5 1 Appendix A: Prcedures and Frms This appendix cntains cmpany-specific perating practices, frms, checklists, reprts, r ther cmpany practices. The purpse f this infrmatin sharing is t prmte pipeline safety by allwing cmpanies t cnsider a variety f cmpany safety practices that may enhance pipeline safety in apprpriate situatins. API takes n psitin n whether the practices described belw are apprpriate fr any particular pipeline peratr, pipeline facility, r perating cnditin. These examples are nt API Standards r Recmmended Practices (althugh sme examples may incrprate prtins f API Standards r Recmmended Practices). Use f mandatry r advisry terms such as shall, shall nt, must, may, may nt, r similar terms reflect the submitting cmpany s particular practice, and shuld nt be interpreted as necessarily reflecting mandatry r advisable practices by API. 1.1 Planning fr Excavatin and Trenching Wrk Prcedure Example 1. The Cntractr cnducting the excavatin activities: a. Is respnsible fr cmpliance with One Call ntificatin, including any needed updates, as defined by applicable State requirements. Emergency One Call ntificatins shuld nly be made as defined by applicable State One Call law. b. Shall attempt t receive psitive respnse frm all undergrund utilities identified n the One Call ticket(s) and ensure Cmpany assets are identified n the One Call ticket(s). c. Shuld cmplete a secndary sweep f planned excavatin area t identify any unknwn utilities. 2. The Authrized Representative will cmplete sectins 1 and 2 f the Daily Excavatin Checklist prir t beginning the excavatin. Nte: All Cmpany wned and/r perated lines must be marked by the Cmpany t ensure the party perfrming the wrk is aware f all lines in and/r adjacent t the excavatin. 1. All abve grund structures within the bm radius f the excavatin equipment being used must be identified, and prper precautins with respect t lcatin and spacing f equipment are taken: a. A minimum 10 clearance frm electrical equipment is required. b. If the 10 clearance distance cannt be maintained, arrangements must be made with the utility prvider t cut the pwer r prvide alternative means t prtect the line frm incidental cntact. 2. Any undergrund vessel r ther structure ftings shuld be lcated and identified t avid damage during the excavatin wrk activities. Page 5 f 110

Alignment Sheets r ther drawings will be reviewed t verify if knwn appurtenances such as stpple fittings, purge r equalizatin fittings, r ther cnnectins are in clse prximity n any undergrund piping being excavated. 4. Pipeline Cntrl Center (when applicable) will be ntified and asked t mnitr the pipeline system during the excavatin. 5. All cntract persnnel must be apprved under the Operatr Qualificatin Prgram fr the wrk assigned when the line is under the jurisdictin f DOT r a State regulatry agency. 6. Prir t any digging, the area f excavatin is prbed (r hydr-excavated) in a pattern extending at least 12 beynd the intended perimeter f the trench/hle: a. At the discretin f the Cmpany Operatins Supervisr r Authrized Representative, hand digging r hydr-excavatin may be required t btain visual verificatin f the line identified during the prbing prcess befre any mechanical digging is cmmenced. b. All Cmpany assets must be marked as designated in the line lcating prcedure. Nte: All excavatins must be in cmpliance with 29 CFR 1926, Subpart P Mechanical Excavatin 1. When sil cnditins permit, all backhe buckets used by cntractrs fr excavatin near existing facilities r equipment must have a safety bar acrss the teeth t prevent line snagging that may cause damage t line and/r catings. a. In additin, side-cutters may need t be remved frm the bucket if sil cnditins permit. 2. The Cmpany Operatins Supervisr r designated emplyee must apprve: a. The use f teeth and/r side-cutters n backhe buckets. 3. The use f handheld mechanical devices such as jackhammers and chipping guns. a. N track-he with a hydraulic excavatr will be allwed t be used n any prject r excavatin jb with quick-cnnect cuplers unless the excavatr has been retrfitted t allw the safety pin t be manually inserted behind the frnt and/r rear lever f the cupler assembly. 4. The Inspectr and Operatins Supervisr r Authrized Representative n the jb site will inspect all equipment with hydraulic excavatrs befre excavatin wrk cmmences t ensure that the safety pin has been prperly inserted behind either the frnt r rear lcking lever. a. Any peratinal and/r mechanical failure f the excavatin equipment is immediately crrected, r the equipment is taken ut f service r replaced. Page 6 f 110

