Acknowledgement by Manager / Captain Date Important points to note: 1) Registration is based on first-come-first-served basis and will be closed once maximum number reached. 2) ate entries will not be entertained. 3) All participants MUST bring along their nebo, NTUC Card, NRIC Card, on the day of the competition for verification by Tournament Officials. 4) Managers / Captains shall undertake the responsibility of ensuring the eligibility of their players competing in this Tournament. If participant(s) is / are found ineligible they will be disqualified from the competition with immediate effect. 5) Managers / Captains shall undertake the responsibility of ensuring the health and wellbeing of their players competing in this Tournament. 6) Only players names that are listed in the Registration form shall be eligible to participate in the Competition. Changes to the lineup will not be accepted, once it has been submitted to the presiding Tournament Official during Captains Briefing. 7) In the absence of any teams in any of the competing rounds, a win or default (walk-over) would be awarded to the other opponent by the end of the grace period. 8) Any protest must be made on an Official Protest Form on the actual event day. A non-refundable protest fee of S$50 must be paid along with the protest form by the Manager / Captain. 9) Submission of the official registration form after the Captain s Briefing would not be included for the Tournament Draw. 10) Any withdrawals of the participating must be made before the Captain s Briefing unless otherwise stated. 11) Any changes / amendments / decisions would be made at the sole discretion of the Organising Committee.
