5 Salem Elementary Schls Safety Plan DEFINITIONS Cntract: A written agreement between the Owner and Gilbane, between Gilbane and a Subcntractr, between the Owner and Other Cntractr(s), r between Other Cntractr(s) and its (their) Subcntractr(s). Emplyer: Any cntractr, supplier, r vendr perfrming wrk under Cntract at the prject site. Prject: The premises wned by the Owner as described in the cntract between the Owner and Gilbane and/r areas and ways cntiguus theret, including any wrk sites set up by the Owner fr use by a cntractr exclusively fr the strage f material r equipment, r fr n-site fabricatin f materials t be used n the jb site, including temprary lcatins. Owner: An entity that has a cntract between themselves and Gilbane r, between themselves and the Cntractr. Cntractr: Any cmpany perfrming wrk under Cntract at the prject site. PROJECT INTRODUCTION The Salem, NH Elementary Schls prject cnsists f renvatin and additins t three, almst identical, elementary schls in Salem, NH. Each additin cnsists f apprximately 20,000 square feet f classrm, bathrm and gymnasium space. Renvatins include the abatement f flring in each f the 33,000 square ft buildings. New ceilings, rf HVAC, sprinkler, lighting, duct, painting, flring, and frnt ffice and media center recnfiguratins are included in the scpe f the prject. ADMINISTRATION MANAGEMENT POLICY STATEMENT The Salem, NH Elementary Schls Prject Safety Plan embdies the plicies and prcedures fr preventin f injury, prperty damage, fire damage and ccupatinal illness. N single feature f ur wrk is f greater imprtance. It is a jint Twn f Salem and Gilbane Building Cmpany plicy t prvide a safe place t wrk at all times and t cnduct all peratins in a manner as t prvide prtectin fr all individuals wh might cme int cntact with these peratins. The Owner's students, emplyees, Gilbane Building Cmpany emplyees, Cntractr and Subcntractr emplyees, and all thers emplyed n this site, as well as anyne wh cmes n the site fr any reasn during cnstructin, are expected t cnduct their wrk in a safe manner and are required t cmply with established safety prgrams. By cntract, every Cntractr n this site is bligated t perfrm all wrk in a safe manner. By cntract, every Cntractr n this site is bligated t cnfrm t the requirements f the Federal Occupatinal Safety and Health Act f 1970 (OSHA) and all additins and revisins theret, as well as ther applicable Federal, State and Lcal requirements and the Prject Safety Plan. All supervisry emplyees must accept their respnsibility fr the preventin f accidents and fr cnducting all peratins under their directin in a safe and efficient manner. The results f ur safety effrts will affect ur verall success in cnstructing the Prject. Our gal is accident-free wrk with the traditinal defect-free quality. We knw this is the mst efficient methd and that all individuals wrking n this Prject will subscribe t the Prject Safety Plan. It is Gilbane s intentin t cmmit itself each day t maintaining an accident-free wrkplace. There is never an acceptable reasn fr cmprmising safety. MANAGEMENT STATEMENT - STATEMENT OF FINAL AUTHORITY All persns wh cme int the wrk area, fr any reasn during cnstructin, will be required t cmply with the established safety regulatins that gvern the prject. Cntractrs are cmmitted by cntract t bserve and cmply with all applicable safety regulatins and prcedures. Each Cntractr will participate in the Prject Safety Prgram, hereafter referred t as the "Prgram". If Gilbane finds Cntractr areas f wrk r individuals being, r acting in nncmpliance with the Occupatinal Safety and Health Act f 1970 (OSHA), as amended, r any ther applicable regulatins, Gilbane shall have the authrity t rder immediate crrectin and cessatin f the nn-cmpliant ccurrence. Nn-cmpliance with Prject Safety Regulatins will be grunds fr Cntractr dismissal and/r emplyee(s) being frbidden entry nt the prject. All csts f crrectin shall be brne by the Cntractr deemed respnsible. Gilbane s decisin f respnsibility shall be final. Nthing cntained herein, hwever, shall serve t relieve the Cntractr f his liabilities and/r bligatins under the "Occupatinal Safety and Health Act f 1970" and