7 5. Inspectins f the equipment, prir t perating, are the respnsibility f the equipment wner/peratr and are dne daily and in accrdance with OSHA requirements. 6. Alternative excavatin methds are required fr the fllwing cnditins: a. Rcky sil cnditins when determining the exact depth and lcatin f a line; OR b. Multiple lines exist in clse prximity t each ther; OR c. Knwn freign crssings are present. 7. These cnditins and situatins create a risk f line damage in using the cnventinal excavatin equipment and require the applicatin f the fllwing methds: a. Excavating parallel t the pipeline(s) frm a lcatin upstream r dwnstream where centerline and depth f cver can be psitively verified; b. Pt hling the area f activity with hydr-excavatin equipment; and c. Hand digging. 8. In sme cases, the cmbinatin f ne r mre f the listed techniques may be used in cnjunctin with cnventinal excavatin equipment. This must be reviewed and apprved by the Authrized Representative. 9. When excavating pipeline(s) fr any type f freign pipeline crssing, maintenance r inspectin activity, the excavatin is dne parallel t the existing pipeline(s). a. Digging perpendicular t the pipeline is prhibited until the line is expsed Tlerance Zne 1. Tlerance Zne is defined as a minimum f 18 frm the uter edge f the pipe (in all directins), r as defined by State regulatins, whichever is greater. 2. Mechanical excavatin equipment used t remve material surrunding a Cmpany pipeline must maintain a minimum buffer f at least 18, r as defined by State regulatins, whichever is greater, between the excavatin equipment and the circumference f the pipeline(s). 3. Nnmechanical excavatin equipment (hydr vacuum, hand tls, water jet) may be used within the tlerance zne at any time Mechanical Excavatin within the Tlerance Zne Mechanical excavatin within the Tlerance Zne f energized undergrund pipelines presents additinal risks that must be addressed. As such, ther reasnable alternatives must be reviewed during the Jb Planning prcess, including lwering the line pressure and/r taking the pipeline ut f service during the excavatin activities. Mechanical Excavatin within the Cmpany s defined tlerance zne f in-service pipelines may be allwed nly if all f the fllwing cnditins have been met: Page 7 f 110

Prject Manager, Field Engineering r Operatins must develp a written Jb Plan that clearly identifies the mechanical equipment type(s), including the use f special attachments (i.e., hammer hes, tampers, augers, etc.). a. Special attachments, including any part f the pwered equipment, may nt be used within 6 f any in-service undergrund pipeline r facility. b. The use f hammer hes, r similar attachments, within the tlerance zne f energized undergrund pipelines, shall be limited t Integrity rehab prjects where it is necessary t prvide clearance t prperly inspect and repair anmalies r where stpples must be installed with the pipeline in-service. Other activities may require a Variance. 2. The pipeline(s) must be free f buried side taps, split tees, r ther buried appurtenances in the area being excavated. The mst recent Internal In-Line Inspectin (ILI) run and Alignment Sheets, fr the pipeline(s) t be excavated, must be reviewed by Field Engineering, Operatins r Pipeline Integrity t identify all PI s, taps, r ther buried appurtenances: a. If any buried side taps, split tees r appurtenances exist r ptentially exist, n mechanical equipment can be used inside the tlerance zne. b. If n pipeline data r drawings exist, n mechanical equipment can be used inside the tlerance zne. 3. A Cmpany representative r Inspectr must ntify the lcal Operatins Supervisr and Pipeline Cntrl Center prir t the Excavatr cmmencing the prject. 4. The Excavatr shall exercise all necessary reasnable care t prtect the pipeline in r near the excavatin area. a. Excavatrs shall maintain a minimum f 6 f clearance between the pipeline and lcatin f the hydraulic arm and any attachments (nt just the end r tip f the bucket, hammer he, tamping r ther device). Nte: Mechanical excavatin inside the tlerance zne f in-service pipelines shuld nly be perfrmed by an experienced equipment peratr wh is prficient with the equipment and attachment(s) being used. 1. Mechanical excavatin is allwed at a prescribed distance per state requirements, frm the tp f pipe (TOP). 