27 Technical note Etimation of body compoition uing multifrequency 8-electrode BIA method Introduction Thi ytem provide etimated value baed on the bioelectrical impedance analyi (BIA) method for fat %, fat ma, FFM, mucle ma, and bone ma uing the DXA method targeted at European and American, and total body water (TBW) value meaured by the dilution method. Select the appropriate mode baed on the body type being meaured. 1) Standard (Target: 6~99 year old) 2) Athletic (Sportmen and women, etc. who do much more exercie than average) Reliability of the analyed body compoition value for portmen, etc. who have a different body compoition from average can be improved by providing a eparate analyi mode per body type a above. Principle of the body compoition etimation baed on BIA method Multi-frequency meaurement Water contained in the human body can roughly be roughly eparated into two categorie. ICW: Intercellular water ECW: Extracellular water (i.e. intercellular water, blood, etc.) Multi-frequency meaurement can be ued to meaure thee two type of water. When the human body i likened to an electrical circuit, the cell membrane that eparate the ICW and ECW i conidered to be a condener. At low frequency, the cell membrane act like a condener and doe not allow the electricity through, o electricity doe not flow through the cell. Accordingly, impedance meaured at low frequency ha a ignificant correlation with ECW. At high frequency, the electric current flow through the cell via the cell membrane condener. Impedance in thi cae ha a ignificant correlation with TBW (total body water). ECW i etimated from the low-frequency reult while TBW i etimated from the high-frequency reult. ICW i calculated a the difference. BIA method refer to the method of etimating body compoition by meauring the body impedance. Electrolyte within the body are naturally conductive. More electrolyte are contained in mucle and blood, but fairly few in fat, etc. In other word, electricity flow more eaily through body tiue with more mucle and le fat while, converely, electricity experience more reitance in body tiue with le mucle and more fat. Uing thi fact, the component ratio of each can be etimated by meauring body impedance. BIA baed on 8-electrode method Uing the conventional 4-electrode method, an electric current i upplied from electrode near the toe of both feet and the voltage i meaured at the heel. In thi cae, current flow from one lower leg to the other via the lower abdomen, and the meaured body impedance (hereafter, impedance) i the impedance between both leg. Uing the 8-electrode method, an electric current i upplied from electrode at the end of the finger on both hand and the toe on both feet, and the voltage i meaured at the ball of the thumb on both hand and at the heel of both feet. Thi method enable meaurement of impedance per egment in other word for the whole body, right leg, left leg, right arm or left arm by witching the point at which the current i applied and the voltage i meaured. For example, to meaure the impedance of the right leg, the current flow between the right hand and leg, and the voltage between both leg i meaured. The impedance meaurement method per egment i hown below for reference. Reactance technology Reactance technology i a method that reduce fluctuation during the day when analying body compoition. The primary caue of fluctuation during the day i change in impedance value, becaue the ECW (i.e. intercellular water, blood, lymph fluid, etc.) move between the upper and lower leg and the trunk in accordance with food and drink intake, exercie, and change in poture, etc. Thi impedance value fluctuation ignificantly affect the etimation of body compoition value. In order to minimie the effect of uch fluctuation, TANITA not only ue impedance value in the conventional way, but alo adopt reactance value that can be acquired by breaking down impedance value under a regreion formula to reduce fluctuation during the day. The example on the right how an approximate 30% reduction in fluctuation during the day. Impedance meaurement method per egment of the body 52 53

28 Reliability of body compoition etimation baed on the multi-frequency 8-electrode BIA method A regreion formula derived from the multiple-regreion analyi while pecifying the height, weight, age, and impedance value between the arm and leg a variable i adopted for the fat %, fat ma, FFM, mucle ma, and total body water calculation uing the body compoition analyer in accordance with the data acquired from the DXA method and dilution method targeted at European and American. The regreion formula i created from egmental impedance baed on the data alo acquired from the DXA method for egmental fat %, fat ma, FFM, and mucle ma. The analyed body compoition value calculated uing thi regreion formula bear a cloe correlation with the calculation baed on the DXA method or dilution method, and offer high reproducibility. A mentioned above, we can tate that body compoition analyi baed on the BIA method i highly reliable. <Correlation of fat % calculated uing BIA and DXA method> <Correlation of the mucle ma calculated uing the BIA and DXA method> Factor that affect meaurement integrity The BIA method i ued to meaure impedance and calculate body compoition baed on the reult. It i known that impedance fluctuate a a reult of change in the total body water which account for about 60% by weight, change in it ditribution tatu, and body temperature. Therefore, meaurement condition need to be fixed to ue the data for tudie and when repeated meaurement are taken daily. Accordingly, we recommend that meaurement be taken on the following in order to obtain teady value. 1) Meaurement hould be taken at leat three hour after getting up, auming normal activity during thi period. (If the peron ha been edentary ince riing or travelling by car, there i a high level of impedance change.) 2) Avoid taking meaurement within three hour of eating. (Impedance tend to be lower during the three hour following a meal.) 3) If trenuou exercie i undertaken, the next meaurement hould be taken at leat 12 hour later. (Impedance tendency doe not ettle conitently in accordance with the type or level of exercie.) 4) Urinate hortly before meaurement. 5) A far a poible, meaurement hould be taken at approximately the ame time each day if done repeatedly. (Conitent timing together with meaurement of weight enable comparion of table value.) Stable value can be obtained if meaurement are taken under the above condition. The following ix condition were tipulated to obtain the regreion formula when thi equipment wa developed. 1) Prohibition on imbibing alcohol during the twelve hour before meaurement 2) Prohibition on trenuou exercie during the twelve hour before meaurement 3) Prohibition on exceive conumption of food or drink on the day before meaurement 4) Prohibition of intake of food or drink during the three hour before meaurement 5) Urination jut before meaurement 6) Avoid meaurement during mentruation (for female) DXA method Dual-energy X-ray Aborptiometry (DXA) ue two type of X-ray, and can ditinguih anatomically between bone mineral, fat, and FFM baed on the extent to which X-ray permeability differ between thee ubtance. The DXA method i an accurate meaurement method with minimum impoition compared with conventional immered weighing. It can be applied to a wide range of individual, including children and the elderly. Segmental meaurement of arm and leg are alo poible, and thi ha been ued a the tandard method for body compoition analyi thu far. Reult of the body compoition baed on DXA method (LUNAR DPX-L) Dilution method The dilution method i ued to calculate the total ma of a olvent containing a labelled ubtance by adminitering a known amount of the labelled ubtance and meauring the denity at equilibrium. Heavy water (D2O) i generally ued a the labelled ubtance to meaure total body water (TBW). TBW can be calculated becaue the TBW i conidered a dilution pace for heavy water. Sodium bromide (NaBr) ha been ued a the labelled ubtance in the method to calculate ECW. It i known that bromine (Br) i not contained in cell, and i diluted in the ECW