all additins and revisins theret, as well as all ther applicable Federal, State and Lcal requirements. RESPONSIBILITIES CONSTRUCTION MANAGER Audit activities f the Trade Cntractr s safety prgram s that it cnfrms t the Safety Prgram. Prvide weekly, written site inspectins f the jb site, ntify the Trade Cntractrs f any unsafe practices and cnditins fr which they are respnsible and will cunsel them n the apprpriate crrective actins when necessary. Site inspectins shall be reviewed and discussed with the cnstructin team. Prvide all new Trade Cntractrs and their subcntractr s emplyees with a Gilbane safety rientatin befre they start wrking n site. The rientatin shall include at least a list f wrk rules, identificatin f hazardus areas, and the lcatin f MSDS sheets. This rientatin will infrm the Trade Cntractr s/ subcntractr s emplyees f hazards specific t the Owner s n site peratins. After the rientatin is cmplete, emplyees shall be required t sign a statement and cmplete an exam in rder t cnfirm that they received and understd the training. Identify the lcatin where MSDS sheets prvided frm the Trade Cntractrs/ subcntractrs can be fund fr the prject. Maintain required recrds and accident preventin materials at the jb site s that an adequate histry is maintained fr the prject. Cntrl the entrance and exit f the Trade Cntractr s/subcntractr s emplyees and visitrs t and frm the jb site. Review the Trade Cntractr s prir injury and illness lg befre awarding them wrk n site. Trade Cntractr s with pr safety recrds may be disqualified frm being awarded wrk n this prject. Review injury and first aid recrds during the prject t identify injury trends t take psitive actin t reduce r eliminate such injuries frm cntinuing t ccur n the prject. The Cnstructin Manager will examine and familiarize himself/herself with the jb site and adjacent areas frm the standpint f access and facilities regarding safety. The jb site shuld be explred with regard t installing and perating the cnstructin plan, and evaluating any difficulties that might be encuntered in cmplete executin f the wrk safely. Make frequent inspectins f the jb site s as t initiate crrective measures t eliminate unsafe practices and cnditins. The Cnstructin Manager shall immediately investigate all accidents r near miss accidents and take Page 6 f 77

7 crrective actins t help prevent reccurrence. RESPONSIBILITIES GILBANE PROJECT MANAGER The Prject Manager directs and administers the Safety Prgram n this Prject. All reprts, surveys, accident reprts and ther infrmatin relating t safety are t be submitted t the Prject Manager. Shall establish a safety rganizatin t assure the invlvement f all persnnel in the safety effrt and t prvide fr their participatin. The Prject Manager appints the Prject Superintendent, as his representative t mnitr all safety activities n the site. The Prject Manager evaluates individual subcntractr's safety perfrmance fr cmpliance with all applicable Federal, State, lcal, Gilbane's and the Owner's safety requirements. RESPONSIBILITIES GILBANE PROJECT SUPERINTENDENT The Superintendent is respnsible fr the active cntrl f the Prject Safety Plan. The Superintendent is respnsible fr planning and requiring all wrk t be dne in cmpliance with the Prject Safety Plan. The Superintendent shall cnduct weekly, written site safety reviews. The Gilbane Superintendent shall cnduct regular and frequent inspectins with the chsen Owner s representative(s) frm the GUSD. RESPONSIBILITIES CONTRACTORS Each Cntractr will cmplete the Cmpetent Persn Sign Off frm (APPENDIX D) prvided by Gilbane r prvide a listing f emplyees and their cmpetencies n the Cmpany letterhead prir t the start f wrk. A safety Crdinatr shall be assigned by each cntractr t assure the safety and health f the persnnel emplyed by their cmpany. The name f and resume fr each Cntractr's prject-site safety Crdinatr will be prvided t GILBANE fr review prir t the Cntractr starting wrk at the prject site. Cntractrs with a staff and crew f 20 r mre n site shall appint a full time safety Crdinatr. Cntractrs with a staff and crew n site f less than 20 shall anticipate that the safety aspects f this