2 Access, update and share health data Track your health with a complete health dashboard HealthPrint is a free online health and wellness education platform The results of your screening tests done at Dis-Chem and Clicks are automatically uploaded to HealthPrint specifically designed in-house for Medihelp members, to add value in a unique and individualised manner to the Medihelp experience. How to join Any member of Medihelp may register via the Medihelp website to join HealthPrint. After free registration and completion of the online mental and physical health questionnaires you will receive a virtual voucher redeemable at one of HealthPrint s partners. Once you ve registered online you will also be able to download the Medihelp member app which will give you access to certain HealthPrint functionalities. 1 HealthPrint allows you to view and forward a detailed record of your Medihelp claims history, screening tests as well as available biometric data to a healthcare provider Verifying available benefits and searching for providers are also made easy via HealthPrint Since the programme integrates with certain fitness devices it displays your activity results on the website

3 Run and cycle for FREE HealthPrint members enjoy FREE participation in all Medihelp-sponsored running and cycling events. 4 Special offer on Planet Fitness gym membership Planet Fitness offers HealthPrint members the following membership fees: Rate Contract term R299 x 3 months and then standard rate Local: All Planet Fitness clubs 24 months R199 x 3 months and then standard rate Local: All Just Gym clubs 24 months Included in the offer is FREE Black Tag membership which allows a partner or friend to accompany the member to the gym at no cost (valued at R149 per month). All you need to do in order to qualify for the discount is to join HealthPrint and then present your proof of HealthPrint membership (available on the HealthPrint website), your Medihelp membership card and identification document (ID) at your local Planet Fitness gym. 5 A healthy weight If your BMI is 30 and higher you can register on HealthPrint for a 12-week healthy weight loss support programme and qualify for a dietician consultation covered by Medihelp (depending on your benefit option). HealthPrint will assist you on your journey to a healthier weight A special discounted rate of R200 per year applies for members with a BMI of 30 and higher when joining Medihelp MultiSport for runners, walkers and cyclists. You also receive the club s free joining pack. You also qualify for a special offer from Planet Fitness, exclusively for HealthPrint members, for the first three months of your gym membership and get a FREE Black Tag membership for a gym partner as well as a free body assessment and a one-week free pass for all participants in the healthy weight programme. After completing your 12-week programme, you also qualify for a free Medihelp goodie bag A weekly challenge to keep you going over the 12 weeks 4

6 Pregnancy and baby programme Once you ve registered for the baby programme you will receive a special gift for your baby from Medihelp. You can collect your co-branded Medihelp/Dis-Chem benefit card from Dis-Chem and start earning double points on all your Dis-Chem purchases within the baby category (excluding feeding formula) and double double points on selected clinic services. Once you ve reached 30 weeks and earned 975 points at Dis-Chem you ll be spoiled with a baby bag filled with gifts and samples. Dis-Chem will send you a notification once you qualify and your bag has arrived in the store you selected for collection. You can look forward to regular newsletters and receiving vouchers when you reach certain milestones in your pregnancy. HealthPrint will also support you with access and information about your pregnancy benefits and the procedures you should follow to access those benefits. Children of Medihelp MultiSport members get free membership of MultiSport Kids Club and a kids MultiSport T-shirt (under 12 years). 5
