1 ELEVATED WORK PLATFORMS PURPOSE AND SCOPE This HSE infrmatin prvides additinal directin and guidance n the use f Elevated Wrk Platfrms. The principles f this infrmatin dcument shall be applied; hwever the practical applicatin in the field may differ accrding t circumstance prviding the standards are nt lwered r safe utcmes are ptentially cmprmised. PLANNING FOR THE USE OF ELEVATED WORK PLATFORMS Accidents invlving EWPs are likely t lead t serius injury r death. Legally, every individual cncerned with EWPs carries full persnal respnsibility fr his part in selectin and peratin. Except where therwise stated, this dcument refers t all types f Elevated Wrking Platfrm. This includes thse EWPS described as: Vertical Scissr Lifts Self-Prpelled Bms Vehicle r Trailer Munted articulated Bms. Where the cmpany is perating n sites under the cntrl f third parties, r in partnership with ther cmpanies, then this bklet is the minimum standard which will apply. Advice may be sught n the applicatin f Regulatins and the cntents f this dcument frm the Select Plant Safety Lead, HSE Leader r yur Safety Advisr. Refer als t FSR 04 Wrking at Heights SELECTING THE RIGHT EWP Technical advice can be btained frm ur suppliers n selecting the crrect equipment fr specific circumstances. The cntact number can be btained by cntacting Select Plant s hire desk. General advice can als be sught frm yur Safety Advisr. The fllwing describes the prcess fr the hire f EWPs. Respnsibility fr using the crrect equipment is the respnsibility f the prject management. HIERARCHY OF CONTROLS Where wrk at height cannt be eliminated the hierarchy f cntrls must be applied t assess and reduce risk and t ensure we prvide the maximum level f prtectin. PS Wrking at Heights requires Risk Assessments t be carried ut befre starting any wrk at height. If wrk at height can be avided, then this must be dne. If there is n alternative then the wrk shall be carefully planned and rganised in advance, making sure that the equipment chsen is best suited t the task and nature f the wrk.

2 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 2 SELECTION CRITERIA Respnsibility fr using the crrect equipment is the respnsibility f the site management. There are many items that yu must carefully cnsider befre specifying yur EWP, including: The verall task and scpe The peratrs Access, interfaces and wrking space Grund cnditins including excavatins, undergrund services, uncmpacted edges Grund bearing pressures r slab capacity including basements Height and reach Persns and lad t be lifted Obstructins r verhead lines Material lifting and handling Ptential wind lads Suitable fuels and refuelling Emergency cnditins and rescue methds An expert Representative frm the supplier shuld be requested t assist in specifying the crrect EWP t use. DOCUMENTATION The fllwing dcumentatin will be required fr all EWP s used n Laing O Rurke sites: EWP Lgbk OEM manual Machine daily pre-start checklists Maintenance histry Plant Risk Assessments (site specific RA s are the respnsibility f site management) If the EWP Lgbk is nt supplied r des nt have an in-date three mnthly inspectin recrded in it then the machine is nt t be accepted. If the safety dcuments/manual r the machine daily checklist is missing then ensure that the driver clearly marks them dwn as deficient n the handver paperwrk. In either case please cntact the Select Plant hire desk immediately t rectify the missing dcumentatin. TRAINING COMPETENCY AND FAMILIARISATION The training, cmpetency and familiarisatin requirements are the same as described in PS Wrking at Heights.

