1 ADDENDUM #003 Solicitation Number: Solicitation Title: Opening Date/Time: Barber Supplies November 23, 2016 at 2:00 p.m., Eastern Time (ET) Addendum Number: 003 Failure to file a protest within the time prescribed in Section (3), Florida Statutes, or failure to post the bond or other security required by law within the time allowed for filing a bond shall constitute a waiver of proceedings under Chapter 120, Florida Statutes. Please be advised that the changes below are applicable to the original specifications of the above referenced solicitation. Added or new language to the ITB is highlighted in yellow, while deleted language has been struck. Change No. 1: The Department s written responses to written inquiries and approved equivalents. Question #1 Can items on a Purchase Order that are temporarily out of stock and or on back order be shipped at separate times from the items that are available for immediate shipping? Answer #1 Yes, however, additional shipping charges shall not be charged. Question #2 Are Vendor Contractor permitted to have any or up to all supplies on a Purchase Order drop shipped to the Department Buyer directly from a supplier or manufacturer the Vendor Contractor purchases the items from? Answer #2 Yes, but all orders shall reference the appropriate purchase order (PO) number and Department contact. Question #3 If partial or entire Purchase Orders are permitted to be drop shipped is the drop shipper (not the freight carrier under the FOB terms of the contract) considered a subcontractor? Answer #3 Yes, per Section 4.3 of the ITB, the Bidder may, with prior consent of the Department, enter into written subcontracts for the delivery or performance of services. Question #4 If a drop shipper is considered a subcontractor, what is the scope of requirements for the Contractor to remain in compliance with the awarded contract? Answer #4 The successful Contractor will remain ultimately responsible for fulfilling all requirements of the Contract, as specified in the ITB. Please see Section 4.3 of the ITB. Question #5 Are costs associated with fulfilling the purchase order, such as shipping, added to each unit price in a BID and therefore included in the grand total? Answer #5 Bids must be firm prices, and where products are included, shall include all packaging, handling, shipping and delivery charges, and environmental & fuel service fees, per Section 3.9 of the ITB. 2 Question #6 If additional costs, such as shipping, outside of the price for each unit on a BID are not included in the unit price, where should those costs be reflected on a BID? Answer #6 Please see Answer #5. Question #7 What is the definition or meaning of the term "extended price" on the BID Price Pages 21-29? Answer #7 The term "" has been change to Total, and should be the Unit Price multiplied by the. Please see the Revised Attachment I, Price Page, included in this Addendum. Question #8 Is it possible to get the previous bid tabulation or current contract pricing for the Barber Shop Supplies on? Answer #8 This is the first time the Department has issued a formal competitive solicitation for barber supplies, so a previous bid tabulation is not available. Question #9 What is the agency solution for an iem that becomes discontinued and is no longer available from the manufacturer, or other third party sources during the term of the first, second or third year of the contract and 3 additional option years? Answer #9 Please review Section 1.7 of the ITB. Question #10 Group III, Item 35 Starflite 939 Barber comb, annual quantity 50, what is the unit of issue? Does the customer request 1 comb, 50 each or 12 comb per on box, with 50 boxes (600 comb)? Answer #10 This item has been updated to specify per dozen as the unit of measure. Please see the Revised Attachment I, Price Page, included in this Addendum. Question #11 Group II, Item 60 Andis T-Edjar Blade screw (requires 2 each), 20, what is the unit of issue? Does the customer request 1 each, total 20 or 2 each, (required for the repair) Total 20? Answer #11 This item has been updated to specify each as the unit of measure and the estimated quantity was increased to 40. Please see the Revised Attachment I, Price Page, included in this Addendum. Question #12 Group I, Clippers, Item 31, Oster Guide #5 - size 1/5,8" appears to be a typo, size is 5/8" Answer #12 The correct size is 5/8, and has been corrected. Please see the Revised Attachment I, Price Page, included in this Addendum. Question #13 Group II, Item 34 Oster Fast Feed Switch, does the customer require an actuator - Switch? Answer #13 Yes. Question #14 Group II, Item 36 Oster Fast Feed Switch Button, does the customer require a switch? (mfg does not list a switch button) Answer #14 Yes, the Department is looking for a switch, not a switch button. This item has been updated. Please see the Revised Attachment I, Price Page, included with this Addendum. Question #15 Group II, Item 37 Oster Fast Feed Lever, does the customer require an adjustment lever? Please see the Revised Attachment I, Price Page, included with this Addendum. Question #17 Group II, Item 42 Andis ML Blade Screw, does the customer require a lower blade screw? Answer #17 No. Question #18 Group II, Item 43 Andis ML Lever, does the customer require an adjustment lever? Answer #18 Yes, this item has been updated. Please see the Revised Attachment I, Price Page. Question #19 Group II, Item 56 Andis T-Edjer Top Housing, does the customer require model or 15430? Answer #19 The Department is requiring Model Question #20 Group II, Item 61 Andis T-Edjar Housing Screw, does the customer require a screw -front cover, a screw -rear cover, cover -screw pad or screw cover screw pad?(mfg does not list a housing screw) Answer #20 This item has been removed. Please see the Revised Attachment I, Price Page, included with this Addendum. Question #21 Group II, Item 62 Adnis T-Edjer Armature Assembly, does the customer require 2 wire or 3 wire? Answer #21 The Department may require the 2 wire or 3 wire. An additional item hasbeen added to Group II of the Revised Attachment I, Price Page. Question #23 The agency solicitation does not specifically state all orders during the term of the contract will be electronic orders or credit card orders. If the agency desires to use a government credit card to order items, Credit Card Companies charge the prime contractor 3% for each government credit card transaction. The cost will be reflected in the quote to the FL agency. Identify if the agency is or is not going to use a government credit card during the term of the contract. Answer #23 During the term of the resulting Contract, the Department reserves the right to issue purchase orders (POs) or use the State of Florida Purchasing Card when ordering from the awarded Vendor. Please see Section 2.8 of the ITB. Of note, all prices submitted must be firm and inclusive of packaging, handling, shipping and delivery charges, and fees, per Section 3.9 of the ITB. Question #24 The agency has knowledge of the location for delivery during the term of the contract. Suppliers for the agency items provide free shipping over a specific amount. Suppliers charge shipping for orders under a specific amount. The agency has not identified the minimum dollar purchase with each agency order or the locations. Consider providing the number of locations and zip codes in the solicitation for delivery during the term of the contract. Addendum #003 3
