3 Dr. Tabassum Ali, DC,CCSP FV member Infraspinatus An infraspinatus tear will be found with pain in the shoulder joint. Anatomically the infraspinatus is found on the back, attaching inferiorly along the spine of the shoulder blade, also known as the scapula, out to the humeral head where it turns into a tendon. Just below this tendon, the infraspinatus bursa is found. The infraspinatus is part of the rotator cuff muscle group of the shoulders. Other muscles that make up the rotator cuff: Supraspinatus Teres Minor Subscapularis The main function of the infraspinatus is external rotation and stabilization of the shoulder joint along with elevation and depression of the shoulder. The first muscle susceptible to a tear within the rotator cuff muscles is the supraspinatus and following is the infraspinatus. An infraspinatus tear is generally due to an overuse injury that has been putting stress on the shoulder joint and leading to instability and joint laxity. Baseball players and volleyball players are known to have this injury due to the constant overheard motion. Other repetitive stress motion injuries of the infraspinatus can be from driving with the arm held stretched out on top of the wheel or reaching back persistently with a computer mouse. Symptoms of an infraspinatus tear: Pain radiating into the neck and upper back areas and down into biceps Inability to raise the arm above head because of stiffness and feeling numb Weakness in the shoulder Discomfort with sleep when rolling onto side When observing the shoulder for motion, the patient may experience pain through the range of motion, also known as the arc of pain, which is a positive indicator of a tear. Muscle weakness will come secondary to shoulder pain and you may also observe abnormal motion of the scapula. If a tear is concluded, scar tissue will be built up along the tendon and around the infraspinatus bursa causing a catching or grinding motion of the shoulder. The degree of a rotator cuff tear is difficult to determine without imaging. Some patients may present will decreased range of motion and in a great amount of pain, and some patients may have no pain and have limited range of motion. The first source of imaging is X-ray, however it will not be able to show any soft tissue damage but it will help rule out other conditions. An x-ray will tell you if there is any joint degeneration, impingement from an osseous structure, or arthritis in the joint. Possible treatments for an infraspinatus tear: RICE (rest, ice, compress, elevate) Surgery based on the severity of the tear to reattach the muscle tendon Steroid injections Non-Steroidal Anti-Inflammatory medications Addressing the scar tissue formation in the shoulder joint Stretching Rehabilitative strengthening exercises

4 Addressing the scar tissue and rehabilitation of the shoulder is very important, avoidance of this can lead to further conditions such as frozen shoulder. Warm up of the muscles is important before beginning any strengthening protocol and can be done with stretching such as the doorway stretch or threading the needle yoga pose. Exercises to strengthen the muscles associated with an infraspinatus tear: Resistance band- external rotation motion Face pulls Approximately 6-8 weeks of treatment will be needed, depending on the severity of the injury, to decrease the pain and strengthen the muscles. At SoCal Sport & Spine, we approach injuries in a conservative manner that will give the patient the most weighted benefits. Our providers are board certified chiropractors and certified in full body Active Release Technique. We examine the patient and determine the most efficient and quickest treatment plan for them. Our goal is not only to get the athlete back out on the field doing what they love to do, but to also enhance their athletic performance. We address the scar tissue that is built up from micro tears in the tissue due to overuse or previous injuries and develop a program to strengthen and train the muscles to be used functionally. Most all of our patients find results within a few treatments with decreased pain, and increased range of motion, strength, and balance. We then move into the second phase of treatment with strength and conditioning which is the most important aspect of the treatment, to prevent future injuries. In summary, to address an infraspinatus tear: Address the scar tissue in primary involved muscle, and any associated muscle or bursa Warm up and stretch the shoulders Strengthen and condition the shoulder to prevent future injury (sport specific functional training) Check our website for more information and to book an appointment today at Works Cited Itoi, Eiji. 5 Jayden Earlywine - Fountain Valley. Sales Associate Where are you from originally and when did you move to So Cal? I am originally from Inland Empire in Corona, California and have lived there for 20 years before moving to Huntington Beach in What got you started on running, when, where, etc.? I started running cross country and track in high school. A good friend of mine recommended that I try it out to stay in shape for wrestling in the off season. Right away I fell in love with running and never looked back. What has been your favorite race and why? My favorite race was competing at the 2012 NCAA DIII Cross Country National Championships in Terre Haute, Indiana. The atmosphere there was unlike any other. It was a brand new experience not only for me, but for my teammates as well because it was the first time in school history to send a full men s team to nationals. What is the coolest place you have gone for a run? The coolest place I have gone for a run was a trail just off of Northern Arizona University s main campus in Flagstaff, Arizona. This was a really cool run because it was all brand new to me, and it was snowing! This at first the thought of running in the snow may not seem the most ideal for a Southern California native, but since it was so new and so unique, it is definitely one to remember. What are you training for now and how is it going? Currently I am training for the LA Rock n Roll Half Marathon coming up on October 25, Training has been going well and this will be my first half marathon so anything can happen. Do you have any races that interest you? Races that interest me are track races. I have always enjoyed the highly competitive atmosphere on the and really enjoy competing the 800 and 1500 meter distances. What are your current running goals? My current goals are to explore the endless possibilities running has to offer. I am really looking forward to new races. What is your best running tip? My best running tip is rest. Many people do not realize the importance of rest. Often times we do not listen to our bodies and when this happens, injuries are not too far behind. On days you feel great, go the extra mile, literally! On days you feel like you can t get out of bed, stay there, your body is trying to tell you something, so listen to it! Why do you run? Whether it is setting a new personal record or just an easy few miles, I run because I can. It is a feeling and accomplishment unlike any other. I always strive to better myself and with each step I know greatness is possible. Any other hobbies or interests? Outside of running I like to slackline, play disc golf, and ride dirt bikes. These are just a few of the many ways I have fun while remaining active. What do you like most about working at A Snail s Pace? I enjoy the atmosphere at A Snail s Pace. Both the employees and the customers are very active in the running community which makes things simple to start up conversations and talk about the thing we all love to do, running!