3 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 3 PERSONNEL The safe use f pwered access equipment will depend n adequate management f all peratins. This in turn depends n the appintment f trained, cmpetent persnnel t manage and perate the equipment. SITE BASED PERSONNEL The site management has verall respnsibility t ensure that all elevated wrking platfrms are used crrectly n their sites, fr this reasn the fllwing guidance must be adhered t: Ensure that a cmpetent persn as designated by the Site Manager is made respnsible fr the management f EWPs n site Ensure all peratrs are in pssessin f a relevant authrised training card (EWPA Yellw Card) r a High Risk Wrk Licence if perating an EWP f 11m r mre and have cmpleted a certified Wrking at Heights curse Ensure that all peratrs receive a Verificatin f Cmpetency Assessment as well as a subsequent Familiarisatin Training fr each additinal make and mdel EWP being perated That the equipment is in pssessin f an EWP Lgbk, Operatrs manual, Machine daily pre-start checklists and 6 mnths maintenance histry That safe wrk methd statements and Risk Assessments have been undertaken and apprved and if a permit t wrk system is in place a permit issued prir t the wrk being undertaken. Site specific Risk Assessments are the respnsibility f site management. That the grund cnditins are suitable fr EWPs t be used safely That n verhead cables are in the wrking area if they are that they are suitably cvered r islated That a full bdy harness and a suitable fall restraint lanyards are used and that n bm type EWPs they are wrn and clipped n t the designated anchr pints at all times That EWPs are nt used as cranes and that nly accessries authrised by the manufacturers are used That daily pre-start checks are carried ut by all peratrs befre starting wrk That smene ther than the peratr n site is trained in the recvery f an peratr if the machine fails while being used. This persn and the recvery methd shall be detailed in the SWMS. Cntrls are n the base f the machine. OPERATOR The EWP peratr shall: Hld a relevant authrised training card (EWPA Yellw Card) r unit f cmpetency and hld a High Risk Licence/Certificate f Cmpetency if perating an EWP f 11m r mre Have cmpleted a certified Wrking at Heights curse and Harness Training in accrdance with AS/NZ Ensure that the equipment is nly used fr its intended use (and nt fr example as a crane)

4 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 4 Be respnsible fr greasing, daily maintenance checks and reprting any defects r incidents Be respnsible fr carrying ut daily pre-start checks Be in the pssessin f a current driving licence if the EWP is driven n the public highway Have received a Verificatin f Cmpetency Assessment as well as subsequent Familiarisatin Training fr each additinal make and mdel EWP being perated In bm type EWPs wear a full bdy harness with a suitable restraint lanyard and be clipped n at all times t the anchrage pint prvided by the manufacturer r where identified by Risk Assessment SITING AND STABILITY GROUND CONDITIONS The EWP can be affected by pr grund cnditins, which may cause settlement and lead t the machine being ut f level and hence unstable. As the peratr yu and yur machine are at great risk frm pr grund cnditins. Typical cnditins that yu must be aware f are: Uncmpacted fill sil r ther fill material may be piled alng the line f a backfilled trench withut being cmpacted. An indicatin f uncmpacted fill can be the cracking f the grund alng the line f the trench. Cellars and basements Many are incapable f bearing the weight f yur machine with r withut a lad, and may cllapse withut warning Undergrund services sewers, drains, manhles, gas and water mains etc. may be damaged by the weight f yur machine r may even cllapse and cause it t verturn Weather cnditins heavy r prlnged rain may alter grund cnditins and cause sinking. Adjust r check levelling, packing mats etc. if yu suspect that the grund is getting sfter. A check n the changing grund cnditins must be made at regular intervals thereafter. The same regular checks must be made when frzen grund is thawing ut. Frzen grund can appear t be much firmer than it actually is. Temprary Hle Cverings in slabs when flush and dusty, can be missed and pse a threat t the EWP. Check the slab area fr traps r penetratins and check the slab ladings befre perating an EWP in that area. Where relevant ensure a permit is in place fr wrking n the slab. USE OF OUTRIGGERS (STABLISERS) The fllwing guidance must be adhered t when using utriggers: When using an EWP fitted with utriggers always fllw the manufacturer s recmmendatins Befre raising the platfrm r cage ensure that the machine is levelled within the manufacturer s limits and lcated n a firm surface. The use f suitable packing must always be cnsidered when it is necessary t spread the lad under the utriggers. Check that each utrigger and its packing is in full cntact with the grund, bth befre starting wrk and regularly thereafter