psitin will encmpass 20 hurs r mre f the wrk week and may ccasinally require full time attentin. Fr this reasn, serius cnsideratin shall be given t the ability f a superintendent r freman t simultaneusly meet the respnsibilities f bth psitins. The abve requirement als applies t all Tiered Cntractrs and Sub-Tiered Cntractrs. Each Safety Crdinatr will meet the fllwing criteria: A minimum f an OSHA 30-hur cnstructin recgnitin certificatin; be certified as a Cmpetent persn in the type f wrk being perfrmed; first Aid & CPR certified; experience in the cnstructin Industry in the type f wrk being perfrmed. Each Safety Crdinatr has the right and authrity t stp any and all hazardus wrk being perfrmed by their emplyer whenever imminent danger t life and health exists. The Safety Crdinatr must be nsite as sn as the trade cntractr mbilizes and must remain until the cmpletin f the wrk. The Safety Crdinatr fr each Cntractr will be required t be n site full time when the Cntractr emplys 20 r mre emplyees n the prject site, including tier subcntractrs. The Safety Crdinatr will be n site during all wrking hurs. When shift wrk is required, the Cntractr will prvide a secnd Safety Crdinatr. Each Safety Crdinatr has the right and authrity t stp any and all hazardus wrk being Page 7 f 77

8 perfrmed by their emplyer whenever imminent danger t life and health exists. Each safety crdinatr shall participate in accident and incident investigatin invlving their wrk and emplyees and thse f their subcntractrs. Each safety crdinatr shall: Attend safety meetings as scheduled by Gilbane. Cnduct regular and frequent inspectins fr their Cntractrs wrk areas Take immediate actin t eliminate unsafe acts and/r cnditins. Ensure that prir t the start f any wrk activity every freman has reviewed each task assignment with every affected emplyee t assure a cmprehensive understanding f the safety requirements and precautins t be taken while perfrming this wrk. Ensure with the Freman that all f the necessary guards are in place, safety equipment is prvided and all ther apprpriate actins are taken prir t starting the wrk. Ensure that apprpriate persnal prtective equipment is prvided and its use enfrced. Each safety Crdinatr shall participate in accident and incident investigatin invlving their wrk and emplyees and thse f their subcntractrs. Cntractr shall instruct each emplyee n prject site in the recgnitin and avidance f unsafe acts and/r cnditins applicable t its wrk envirnment t cntrl r eliminate injury r illness. Fr Prjects in States where the OSHA 10-Hur Hazard Recgnitin Curse is required, each Cntractr will cmplete and submit the OSHA 10 Hur Safety Awareness Training Cmpliance Affidavit (APPENDIX F) frm prvided by Gilbane. Cntractr is respnsible fr prviding and requiring the use f apprpriate persnal prtective equipment in all peratins where there is an expsure t hazardus cnditins. All recrds shall be maintained at a lcatin accessible t Gilbane. Cntractr is respnsible fr ntifying Gilbane f any hazardus chemicals r substances that are brught r cause t have been brught n prject site. Cntractr shall prvide Gilbane with a cpy f Cntractr's Hazardus Cmmunicatin Prgram, Chemical infrmatin list, and Material Safety Data Sheet(s) (MSDS) fr the chemical(s) r substance(s) intended fr use n the site. Gilbane will prvide a centrally lcated place fr this infrmatin. Cntractr is respnsible fr maintaining a cpy f Cntractr's Hazard Cmmunicatin Prgram, Chemical Infrmatin List, and Material Safety Data Sheet(s) n site fr Cntractr's wn reference and emplyee training. The prper strage, use and dispsal f wastes f any hazardus chemicals r substances are the respnsibility f Cntractr. Cntractr is respnsible fr cnfrming t OSHA and NFPA standards f fire prtectin and preventin practices. Cntractr shall als cmply with all fire and safety rules and regulatins established n the prject. If Cntractr fails t crrect safety vilatins, Gilbane will issue the Cntractr written ntificatin, utlining safety vilatins. Failure f the cntractr t abate may result in the remval f the Cntractr frm the prject site, and Gilbane s apprved bidders list, r ther apprpriate measures. Cmpliance