2. Once within the prescribed distance per state requirements, the TOP must be expsed via hand digging r vacuum excavatin until the TOP is cntinuusly expsed and sides f pipe identified via prbe bar(s). 3. If it has been determined, there are n buried side taps, split tees r ther buried bjects n the pipeline(s), then with the TOP expsed, and sides f pipeline identified via prbe bars, mechanical excavatin can begin t within 6 f the uter edge f the pipeline(s). 9 4. The remaining 6 f sil must be remved via hand digging r vacuum excavatin Cmpetent Persn 1. A designated Cmpetent Persn is respnsible fr each excavatin when the excavatin is t be entered by wrkers. 2. The cntractr is accuntable fr designating the qualified Cmpetent Persn fr the jb. 3. The Cmpetent Persn is respnsible fr: a. Cnducting sil testing by a visual and a manual methd t determine the prtective system(s) t be used in the excavatin and dcumenting the results; b. Remval f emplyees frm the hazardus area where evidence f pssible cave-in, failure f prtective systems, hazardus atmspheres, r ther hazardus cnditins exists; c. Cmpleting the designated prtin f the Daily Excavatin Checklist frm and maintaining the dcumentatin at the wrk site; and d. Identifying hazards and taking prmpt crrective measures t eliminate identified and predictable hazards which endanger excavatin entrants. 4. The Cmpetent Persn will have and be able t demnstrate the fllwing: Training, experience, knwledge f sil analysis; use f prtective systems; and requirements f 29 CFR Part 1926, Subpart P. a. The Cmpetent Persn certificate f training will be cmpleted, retained and available fr review by an Authrized Representative upn request Daily Inspectin 1. The Cmpetent Persn shall make daily inspectins f excavatins that will be entered by persnnel, the adjacent areas, and prtective systems fr evidence f a situatin that culd result in pssible cave-ins, indicatins f failure f prtective systems, hazardus atmspheres, r ther hazardus cnditins. The Cmpetent Persn shall dcument the daily inspectins in the Daily Excavatin Checklist. a. An inspectin shall be cnducted prir t the start f wrk and as needed thrughut the shift. b. Inspectins shall als be made after every rainstrm r ther hazard increasing ccurrence. 2. If the Cmpetent Persn finds evidence f a situatin that culd result in a pssible cave-in, indicatins f failure f prtective systems, hazardus atmspheres, r ther hazardus cnditins, expsed emplyees will be remved frm the area until the necessary precautins have been taken t ensure their safety. 10 1.2 Engineering and Cnstructin Guideline Example Engineering Guidelines 1. OPERATOR cnstructs, repairs, perates and maintains its pipelines in cmpliance with current U.S. Department f Transprtatin (DOT) regulatins, industry and cmpany standards fr safe peratins. Shuld Encraching Party prpse plans that infringe n OPERATOR s rights r affect OPERATOR s ability t meet these requirements, mdificatins t the pipeline r plans will be required. The cst f all such mdificatins shall be brne by the Encraching Party. The fllwing guidelines apply t Encraching Party and any cntractrs, agents and r representatives it uses fr cnstructin activities cnducted in OPERATOR s right-f-way and /r affecting OPERATOR s pipelines: 2. In rder t maintain immediate and unimpeded access t the pipeline, n trees, shrubs, permanent structures (i.e., buildings, decks, sheds, swimming pls, inlets, drainage structures, hydrants, ples, etc.) bdies f water shall be placed within the pipeline right f way. 3. The depth f earth cver ver the pipelines shall be maintained and never changed in any manner withut the express written cnsent f OPERATOR. In sme cases, additinal cver may be required. 4. In areas where the pipeline currently has less than three feet (3 ) f cver, n grade cuts will be allwed. Cver ver the pipeline shall be increased t a minimum f three (3) feet if there are prpsed "imprvements" ver the pipeline r within the pipeline right-fway. Prpsed rad crssings have additinal requirements. 5. Prpsed grading which will place the pipeline at depths greater than seven feet (7 ) shall require an OPERATOR Engineering Department written apprval. 