5 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 5 If yu are in any dubt abut the grund cnditins at any time cnsult with the site management, wh if necessary can have it tested by the Temprary Wrks/Engineering department With sme types f machine the full weight f the machine has t be taken befre the platfrm is raised. (Refer t manufacturer s instructins prir t starting wrk) The utriggers if fitted must be fully extended hrizntally n bth sides unless the machine is specifically designed t allw part extensin Never travel the machine with the utriggers in the extended psitin EXTENDING AXLES Extending axles, if fitted, must be fully extended hrizntally n bth sides unless the machine is specifically designed t allw part extensin Axles must be retracted when travelling n the rad and when travelling t and frm the place f wrk The nly circumstances where the EWP can be mved with axles extended, is when an adjustment t the psitin f the machine is necessary as part f the wrk being undertaken SAFE WORKING LOADS It is critical that the safe wrking lad is nt exceeded. Ensure that yu adhere t the fllwing guidance: The Safe Wrking Lad (SWL) is the maximum lad that the EWP will safely carry. This SWL includes the weight f all persns, tls, equipment, hses, cables and materials. Overlading by exceeding the SWL is extremely hazardus and must never take place. Nt nly may it cause the machine t verturn but it may damage the machine. Yu must ensure that the SWL f the machine is sufficient fr the maximum cmbined weight f persns, tls and equipment befre starting wrk. An allwance must be made fr any additinal lads that will need t be carried by the platfrm during the wrk. It must be nted that sme manufacturers allw varying SWLs fr particular machines. Cnsult the manufacturer s lad chart and manual. Any attachments used must have the necessary examinatin certificates befre being used SWLs must always be marked n the machine in a prminent psitin and shwn in kilgrams (and Ibs) by diagrams indicating the maximum number f persns OTHER HAZARDS AFFECTING STABILITY Other factrs that will reduce the stability f the EWP and cause verturning r cllapse are: The uneven distributin f the lad n the wrk platfrm. Lads must nt be balanced n the guardrails and if lads will prject beynd the basket, an apprved material handling attachment must be specified and used. Using the machine in high winds. The maximum wind speed in which the machine may be safely used will be specified by the manufacturer in the perating manual and marked n the machine.

6 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 6 Sudden impact (Shck) lads frm falling bjects r being struck by ther site vehicles Pushing r pulling (hrizntally) n a structure alngside the platfrm can cause instability, verturning and damage t the EWP when the lads are greater than specified by the manufacturer There may be a risk f the peratr being crushed against verhead structures r beams particularly if there is any ver run n the cntrls (see Aviding Trapping/Crushing Injuries). A Risk Assessment must take int accunt any risk invlving height restrictins. Other plant r cranes may be wrking in the area. This hazard must be included in all Risk Assessments and SWMS carried ut. SAFE WORKING PROCEDURES The fllwing guidance must be fllwed t ensure the safe and crrect use f EWPs: The EWP is a wrking platfrm and must nt be used as a crane by suspending lads beneath using slings r any ther type f lifting accessries Operatrs shall always access and egress frm the platfrm when it is fully lwered and by the steps/ladders prvided fr that purpse Persns shall nt attempt t leave the platfrm whilst in the elevated psitin, nr must materials be transferred Always ensure that the EWP is level befre starting wrk Never carry mre persnnel in the cage than as indicated Never use the EWP t carry material utside f the cage Step ladders, hp ups r the guard rails must nt be used in the cage t gain extra reach r height If EWPs are self-prpelled the peratr shall ensure that he walks the rute befre mving t ensure that there n hazards in his path Laing O'Rurke has impsed a prhibitin n the mvement r peratin f any electric elevated wrk platfrms, whatever make r mdel, n any inclined surface WORKING NEAR HIGH VOLTAGE LINES Wrking near high vltage cables must be avided if at all pssible if it is nt a chice then the fllwing precautins must be taken: Always maintain a safe distance. Exclusin znes apply whenever yu need t carry ut sme wrk, r perate plant r a vehicle, arund a live electrical part and yu cannt eliminate the risk. The exclusin zne is based n the verall situatin and what yu can d t reduce the electrical risk. Yu must gather the necessary facts and then use the infrmatin t lk up the crrect exclusin zne n the tables in the Electrical Safety Cde f Practice. Fr details f exclusin zne distances, see Appendix B f the Electrical Safety Cde f Practice and als schedule 2 f the Electrical Safety Regulatin 2002.