with Federal, State, Lcal Laws and regulatins is the cntractual bligatin f Cntractrs wrking n this prject. Cnflicts between current laws r cntractual requirements shall be reslved by adhering t the mre stringent requirement. Any prject site safety regulatins which exceed the minimum standards established by OSHA shall be incrprated in Cntractr's safety prgram. The Cntractr shall ensure that its supervisrs are aware f their respnsibilities, which include: Becme familiar with the requirements f all accident preventin standards and safety rules pertaining t their jb. Be respnsible fr carrying ut the prcedures required by the prject safety plan. Page 8 f 77

9 Ensure that emplyees under their supervisin have received the initial prject safety rientatin prvided by Gilbane. Explain t all emplyees applicable safe practice rules and regulatins under their direct supervisin. Supervise the instructin and training f new emplyees either persnally r thrugh delegated experienced persns until the new emplyee satisfactrily demnstrates their ability t perfrm the wrk in a safe and efficient manner. Be respnsible fr cntinuus husekeeping in their area and fr the use and maintenance f all persnal prtective devices, equipment, and safeguards. Ntify their direct supervisr and/r the cntractr s safety Crdinatr cncerning wrk areas where they believe prtective devices are required. NOTE: Such safety devices will include, but nt limited t, the fllwing: machine guards, peratinal shields, exhaust vent hds and systems, welding shields, apprved persnal prtective equipment, autmatic stps and cntrls, barricades, railings, etc. Reprt t their wn direct supervisr all cases f emplyees wh, in their pinin, are nt qualified fr the wrk t which they have been assigned r wh engages in unsafe practices. Attend and participate in all supervisrs safety meetings. Cnduct r arrange fr weekly "tlbx" safety meetings fr all emplyees under their supervisin as required. Minutes f Tl Bx Talks are t be maintained and a cpy f each Talk is t be given t Gilbane befre end-f -shift the day given. Each Cntractr shall cmplete a Safety Task Assignment Prcess frm each day fr all wrk crews, discuss with each wrk crew n a daily basis r when nn-rutine tasks ccur and prvide a cpy t Gilbane at the end f the wrk day with their daily reprt. Reprt immediately, all accidents in which persnal injury, prperty damage r a near-hit ccurs. Shuld an accident ccur invlving a cntractr s emplyee, the Principal / Owner f the Cntractr shall attend a Principal s Meeting at the prject lcatin t review the accident. Gilbane will cnduct this meeting. Assist in accident investigatin and submit a reprt prmptly n required frms. Lessns learned frm such investigatins shall be incrprated int all future daily activities and plans f the cntractr. In the event a cntractr utilizes emplyees whse primary language is nt English, the cntractr shall prvide fr apprpriate interpretatin t assure cmplete cmprehensin. Peridically analyze wrk methds in detail fr the purpse f jb simplificatin and fr the establishment f safe wrk methds. Site safety inspectins are t be an nging prcess and dcumented at least weekly. Cntractrs shuld dcument inspectins n the Site Audit Checklist r apprved Cntractr's frm and submit t Gilbane. Ensure that all hazards created in an area as a result f wrk activities are addressed befre the crew leaves the area, including breaks r lunch. Cntractrs Prject Supervisrs will be required t attend Gilbane s Supervisr Skills Wrkshp when ffered. The training will cnsist f 2 (tw) 4 (fur) hur sessins and be taught by a designated Gilbane emplyee. This training is in supprt f Gilbane s gal f creating an Incident and Injury free culture n the jbsite. RESPONSIBILITIES - EMPLOYEES N emplyee shall be required r knwingly permitted t wrk in an unsafe envirnment except fr the purpse f making safety crrectins and then nly after prper precautins have been taken fr their prtectin. 