6. In areas where buildings are prpsed within fifty feet (50 ) f the pipeline facility, vertical cver ver the pipeline shuld be increased t a minimum f fur feet (4 ), maximum f seven feet (7 ). 7. The creatin f steep slpes within the pipeline right f way that will hinder access and maintenance shall be avided. Maximum slpe f 5 t 1 within the ROW. 8. The creatin f strmwater utfalls r ther water management cntrls which wuld make the pipeline right-f-way mre susceptible t ersin shall be avided r mitigated. Prpsed gabins and rip rap structures must adhere t the vertical ne ft (1 ) vertical clearance requirement. Getextile prtectin may als be required. 9. Drainage swales shall maintain a minimum f three feet (3 ) f vertical clearance between the O.D. f the pipeline and the bttm f the swale. Additinal prtectin may be required in rder t minimize ersin susceptibility ver the existing pipeline and acrss its assciated right f way. Page 10 f 110

11 10. N fences will be allwed n the ROW withut OPERATOR s prir written apprval. Fences shall be easily remvable and nt t bstruct the view f the ROW fr inspectin purpses. N Masnry, brick, r stne fences will be allwed. Fences that are perpendicular t the pipeline(s) shall include a gate sixteen feet (16 ) in width fr equipment t pass thrugh. Fence psts shall nt be placed within 5 feet f the pipeline(s). Fences that are parallel t the pipeline(s) shall be lcated utside the pipeline s ROW. 11. Prpsed residential driveway crssings shall be designed t prvide a minimum f three feet (3 ) (maximum f 7 feet) f vertical clearance between the O.D. f the pipeline and the finished surface. Exceptins must be apprved by OPERATOR s Engineering Department 12. Prpsed driveway/rad crssings shuld be designed t be as perpendicular t the pipeline as pssible. Prpsed crssings f angles less than 30 degrees will nt be accepted. 13. Prpsed cmmercial entrance crssings shall be designed t prvide a minimum f fur feet (4 ), maximum f seven feet (7 ) f vertical clearance between the O.D. f the pipeline and the finished surface. 14. Prpsed rad ditches shall be designed t prvide a minimum f three feet (3 ) (maximum f 7 feet) f vertical cver abve the tp f the pipeline and the bttm f the ditch. 15. Rad underdrains shall maintain a tw ft (2 ) r greater minimum vertical clearance frm the tp f the pipeline. 16. Cnstructin f parking lts ver the pipeline(s) shall nt be permitted withut OPERATOR s prir written apprval in an Encrachment Agreement releasing OPERATOR frm any and all future damages t the parking lt due t pipeline maintenance and repair. 17. Structures such as guide rails, cncrete paving, sidewalks, curbing, etc. shall be designed in a manner that wuld facilitate their remval in the event f pipeline maintenance r an emergency repair. 18. Prpsed crssings by utilities r undergrund structures shall be designed t pass under the pipeline unless therwise apprved by OPERATOR s Engineering Department. 19. Prpsed utilities shall crss as perpendicular t the pipeline as pssible. 20. All undergrund facilities r structures crssing the pipeline shall maintain a tw ft (2 ) minimum vertical clearance between the O.D. f the pipeline and the prpsed utility structure. 21. Utility crssings f the pipeline shall be shwn n prfile with field verified pipeline depths and prpsed clearances clearly labeled. Page 11 f 110

12 1.2.2 Pwer Cable Installatins 1. Secndary Crssings (less than 440 Vlts): a. Must be installed UNDER the pipeline, (unless therwise apprved by OPERATOR s Engineering Department), prvided the tw-ft (2') minimum vertical clearance is maintained between the bttm f the pipeline and the tp f the cnduit. The cable must be placed in cnduit fr the width f the right-f-way, and a drive pst with an electric cmpany marker shall be placed n each side f the pipeline rightf-way. 2. Primary Crssings (greater than 440 vlts): a. Must be installed UNDER the pipeline, (unless therwise apprved by OPERATOR s Engineering Department), with a minimum tw-ft-six-inch (2 6 ) vertical clearance between the pipeline O.D. and the tp f the cnduit. The cable shall be placed in cnduit fr the width f the pipeline right-f-way. The cnduit shall be prtected by puring f 2000 psi cncrete, dyed red, int the ditch fr a minimum distance f five feet (5 ) n bth sides f the pipeline. The cncrete must span the width f the ditch. A minimum tw-ft (2') vertical clearance must be maintained between the O.D. f the pipeline and the tp f the cncrete. b. Fr all electrical crssings, a drive pst with an electric cmpany marker shall be placed and maintained n each side f the pipeline right-f-way Open Trench Fiber Optic Mainline Cable Installatins 1. The cable must be installed UNDER the pipeline with a minimum tw-ft-six-inch (2 6 ) vertical clearance between the bttm f the pipeline O.D. and the tp f the fiber ptic cable. 