7 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 7 On cnstructin sites where EWPs have t pass under verhead electric lines, grund level barriers must be psitined and gal psts erected at the place where yur machine may pass under the verhead lines If there is n need fr the EWP t travel under verhead lines, bth grund level barriers and high level markers must be placed t keep the machine at a safe distance Emergency Drill n Cntact with a Live Electric Line If the EWP makes cntact with a live electric line, bserve the fllwing precautins in rder t minimise the risk f electrcutin: The peratr is t remain in the platfrm r in the cage Warn all ther persnnel t keep away frm the machine and nt t tuch any part f it Place suitable guards arund the machine t ensure ther peratives n site d nt cme int cntact Try, unaided, and withut anything appraching the machine, t mve the machine until it is clear f the pwer line and/r lwer the platfrm t the grund If the machine cannt be mved away, the peratr is t remain in the cage Infrm the lcal electricity supply authrity at nce. Take n further actins until they give clearance that the pwer has been turned ff. Devices are available that are designed t give warning when a machine mves within a predetermined distance f verhead lines. Such items shall nt be cnsidered as a substitute fr a safe system f wrk. WORKING ON ROADS AND HIGHWAYS When wrking n the public highway the fllwing precautins must be fllwed: Ensure that yur perating area is fully segregated using a physical barrier e.g. Jersey Kerbs Under n circumstances must yu allw any part f the EWP t extend r swing int a line f traffic If arrangements are required t divert the traffic this shall be undertaken by a cmpetent trained persn in full cmpliance with the relevant state s cde f practice fr rad wrks When peratins are t be carried ut during the hurs f darkness at a lcatin where the public have access, barriers must be prvided tgether with yellw flashing beacns If the wrk platfrm is t be left at the wrk site vernight, permissin must be btained frm the relevant authrity, e.g. lcal and main rads, cuncil etc. FALL PROTECTION IN EWP's Each year there are a number f serius accidents in which peratrs are thrwn frm EWPs. In many situatins, the wearing f a safety harness wuld have prvided gd prtectin in the event f falling, r being thrwn, frm the EWPs basket. It is the grup plicy that all peratrs wrking in Bm lifts will wear full bdy harnesses with a fall restraint lanyard and be clipped n t manufacturers anchrage pints at all times. The fllwing guidance must be fllwed:

8 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 8 If any ther type f EWP is being used a Risk Assessment must be undertaken t see if fall arrest equipment is required, i.e. scissr lift All peratrs wearing harnesses shall be trained and cmpetent in the selectin, maintenance and use f fall arrest equipment Safety harnesses and assciated accessries must be inspected in accrdance with legislatin and PS Wrking at Heights which fr a full bdy harness is every three mnths, nce issued All equipment prvided by Select plant is equipped with anchrage pints which can nly be used with fall restraint lanyards OPERATION IN CONJUNCTION WITH OTHER EQUIPMENT When an EWP is t be perated in cnjunctin with a crane r sme ther appliance, the wrk must be prperly planned by the manager respnsible fr the management f EWPs n site and a safe system f wrk develped which must be clearly understd by all persns wh are participating. All persns must als knw hw t deal with any freseeable emergencies. Arrangements must be made fr peratrs t be able t cmmunicate clearly with each ther. WIND A mbile elevating wrk platfrm must nt be used in wind speeds exceeding thse specified by the manufacturer. One cmmnly specified wind speed is that f 45kph (Beaufrt Scale 6 Strng Breeze), r abut 12.5m per secnd. This is als generally accepted as the maximum wind strength in which an peratr can wrk withut undue discmfrt. An anemmeter shuld be present n site in rder t measure wind speed accurately. Other prblems assciated with perating in windy cnditins include: Funneling effects f winds between