10 Each emplyee is respnsible fr learning and abiding by thse rules and regulatins that are applicable t the assigned tasks and fr reprting bserved r anticipated hazards t their immediate Supervisr. If the hazard is nt immediately crrected, the affected emplyee will reprt the hazard t Gilbane. Reprt t their wn direct supervisr all cases f emplyees wh, in their pinin, are nt qualified fr the wrk t which they have been assigned in additin t thse wh engage in unsafe practices. All emplyees shall bserve the fllwing rules f cnduct: Curtesy: Emplyees shall bserve standards f behavir and cnduct their wrk in a manner t avid ffending any Owner emplyees r visitrs. Each individual n this Prject must be given the curtesy that wuld be extended t ne's family r best friend. Persnal Prtective Equipment: all persns while n the site will wear hard hats, eye prtectin, high visibility (flurescent yellw, green, r range nly) shirts, jackets, r vests and gd wrk bts with leather uppers and substantial sles with sufficient tread. Lightweight wrk shes with nyln, canvas, r synthetic uppers are nt acceptable. All ther persnal prtective equipment, including respiratrs r glves, as apprpriate t assigned tasks, shall be utilized in the prper manner at all times while there is expsure t the hazards. Clthing: Clthing suitable fr the weather and yur wrk shall be wrn. Trn r lse clthing, cuffs r neckwear that may be a hazard are nt allwed. Shirts must be wrn and have shrt sleeves. Pants must have legs (n shrts allwed). Clthing shall be maintained in a clean, neat and repaired fashin. Vehicles: Emplyees shall park their vehicles in designated areas. Operatin f vehicles n the prject site shall cnfrm t all lcal traffic laws. The maximum speed limit n the prject site is 15 miles per hur. Smking: Smking is permitted nly in designated areas Intxicants: Cnsumptin f alchlic beverages r cntrlled substances is nt allwed n the prject. All wrkers wh are taking physician-prescribed r ver-the-cunter medicatin must be fit fr wrk. All emplyees are specifically directed t the "Drug Plicy" which is a part f this Prject Safety Plan. Accidents: All emplyees must immediately advise their Supervisr f any injury n the prject r any nn-injury accident that invlves damage t prperty r equipment. Persnal Cnduct: Practical jkes, hrseplay, scuffling, wrestling r fighting is prhibited. Gd Husekeeping: Gd husekeeping n the prject is mandatry and every emplyee must d their part daily t minimize dust and t clean up their wrk area t keep the prject clean fr safety and efficiency. Cntrls shall be bserved which keep dirt frm being tracked int areas utside the wrkspace. Clean up methds shall fllw prescribed techniques t minimize the distributin f dust int the air. Authrized Access: Emplyees shall cnfine their activities t the areas designated as the wrk site. The emplyee s Supervisr shall btain permissin frm the apprpriate Owner representative prir t entry int any areas utside the wrk site. Fire Prtectin: Emplyees shall adhere t all fire prtectin regulatins, and shall cnduct their wrk in a manner t preserve the fire safety integrity f the building. Music. N televisins, radis, cassette tape players, CD players r MP3 type players, r ther similar persnal audi/visual devices (with r withut headphnes) are allwed. GENERAL REQUIREMENTS ACCIDENT INVESTIGATION Fr all injuries r near-hits, Gilbane is t be ntified immediately. Cpies f ALL accident reprts must Page 10 f 77

11 be filed with Gilbane immediately. It will be each Cntractr's respnsibility t cmplete the First Reprt f Injury fr his emplyees and t transmit cpies f these reprts immediately t Gilbane. Any accident r incident resulting in a lst-time injury, fatality, damage t prperty r equipment exceeding U.S. $1,000, a serius "near-hit" r the recgnitin f a ptential hazard t health and envirnment is t be investigated by a cmmittee cmprised f the fllwing, as appinted by the Prject Manager: the Prject Superintendent, the Prject Safety Crdinatr and Cntractrs Supervisr r anyne familiar with the practices invlved in the incident wh can cntribute t its analysis and make recmmendatins fr actin t prevent a reccurrence. The investigatin shall begin prmptly after the incident. Results f the investigatin and recmmendatins fr preventive actin shall be dcumented within five (5) wrkdays f the incident. If the Owner agrees, a brief news release shall be psted, fr