2. Fiber ptic cables must be encased in six inches (6") f cncrete, dyed range, fr a minimum distance f five feet (5') n bth sides f the pipeline. The cncrete must span the width f the ditch. 3. Fiber ptic cmpany markers must be installed and maintained at the crssing lcatin n bth sides f the pipeline right-f-way. 4. Fr bred r HDD installatins see OPERATOR Cnstructin Guidelines Cathdic Prtectin: 1. Cathdic prtectin is emplyed t cntrl crrsin n OPERATOR pipelines and ther facilities. Cathdic prtectin may have a detrimental (interference) effect n adjacent and crssing metallic structures. 2. Pssible detrimental cathdic prtectin effects shuld be evaluated by the wner/develper with apprpriate mitigative actins taken if these effects are fund t be an issue fr the wner/develper facility. Mitigatin f detrimental effects frm cathdic prtectin is pssible by using nn-metallic structure materials, lse barrier Page 12 f 110

13 catings, bnded catings and/r bnded catings with cathdic prtectin. Hwever, it is best t cnsult with thse knwledgeable in the field f cathdic prtectin and cathdic prtectin interference mitigatin t determine what is best fr the wner/develper structure and cnfiguratin 3. If cathdic prtectin is planned fr the wner/develper structure, the OPERATOR One Call Engineering Department must be ntified t allw ur crrsin cntrl persnnel t install apprpriate test facilities and evaluate pssible detrimental cathdic prtectin effects n the OPERATOR facilities Prpsed Residential Develpment 1. Submit a full set f plans, including detail sheets, whereby OPERATOR s easement is depicted thrugh the prpsed develpment. 2. Prpsed residential layut shall be designed nt t impede access alng the OPERATOR right f way. 3. Designated equipment crssing lcatins must be identified, and prir t cnstructin, a list f equipment types and weights must be supplied s that stress calculatins can be cmpleted t ascertain what type f prtectin ver the pipeline right f way will be required t allw the crssings. 4. Prpsed landscaping, utilities, drainage, grading and radways and means f excavatin, cmpactin, blasting, and rck remval within the develpment must cmply with all restrictins stated in this dcument 5. Encraching Party shall incrprate OPERATOR s Engineering and Cnstructin Guidelines cntained, herein, int any f the Encraching Parties design and cnstructin drawings issued FOR BID purpse. All plan drawings issued either FOR BID r Fr Cnstructin will display the fllwing statement n the drawings in areas arund OPERATOR s pipeline(s). Page 13 f 110

14 1.2.6 Cnstructin Guidelines The fllwing guidelines apply t Encraching Party and any cntractrs, agents and r representatives it uses fr cnstructin activities cnducted in OPERATOR s right-f-way and/r affecting OPERATOR s pipeline(s). 1. State Law requires cntractr and excavatin cmpanies cntact the State One Call Center, prir t any excavatin activity as required by yur State Law. The natinwide telephne number fr the State One Call Center is Encraching Party shall cnduct their activities in cmpliance with OPERATOR s Cnstructin/Design Guidelines as well as any applicable agreement in place with Encraching Party 3. All OPERATOR excavatins (including lcatin by pt-hling) shall be cnducted in the fllwing manner: a. Prir t excavatin, clearly mark the area t be excavated per OPERATOR Prcedure, Perfrming Line Lcates and ntify the apprpriate One Call agency f planned excavatin activities per lcal requirements. Marking requirements (methd and clr f markings) and the One Call ntificatin requirements (timing and infrmatin) vary by state. b. Ensure all utilities and knwn appurtenances are prperly marked by the wner r lcated by the cntractr and marked fr exact lcatin prir t the excavatin. i. Acquire frm a Cmpany representative any specific third party crssing requirements. e. Perfrm Site Evaluatin nting all ptential hazards and develp an actin plan t eliminate ptential danger (Hazard Assessment). Cnduct a