buildings, where actual wind speeds may be duble that measured in the pen In the vicinity f large slab-sided buildings, high wind speeds and eddy currents may be created at the crners and n the side facing the wind The effect f height - wind speed may be 50% greater at a height f 20m than it is at grund level Wind chill factr - n a calm day 10 C is cl but nt unpleasant, but with a wind f 30kph the temperature experienced n the face and hands is 0 C. If the day is very cld, abut freezing, the temperature experienced n the skin will be dwn t 15 C, making it very difficult fr the peratr t wrk safely unless prperly clthed and equipped. Care must be taken when handling building cladding, sheet materials, panels and ther such materials which can act as sails and seriusly affect the stability f an EWP, especially in gusty wind cnditins. Fr the same reasn, signbards and the like must nt be attached even temprarily t the platfrm. Other surces f lcal high wind speed t cnsider are aircraft slipstreams at airprts and high sided vehicles n mtrways TRAPPING / CRUSHING INJURIES The increased use f EWPs in cnstructin, maintenance and ther applicatins where trapping risks are present has led t accidents in which peple in the platfrm have been trapped between the platfrm (ften referred t as a cage r basket) and bjects in the wrk area.

9 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 9 Where trapping risks are present extra care is needed and all invlved must understand what they need t d t avid r reduce the risks invlved. The basis f preventing trapping accidents must be task, site and equipment specific risk assessment. All invlved in the management and peratin f EWPs need t understand hw t minimise the risks f smene being trapped in the basket and the imprtance f having effective rescue prcedures shuld such an entrapment ccur. Wrking clse t verhead structures shall be regarded as a higher risk EWP peratin. If yu are expected t carry ut this srt f wrk, yu must ensure that the fllwing issues have been prperly addressed, and that management has explained the steps taken t minimise trapping/crushing risks in a planning the wrks. Plan the EWP rute carefully Keep a sensible distance frm bstructins The rute taken by the EWP must ideally be planned s as t keep a sensible distance between the EWP and any verhead bstructin. This distance will need t be greater fr a bm-type EWP being driven at height t allw fr the pssible bunce and see-saw effects. Avid the drive / elevate / slew cntrls when clse t an bstructin If wrking clse t an verhead bstructin is unavidable, it is strngly recmmended that, where pssible, nly the fine-psitining cntrls f a bmtype EWP must be used. Once the EWP is clse t the bstructin the carser drive, elevate and slew cntrls must be avided. Mvements must always be slw, deliberate and planned. This is achieved by careful use f the EWP s prprtinal cntrls. The sequence f cntrl use given belw is recmmended: WHEN ELEVATING WHEN DESCENDING 5. Fine Cntrl 1. Fine Cntrl 4. Telescpe 2. Telescpe 3. Slew 3. Slew 2. Elevate 4. Descend 1. Drive 5. Drive Driving at height shall be the last resrt Driving a bm-type EWP at height must be the maneuvre f last resrt when psitining the platfrm clse t an verhead bstructin since it can create unexpected mvements that make fine adjustment f the platfrm psitin difficult t achieve If driving at height is cnsidered the least risk ptin, bms must be driven at their slwest speeds (this is f particular relevance at lwer heights, when drive speeds are faster) Ensure gd visibility at height

