hours to kill. As soon as we got there, we all had one goal- get in line to the restroom! Ilaughed because one runnertold me that he was just going to sit on the toilet fortwo hours until the race started, because as we all know, you just go back and forth before a race starts. Idid not see him foroveran hour. After the pit stop, we then allgathered by the fireplace to stay warm, taking selfies etc. then someone said there are free bananas inside the store we can go inside and grab some. Michaelwent and came back with a couple and shared them with Kevin. We asked him, are

10 they free? He said Oh yes, they re free. Itold Jimmy J. that we needed to get some as well. We went inside and saw the bananas, butit didn t look like they were free. Jimmy J. grabbed two and gave me one. We didn t have anycash so we did not bother asking if they were free. Did we just steal bananas, I thought to myself? When we did our training, the owner of Crystal Lake Café told us that he did not make any money from the runners! Do Ineed to explain why? Then the race started. My plan has always been not hold back on the downhill, and let gravity pull me down with less effort. Idid that forthirteen miles, it was awesome, then the uphill started at mile fourteen to seventeen and a half. Oh that was tough. Istarted to run slowerbut was still on pace to finish at 3:30. At mile eighteen, Istarted to feeltired. Ihave been holding a whole bagel on my hand all this time as I always planned to have something to fill my stomach and keep my energy level. By this point, my mouth was so dry, but Itook a bite of that bagel anyway. Ichewed and chewed, and chewed more on that bagel butwas having a hard time to get it down because my mouth was so dry, and I needed waterto wash it down. Finally Isaw a waterstation (they seemed to be so few and farin between). The kid handed me with a cup of water. Inoticed the cup was barely filled with water, so I said to her Why is the cup only filled with little water? She just laughed and said I know! Huh? Mile twenty-one is anotherdownhill. Normally at any race, I get excited when I get to a downhill finish but forthis one, it was the opposite, Ihated the downhill because the more Irun downhill, the worst my quads felt. Imade it to twenty-fourmiles, and it was mind over matterby this point. My body just enough, Iwanted to stop and walk but Ikept looking at my Garmin and thought that maybe Icould still qualify forboston. When Ireached mile twenty-five, Ihad a renewed strength. There were uphill but I felt much betterand finally at the last turn, Iheard the crowd cheering. All Iwas thinking was I am so glad it s over! Ilooked at the finish line clock and it read 3:39:36. Wow! Did I just qualify forboston by about24 seconds? Hey, no matterhow you look at it, I BQ d! Thanks to allmy Snails running buddies. Now that it s over, Ican say it was a great race but let me tell you I LL NEVER RUN ANOTHER MARATHON EVER AGAIN! Jimmy Andres is a Brea Snail and a former president of the club. He ll likely tell you that he ll never run another marathon again in April 2017 after he completes the Boston Marathon.