detailed dcumented Pre-Jb Safety Meeting and cmplete the Excavatin Wrk Permit. f. Ntify the Cntrl Center f the lcatin and type f wrk being perfrmed and, if necessary per Technical Prcedure ensure that the apprpriate pressure restrictin has been implemented prir t the excavatin f an Immediate Repair Cnditin as defined in Operatr Prcedure. This will be achieved by using an Insulated Metal Sil Prbe equipped with a blunt r ball nse prbe bar tip (ex: Mighty Prbe make r equal) t prtect individuals frm undergrund electrical ptential. Cmpany r cntract persnnel shall use extreme care during prbing t nt damage the pipeline prtective cating. After lcating the exact depth f the pipeline, the prbes (a minimum f tw prbe bars per lcatin required) will be ffset t a minimum f ne ft frm the pipeline. These prbes will be placed t the side f the line that the peratr intends t dig n (this will allw the peratr t view the angle that the pipeline is running). i. Additinal Excavatin Safety Requirements that shuld be cnsidered include Cmpetent Persn Requirements, prper slping and benching, and the need fr trench bxes. j. The peratr will cmmence digging t the side f the pipeline (maintaining an 18- inch distance until the pipeline has been expsed) until reaching a depth f tw feet belw the bttm f the pipeline. N machine wrk permitted within 18 inches f the pipeline. k. The peratr may use the back f the digging bucket (nly if deemed safe by the cmpany representative r inspectin persnnel) t crumb the sil ff the tp f the pipeline (crumbing can ccur n clser than 12-inches abve the pipeline). l. Cmpany r cntract persnnel shall remve the remaining sil (expsing pipe) by means f a shvel r ther apprved means, such as hydrvac. m. Once the pipeline is clearly visible, the excavatin f the pipeline using an excavatr may cntinue. The peratr shall exercise extreme care within the 18-inch tlerance zne fr prtectin f the pipeline and undergrund utilities at r near excavatin area. n. Under n circumstances shall the peratr be allwed t align the center f the excavatr ver the center f the pipeline. The excavatr will be aligned in such a manner that the bm f the excavatr will push straight dwn the lng axis f the pipeline maintaining a minimum f ne-ft distance frm the pipeline. If the depth f the pipeline is shallw, cnsideratin shall be given t the lad distributin ver the pipeline and the need fr ramping r plating.. At any excavatin site, all OSHA r State required equipment will be maintained. 2. Third Party Excavatins Near OPERATOR Pipelines - N third party excavatins f OPERATOR s pipelines shall be initiated until OPERATOR pipelines have been marked per the Perfrming Line Lcates prcedure. N excavatin wrk shall be cnducted within 25 feet f OPERATOR pipelines withut an OPERATOR inspectr present. The fllwing limits Page 19 f 110

All crssings handled under these rules shall be dcumented utilizing the prper OPERATOR Maintenance Frm, a cpy f which shall be attached t the apprpriate One Call ticket in the One Call Ticket Management system. The Excavatr/Lcatr Orientatin dcument shall als be cmpleted and executed by the encraching party and a cpy attached t the apprpriate One Call ticket in the Ticket Management System. 5. Secndary crssings (less than 440 vlts) must be installed with the required tw-ft (2 ) minimum clearance, placed in cnduit r cncrete fr the width f the right-f-way and a drive pst with an electric cmpany marker installed n each side f the pipeline right f way t identify the crssing. 2. Primary crssings (greater than 440 vlts) shall be installed with a minimum clearance f tw-ft-six-inch (2 6 ), placed in cnduit fr the width f the right- f-way and prtected by puring 2000psi cncrete, dyed red, int the ditch fr a minimum distance f five feet (5 ) n bth sides f the pipeline. The cncrete must span the width f the ditch. A minimum tw-ft (2 ) vertical clearance must be maintained between the O.D. f the pipeline and the tp f the cncrete. Electric cmpany marker must be installed n each side f the pipeline right f way t identify the crssing Fiber Optic Open Trench Installatins (See Engineering Guidelines) 1. The cable must be installed UNDER the pipeline with a minimum tw-ft-six inch (2 6 ) vertical clearance between the pipeline O.D. and the tp f the fiber ptic cable. 