We continue to grow and hope to continue to meet your needs as we do. Please indicate which you are participating in on the form below and send with your payment prior to June 30 th. There will be a $10 late fee for paying on the 4 th. (Encouraging teams to sign up and pay early) No Excuses you will be required to write a new check if yours is in transit on the 4 th. There is plenty of room to have pop up tents and we suggest bringing some sort of shade for your group. Waivers: All players must sign a medical waiver prior to playing. You can print them off and bring them along or sign one when you arrive (we will provide copies). Anyone under 18 must have a guardian sign the waiver, so please contact us if you need that in advance of the tournament. Please note that this is a public beach and while the city is helpful to us in combing out the sand, we are not liable for what is in the sand or for any injuries incurred from playing on the beach. Directions: From Hwy 43, take the Hwy 23 exit and head east toward the lake. Continue eastbound until you get to 4 th Street. Your best bet is to park somewhere on 4 th street anywhere between Niagara and St. Clair Streets and walk toward Lake Michigan. **There is a 5K run and a parade that will be running along Broughton Drive on Wednesday morning that will cause problems with parking and driving near the lake. Please leave plenty of time to get yourself to the lake ON TIME. Teams must check in BY 8:00 am and play will begin by 8:45 at the latest. All matches will be played on Sheboygan s Lakefront RAIN or SHINE.

4 King of the Beach Tournament Playing Rules: THERE WILL BE A CAPTAIN S MEETING AT 8:00 AND PLAY WILL BEGIN AT 8:30!! All teams will be responsible for keeping their own score- the captain of the winning team is responsible for writing the scores on the clipboard by their court after the match is completed. We ask that you provide your own balls for warm-ups and matches. 1- No let serve 2- Players do not need to rotate 3- Players MAY hand pass a first contact 4- No Girl/guy rule 5- No open-hand tipping 6- No touching the net at all during play. 7- USA Volleyball rules will be used in case of any rules not listed here. All courts will use rally scoring (a point is awarded on every serve and sideout) -Pool play for pools of 5 teams -2 games to 21 cap at 25 -Pool play for pools of 4 teams -2 games to 25 cap at 25 -Pool play for pools of 3 teams 3 games to 25, no cap Playoff seeding will be determined using the following criteria: 1) Overall game percentage 2) Head to head games, then point differential 3) Point differential 4) One-game playoff to 21 Quarter-final rounds in playoffs will be 1 game to 25 All other rounds will be best 2/3 to 25, 3 rd game to 15

RELEASE FORM: I hereby release Team Elevate from all claims of damage, demands and legal actions whatsoever, including attorney s fees, arising in any manner from my participation in the King of the Beach Tournament. I have full knowledge of the risks involved in this sport, and I warrant herby that I am physically fit and sufficiently trained to participate. I freely agree that I am personally responsible for all risks of injury or damage to persons or property arising in any manner from my participation in this activity.
