3 2018 League Team Trials NB!! Trials are for BOYS and GIRLS from ages 7 to 18. Only members who have paid their 2018 registration fee will be allowed at trials. League team trials will be on the 16-January-2018 till the 27-January Every session relevant to your age group must be attended for the trials. This will be done as part of the normal training sessions and special sessions on a Saturday as follows: Trial Training on Mondays & Wednesdays th, 22 nd, 24 th January 2018: Ages 17 to 18 (Under 19) from 17h00-18h00 Trial Training on Tuesdays & Thursdays th, 18 th, 23 rd, 25 th January 2018: Ages 7 to 10 (Under 9 and Under 11) from 16h00-17h00 Ages (Under 13, Under 15 and Under 17) from 17h00-18h00 Trial Training on Saturdays 20 th, 27 th January 2018: Ages 7,8,9,10 (Under 9 and Under 11) 8am to 9am Ages 11,12 (Under 13) 9am to 10am Ages 13,14 (Under 15) 10am to 11am Ages 15,16 (Under 17) 11am to 12pm Ages 17, 18 (Under 19) 12pm to 13pm

4 Club Kit We dearly wanted to start this season with Adidas kit, but it is not going to be possible. The main factors involved in this outcome are the large numbers of players interested in the club (resulting in an increase in previous number estimates) and the short amount of time in which the kit needs to be ordered, manufactured, delivered and branded before matches start this season. Adidas, as with most professional brands, order their stock a season in advance (basically, the kit that the PSL teams will start their season off with in July 2018 was wrapped up and ordered between May and September of 2017). We would need to sit down with them in May 2018 to finalize kit for the 2019 season. In light of this, for the 2018 season, we will be using another global brand steeped in football tradition and on the cutting edge of football technology and fashion, KAPPA. Kit fitting for sizes will take place from the 16-January-2018 to the 27-January Once sizes, player name and number are confirmed then the kit will be sent for printing. Each player will then receive their individual branded kit latest middle February Rules for selecting name and numbers to print on kit will be discussed with the member by the relevant coach assigned to the age group. The coach will abide by Club rules in determining numbers and resolving conflicts. Each member will receive one club kit (Shirt, Short, Socks) as part of their registration fee. A new kit will be given each year as part of the yearly registration fee. The kit received is the kit that players will use when they play matches and attend training. Additional kits are available for purchase at R700 per kit if the player wants additional training kits.

5 Corporate Social Investment Club Football at Midstream functions as a Private Football Club and Private Company that has an agreement to use an allocated area of Land owned by Bondev, to run the Football Club. We do not own any facility built on the allocated area of land or the land, it will be owned by Bondev. Bondev has agreed to allow the club to initiate a development project for the community and build facilities that would benefit the community. These facilities will be managed and used by Club Football at Midstream. The aim is to build facilities through Corporate Social Investment. A Non Profit Organisation has been setup with the necessary credentials to administer and fund this project. We will be approaching private companies for Corporate Social Investment funds and sponsorships to build the community facilities. CSI encompasses projects that are external to the normal business activities of a company and not directly for purposes of increasing company profit. These projects have a strong developmental approach and utilise company resources to benefit and uplift communities and are not primarily driven as marketing initiatives. 6 Fees There is a choice of one of the following or both options can be taken if required: 1. Club Football Training Fee 2. Extra Skills Training Fee FEES Yearly Registration Fee (Payable up front and applicable to both options below) R700 Once Off Per Year Includes 1 Training Kit: 1 x Shirt, 1 x Short, 1 x Pair Socks Monthly Club Training Fee (NB!! Payable at the beginning of each month in advance), One month notice to leave this option and club. Registration Fee payable again if joining again in same year. R700 Per Month x10 months (No payment for December and January) Includes Club Training Twice a Week and Matches on Saturdays. (Pro-rata fee will be calculated if membership doesn t start from the beginning of the month) Extra Skills Training Fee (NB!! Payable at the beginning of each month in advance, one month notice to leave this option. Can join this option again at a later stage within same year without paying registration fee again) R450 Per Month x10 months (No payment for December and January) Includes Extra Skills Training once a week (Only Wednesdays) (Pro-rata fee will be calculated if membership doesn t start from the beginning of the month)

7 Location From Centurion: N1 south, take Brakfontein rd / R101 off-ramp, at T junction left into Brakfontein rd towards Midstream, drive 2.3 km along Brakfontein rd. From Johannesburg: N1 north, take Old Jhb rd R101 South off-ramp, turn left into old joburg road, back over the N1, at the robot turn left into Brakfontein rd, drive 2.5 km along Brakfontein rd. At intersection of Brakfontein rd and Midstream drive go straight. Enter security entrance in Brakforntein rd and continue straight past 3 traffic circles, at the fourth traffic circle turn right towards midlands estate into Midway Blvd, go past Midstream on right, at the K220 (Meadows Drive) intersection go straight across, at the traffic circle just before the midlands security gate entrance turn left into Mountain Quray St, continue straight past the offices on left and the NG church on left, go through the security gate and continue straight on the ground road. Take the first road to the right and continue till you see an open area for parking next to the soccer field.

TERMS, CONDITIONS & INDEMNITY Please note that our terms and conditions ensure a safe and pleasurable learning environment and experience for all players and parents! -No credit or reduction in fees will be granted for non attendance due to illness or for any other reason -Training sessions missed or cancelled by the parent, guardian, responsible person or player will not be made up or credited -Training sessions cancelled by the Club Midstream) will be made up -Training sessions to run on time at the agreed times set. If a player is late for a training session, it can unfortunately not be extended beyond the specified time -Once a session has begun no player may enter the field before being invited in by the Coach -Players may only leave the area when dismissed by the Coach, should you require to leave early please notify the Coach in advance -No eating during a lesson, refreshment breaks will be provided -The Club reserves the right of admission - Coaches are employed to teach and not administer discipline. I have read and understand the terms and conditions and agree to abide by them. I further agree to the condition that, while every precaution will be taken for the safety and welfare of my child and for the care of his/her possessions, I will hold blameless and indemnify all persons, the Club Midstream) and all other organisations associated with the activity, should any prejudice, loss, damage, illness or injury occur to my child during the above activity. This includes an indemnity against recovery of costs resulting from damage, loss and/or medical conditions or hospitalisation, unless such loss is caused by the negligence, willfulness or deliberate act of the Club Midstream) or one or more of its employees.