10 Chapman University 5k by Rick Green The 2015 Chapman University 5k On October 3, 2015 I ran the Chapman University 5k. I am both an alumnus and an adjunct professor at Chapman. Although this is not the race that motivated me to get off the couch, the 2011 running of this race was my first foot race ever. PR and Goal My 5k PR is 25:05. I set it on an untimed, unofficial course at the Neverland 5k at Disneyland in May. I ran the exact same time on Independence Day at the Surf City 5k in Huntington Beach. My goal today is to get rid of the number twenty-five from my PR time. I want a PR that starts with the number twenty-four. It s just six seconds. How hard can that be? Training wise I m currently between marathon cycles. I ran the Santa Rosa Marathon in late August. The training cycle for the Los Angeles Marathon starts October 12, In this lull, I m running between ten and twenty miles a week. It s just enough so I don t forget how. In an effort to heal from the ravages of my last marathon training cycle, I m also swimming more. The real focus of my brain is in this period my weight. I ve dropped 12 lbs in the last month. I m down to 219 lbs. I d like to toe the line at LA at 199 lbs. The Course The course remains unchanged from years past. The race starts at the Schmid Gate at Chapman University. It heads south, through the traffic circle of Old Towne Orange, through the neighborhood of Arts and Crafts style homes south of the university, and finishes in front of the Leatherby Library back on campus. 10

11 Chapman University 5k The First Mile I lined up near the starting line behind the really fast people, and the We Run OC kids, an after-school running club for middle and high schoolers. At 7:30 am, the horn went off and off we went. About three tenths of a mile into the course, I took a glance at my watch: 7:48 mm pace. Good. If I can hold this pace, the shiny new PR shall be mine. Seeds of doubt enter my mind as I notice that this early in the race, I m starting to huff and puff. This is mile one and this pace is real work. Passing the first mile marker, I eschew the water stop and hear the volunteer yell out the time: 8 minutes! Mile 1 7:54 The Second Mile The course now turns north and heads back towards the university. I find myself surrounded by Chapman students and the We Run kids. The difference between them and me is that they are twenty to thirty years younger than me, and this pace appears to be somewhat effortless to them. I feel like I m about to die. Half way through the second mile, I pass St. John s Lutheran School. They have an unofficial water stop set up along with a rock and roll band on the school s loading dock. I ran right down the middle of this water stop and wave. I ran right through the official water stop at mile 2 as well. As I ran through, a volunteer reading out times said something I found quite alarming: Sixteen minutes and thirty seconds! Mile 2 8:03 The Last Mile and a Tenth The last mile is something of a blur. I don t remember much. Just my brain whipping my body like a jockey whips a horse. The volunteer at the mile three sign said something. I m not quite sure what it was. I m running like a madman, trying to keep up with these tweens, teens, and twenty-something kids. I saw the time on the clock crossing the finish line, and felt somewhat dejected: 25:09. Mile 3 8:10 Chip time 25:01 So I ve got a shiny new PR that starts with the number twenty-five. In years past, I would just show up and lop minutes off my PR. Those days are gone. Now I gotta fight for every second. Going forward, I m going to keep living the calorically limited life and start training to run with SnailNation at the Los Angeles Marathon on Valentines Day. My next attempt to dump the number twenty-five is at the Carlsbad 5000 in April of