the infrmatin f wrkers, cvering fatalities and serius ccurrences. The ccurrences are als t be discussed at the regular r special safety meetings. This investigatin and reprt shall be made immediately, but release may await any similar investigatin and reprts required by gvernmental regulatins. Gilbane shall als review first aid injuries t establish trends and practices that deviate frm wrk standards and shall reprt and take crrective actins. Gilbane shall prvide fr the Owner, in the Mnthly Prgress Reprt, a safety reprt cvering safety activities fr the preceding mnth. The reprt shall include: The accident experience, recrdable and lst time, first aid, and near hit incidents fr the mnth. The relatinship f the accident experience t the number f peple emplyed using a recgnized natinal standard fr recrdable injuries and lst time injuries. A review and summary f the safety activities, prblem areas, and cntemplated actin, including fire hazards and envirnmental hazards. ACCIDENTS PRINCIPALS MEETINGS Recrdable Accidents If a Cntractr has an OSHA Recrdable Accident n the Prject, the Cntractr s Site Management (Prject Manager and Superintendent at minimum) shall attend a meeting at the jb site t discuss the accident. The meeting will be called and chaired by the Gilbane Prject Manager and will be held within 72 hurs frm the time the accident ccurs. This meeting will be called and chaired by the Gilbane Prject Manager and will be held within 72 hurs frm the time the accident became Lst Time. COMPRESSED GAS CYLINDERS Cmpressed gas cylinders shall be secured in an upright psitin at all times by means f a nncmbustible material. Gilbane site management must apprve all gas cylinder strage areas. Strage f cmpressed gas cylinders in gang bxes is prhibited. All cmpressed gas cylinders are t be clearly labeled with the Cntractrs r users name. CONCRETE (CAST-IN-PLACE) All equipment and materials used in cncrete cnstructin and masnry wrk shall meet the applicable requirements as prescribed in ANSI-A10.9-mst recent versin, "Safety Requirements fr Cncrete Cnstructin and Masnry Wrk." 12 CONCRETE (PRECAST UNITS) The hks n all rigging used fr the erectin f precast members shall have spring-laded keepers r be mused befre the lad is raised. Rigging equipped with shake-ut style hks is prhibited fr the erectin f precast members. CONFINED SPACE ENTRY The need fr cnfined space entry has been identified fr this prject. Cntractr shall develp an entry prcedure t be used when Cntractr's emplyees are required t enter cnfined areas r spaces that are either permit required r nn-permit required. Cnfined Space entry prcedures will cnfrm t OSHA and the wner s requirements. Befre Cntractrs emplyees are permitted entry int any cnfined space, the atmsphere within shall be tested t determine the xygen level and ptential cncentratins f flammable and txic vaprs, gases, liquids, etc. Cntractr needing access t the cnfined space must furnish the testing equipment and an emplyee cmpetent in the peratin f said equipment. A cnfined space entry permit must be cmpleted and psted at the entrance t the cnfined area. Dcumentatin f apprpriate frmal training fr all invlved in the cnfined space activity (entrants, attendants, supervisr, and rescue persnnel) shall be submitted t Gilbane fr apprval prir t any entry. In spaces where mechanical retrieval devices are impractical, all entrants will wear a lifeline t assist in lcating them in the event that they have t be assisted r remved by rescue persnnel. CRANE SAFETY AND RIGGING Cntractrs whse activities require the use f cranes, derricks, and ther histing machinery shall be respnsible fr their prper set up and peratin and shall advise Gilbane prir t the arrival n-site. The Gilbane Use Permit Frm (refer t Reginal Safety Manager fr a cpy), shall be used. The Cntractr shall supply Gilbane with dcumented evidence f their Cmpetent Persn s training. Inspectin Cntractrs shall prvide Gilbane evidence f annual inspectin by a third-party independent inspectin agency nt under the cntrl r wnership f the crane wner and recgnized by the US Department f Labr, Maritime certificatin "Regulatin f Title 29, Chapter XVII, Part 