2. Fiber ptic cables must be encased in six inches (6") f cncrete, dyed range, fr a minimum distance f five feet (5') n bth sides f the pipeline. The cncrete must span the width f the ditch. 3. Fiber ptic cmpany markers must be installed and maintained at the crssing lcatin n bth sides f the pipeline right-f-way Fiber Optic Installatin by Methd f Drilling r HDD (See Cnstructin Guidelines) 1. Expsed Pipeline Supprt Expsed pipeline must be supprted if the excavatin exceeds a certain length. Un-supprted expsed pipe length varies based n pipe diameter and weld type. Refer t Attachment B fr the maximum limits a pipeline can be expsed withut supplemental supprts. Acetylene Welds - The supprt guidelines listed in Attachment B d nt apply fr acetylene-welded lines. Acetylene welds shuld always be fully supprted hrizntally and vertically during excavatin. A planned excavatin lnger than 5- feet n an acetylene welded line shuld be brught t the attentin f Engineering and/r the Area Maintenance Supervisr. When acetylene welds are expsed, weld-wraps shall be installed per the OPERATOR Welding Manual (reference listing f identified acetylene welded OPERATOR pipelines). Care shall be taken t ensure n deflectin ccurs in an acetylene weld 1. Pipeline Inspectin - Expsed pipe must be inspected prir t backfill. Cating Requirements If the external cating is damaged r remved, the external surface f the pipeline shall be cated with an apprved cating per OPERATOR Cnstructin Specificatins, The cating shall be applied and cured (if applicable) per the manufacturer's specificatins prir t backfill. The cating shall be inspected fr cating hlidays using visual inspectin r a hliday detectr, whichever is apprpriate fr the existing r repaired cating. The use f rck-shield shall be cnsidered t prtect the external pipeline cating frm rcky backfill and undergrund pipe mvement. 3. Backfill Requirements - The fllwing steps shall be taken t ensure the OPERATOR pipeline is nt damaged during the backfill peratin: a. Prir t excavatin, the apprpriate Maintenance Recrd must be filled ut. b. Excavatin and backfill material shall be free f any trash and debris. In particular, any metal bjects as they may damage cating and cause interference n ILI assessments. c. Backfill material shuld be free f rcks r any nn-sil material. If the material that came ut f the hle has been mixed with rcky material, it shuld be manually segregated using the backhe befre returning t the area within a tw-ft diameter arund the pipeline. d. Clean imprted material such as sand is acceptable. Cntaminated sils can damage the cating and r be mistaken fr a release. N cntaminated sils may be used as backfill unless apprved and dcumented by the Maintenance Supervisr and Crrsin Cntrl Supervisr. e. Place backfill arund the pipe. The material must be placed arund (nt pushed) n tp f the pipe until there are tw feet f clean fill arund the pipe. Make sure the pipe is supprted with clean fill belw the pipeline prir t placing backfill n tp f the pipe. Sandbags cnstructed f wven fabric (like burlap r wven getextile) may be used t ensure adequate pipeline supprt. Slid plastic bags r cncrete bags are nt permitted fr use. f. Ensure that the material is prperly cmpacted Refill hle in maximum g. 12-inch lifts and use hand r pwered mechanical tamper t cmpact. Cmpactin equal t undisturbed sil must be achieved. 38 In cases where municipalities r privately wned facilities are nt required t participate in an established One Call system, and the lcatin f the Excavatin appears t be in prximity t an Undergrund Facility wned/perated by a municipality r private entity, the Excavatr shuld cntact the municipality r private entity within established r custmary lcal ntificatin times t advise f the prpsed Excavatin, and request that they mark the lcatin f their Undergrund Facilities prir t the start f Excavatin. If utility cmpanies r private wners d nt prvide Psitive Respnse t a lcate request within the required timeframe, r cannt establish the exact lcatin f their Undergrund Facilities, the Prject Manager is required t ntify the apprpriate OPERATOR peratins supervisr; the peratins supervisr r designee shall make additinal attempts t ntify and request the Undergrund Facilities wner/peratr(s) t prvide a Psitive Respnse fr their Undergrund Facility prir t the start f Excavatin. If all attempts t establish a Psitive Respnse are unsuccessful, the OPERATOR Operatins supervisr may allw the Excavatin t prceed.