10 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 10 When wrking inside the building, and at times f lw light (e.g. in winter mnths r in pr weather), adequate lighting must be prvided r wrk suspended Minimise Distractins Distractins in the platfrm/basket, such as mbile phnes and trailing cables must be strngly discuraged. Lse materials n the EWP handrails r in the basket f the EWP must be prhibited and shall be carried in apprved cntainers and/r using apprved materials handling attachments. Distractins n the grund (peple r bjects near the EWP base) must be remved befre perating and exclusin znes cmplied with D Nt Obstruct Cntrls Basket cntrls - basket/platfrm hand and ft cntrls must nt be bstructed. Tls and materials which culd bstruct the cntrls must nt be placed n the EWP cntrl panel but stred in apprved cntainers and and/r using apprved materials handling attachments. Once in psitin, cnsider islating the pwer until yu need t re-psitin t reduce the risks f accidental peratin Emergency lwering cntrls - these cntrls culd be required t effect an emergency rescue and must nt be bstructed by bjects n the grund (e.g. perating EWP clse t a wall with emergency cntrls facing the wall) Slw dwn, dn t cruch ver the cntrls and lk! Slw drive speeds must be used, particularly when reversing Cruching ver the cntrls significantly reduces the peratr s margin f safety Scan the area fr bstructins bth befre and during EWP peratin D nt lean ver the guard rails while perating the EWP D nt verride the EWP cntrls r use faulty EWPs Check that EWP has a valid Quarterly Inspectin Always perfrm daily checks Reprt all faults Any faults must be rectified befre using EWP D nt verride the cntrls EMERGENCY RESPONSE AND RESCUE PROCEDURES Emergency arrangements shall always state that an EWP peratr must always have an apprpriately trained persn wrking with them in case the platfrm needs t be lwered t the grund. They must als include the call ut details fr a service engineer wh is cmpetent and authrised t lwer the platfrm in the case f an emergency

11 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 11 Prcedures must be in place fr the rescue f peratrs wh have fallen ut f the basket and are suspended in their harness. These prcedures must be prepared and cmmunicated t all users and must be cntained within the accepted SWMS fr the wrk. EWPs are fitted with a secndary guarding / push bars t prevent entrapment and / r t minimise the impact f crush related incidents EWPs are fitted with emergency (Auxiliary) lwering cntrls and an emergency stp switch. Befre using an EWP the peratr(s) and anther respnsible persn n site must knw hw t use the emergency cntrls. Safety harnesses/lanyard cmbinatins must nly be attached t the anchrage prvided by the manufacturer. Lanyards must never be attached t any ther bject r a structure utside the platfrm. Never attempt t climb the lattices frmed by the scissr mechanism f a scissr lift. If yu break dwn wait fr recvery prcedures t be fllwed. Never interfere with, wedge r verride hydraulic, electrical r mechanical safety devices r cntrls If yu are using equipment which has cables r hses attached, these must never be left hanging free, but must be prperly supprted D nt use the machine t tw anther vehicle, unless it is specifically designed and equipped fr this purpse Self-prpelled EWPs must nt be twed. This can cause serius damage t the machine. Travelling with the platfrm f an EWP ccupied and raised must nly be undertaken when the machine is specifically designed t be used in this way. Check the manufacturer s instructins befre attempting this. When nt in use, the peratr must always remve the keys r therwise islate the machine t prevent unauthrised use The grund key fr the EWP shall ideally be left in the base unit where this is practicable, r at least quickly available at grund level if nt While the EWP maneuvre is taking place at least ne (and as many as is apprpriate) designated grund rescue persn must be appinted wh knws the rescue prcedure and has been familiarised with the EWP being used (including emergency rescue cntrls). They must always be readily available in the event f an emergency. Implementatin f the rescue plan depends upn the cmplexity f the peratin and therefre the relative risk f effecting a rescue frm the grund cmpared t the risk f an peratr, pssibly in a state f panic, trying t rescue himself. It als depends n hw the cntrls fr the specific EWP being used functin if the lad cell has been activated. The rder f pririty shall be: Operatr: The peratr, r ther cmpetent peple in the basket, must try t rescue themselves by retracing the steps they tk in reverse rder Grund staff:

12 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 12 If visibility and understanding f situatin frm the grund are gd, grund staff must effect a rescue using the grund cntrls in the fllwing rder: Anther EWP: Auxiliary pwer at first which gives the slwest and mst cntrlled maneuvre f the bm until it is bvius that the basket is clear f any bstructins at height Pwered descent: nce clear f bstructins, it is then recmmended t switch t pwered descent t maximize the speed f recvery In sme situatins the use f anther EWP t gain access t the platfrm may be the safest ptin. This will nly be acceptable if such rescue has been planned and includes means f transferring between platfrms which prevents anyne falling. MOVEMENT BY ROAD The fllwing guidance must be fllwed befre mving EWPs n the rad: Befre travelling n the rad with a vehicle munted EWP, make sure that the driver knws the clearance height and width f the vehicle which must be marked in the cab If the machine is fitted with utriggers, r extending axles, check that these are fully retracted and lcked in place Where apprpriate, check that the slewing lck has been applied When lading r unlading EWPs frm transprters, adequate ramps must be used and crrectly psitined. The use f the vehicle winch is recmmended n certain vehicle types f EWP. When negtiating ramps with a self-prpelled machine with a bm, the manufacturer s instructins must be fllwed INSPECTION AND MAINTENANCE The inspectin and maintenance f EWPs must be carried ut in accrdance with the lcal legislative requirements. If any 3 mnthly inspectin shws that the EWP cannt be used safely, the equipment must nt be used until the faults are rectified. This applies t all types f EWP. DAILY PRE-START CHECKS At the beginning f each shift r wrking day, rutine checks as apprpriate fr the type f EWP must be carried ut prir t starting wrk. This cmprises f bservatins and functinal checks. These checks are intended t reveal nly bvius defects. The persn carrying ut the daily pre-start checks shall reprt any defects t the appinted persn, wh must ensure that all defects are reprted t the Select Hire Desk wh will arrange fr the machine t be repaired. MAINTENANCE EWPs must be maintained in accrdance with the manufacturer s instructins and at intervals, which take int accunt the intensity f use, perating envirnment, variety f peratins and the cnsequences f malfunctin r failure. Maintenance shall nly be carried ut by persnnel wh are bth familiar with the equipment and cmpetent t carry ut the wrk. Sufficient time must be allwed in the site prgramme fr maintenance t be carried ut

13 HSE Infrmatin Elevated Wrk Platfrms (Rev ) Sectin 13 effectively. Sites will be given suitable ntice when fitters are due t carry ut n site maintenance. ROUTINE MONTHLY / QUARTERLY INSPECTIONS Rutine inspectins and maintenance shall be based upn the wrking envirnment, and the frequency and severity f use f the EWP while in service. The inspectin shall be carried ut at n mre than three-mnthly intervals unless the EWP is nt in-service. Sufficient time must be allwed in the site prgram fr inspectins t be carried ut effectively. PERIODIC / ANNUAL INSPECTIONS A prgram f peridic inspectin shall be carried ut. The frequency f peridic inspectin shall be based n the wrking envirnment and the frequency and severity f use f the EWP. Fr all EWPs that remain in-service, the inspectin interval shall nt exceed 12 mnths. Sufficient time must be allwed in the site prgram fr inspectins t be carried ut effectively. ENHANCED PERIODIC 5 YEAR +/ 10 YEAR MAJOR INSPECTIONS After the first 5 years f service, and each and every year thereafter, peridic inspectins shall be structured t ensure all critical cmpnents are inspected, and tested where apprpriate, within a 5 year perid, until the EWP is scrapped r decmmissined. Inspectin f the critical cmpnents may be deferred till the end f the 10th year f the machines life, in which case the EWP shall be subjected t a majr inspectin. The majr inspectin shall invlve examinatin f thse critical cmpnents identified by the manufacturer r a cmpetent persn. Where necessary, the EWP shall be stripped dwn and paint, grease and crrsin remved frm critical cmpnents t allw a cmplete and thrugh inspectin.