1919 and r apprved by the Gilbane Reginal Safety Manager. The Cntractr must pst the mst current inspectin certificate in the cab r peratr s statin f the histing machinery and prvide Gilbane with a cpy f the inspectin certificate upn request. All repairs and/r adjustments nted n the inspectin shall be crrected prir t next use. This applies t pwer-perated equipment used in cnstructin that can hist, lwer and hrizntally mve a suspended lad. Such equipment includes, but is nt limited t: articulating cranes (such as knuckle-bm cranes); crawler cranes; flating cranes; cranes n barges; lcmtive cranes; mbile cranes (such as wheel-munted, rugh-terrain, all-terrain, cmmercial truck-munted, and bm truck cranes); multi-purpse machines when cnfigured t hist and lwer (by means f a winch r hk) and hrizntally mve a suspended lad; industrial cranes (such as carry-deck cranes); dedicated pile drivers; service/ mechanic trucks with a histing device; a crane n a mnrail; twer cranes (such as fixed jib ( hammerhead bm ), luffing bm and self-erecting); pedestal cranes; prtal cranes; verhead and gantry cranes; straddle cranes; side-bm tractrs; derricks; and variatins f such equipment. All frictin-type cranes and any cranes utilizing a lattice bm must have an independent 3 rd Party Inspectin as referenced abve that is perfrmed n the site in the cnfiguratin that the crane is intended t be used in. This inspectin must be perfrmed immediately befre the crane is put int Page 12 f 77

13 service. This certificatin will be fr each crane and lifting device and assciated rigging equipment brught nt the site. If ne year has elapsed since the last inspectin, r if the crane r its assciated rigging has sustained any incident which may have resulted in damage, the crane and its assciated rigging shall be fully re-inspected with prf f inspectin prvided t Gilbane. Daily Inspectins Shall be perfrmed by a cmpetent persn designated by the cntractr in accrdance with the manufacturer s recmmendatin and ANSI B30 Standard fr the type f crane being inspected and the mst current versin. This inspectin shall be cmpleted prir t each shift starting wrk. Operatin N wrk shall prceed withut evidence f a current annual inspectin meeting Gilbane requirements. N claims will be accepted fr lsses sustained by the cntractr fr delays caused by failure t cmply with these requirements. Cranes and ther pwered lifting devices shall be inspected by the peratr: After set up and prir t initial lift. Befre each shift. After every malfunctin. The use f a functining Anti tw-blcking devices t prevent cntact between the lad blck r verhaul ball and the bm is required n all Gilbane NENG prjects. This feature will be tested n a daily basis. (12/04) The lad line(s) will be secured t a substantial anchrage pint when wrk has been stpped r at the end f the day r shift. Operatinal Aids - Anti tw-blcking device Telescping Bm Cranes Telescpic bm cranes manufactured after February 28, 1992, shall be equipped with a device which autmatically prevents damage frm cntact between the lad blck, verhaul ball, r similar cmpnent, and the bm tip (r fixed upper blck r similar cmpnent). The device(s) must prevent such damage at all pints where tw-blcking culd ccur. Temprary alternative measures: Clearly mark the cable (s that it can easily be seen by the peratr) at a pint that will give the peratr sufficient time t stp the hist t prevent tw-blcking, and use a sptter when extending the bm Lattice Bm Cranes Lattice bm cranes manufactured after Feb 28, 1992, shall be equipped with a device that either autmatically prevents damage and lad failure frm cntact between the lad blck, verhaul ball, r similar cmpnent, and the bm tip (r fixed upper blck r similar cmpnent), r warns the peratr in time fr the peratr t prevent tw-blcking. The device(s) must prevent such damage/failure r prvide adequate warning fr all pints where tw-blcking culd ccur. Lattice bm cranes, and derricks, manufactured ne year after the effective date f this standard shall be equipped with a device which autmatically prevents damage and lad failure frm cntact between the lad blck, verhaul ball, r similar cmpnent, and the bm tip (r fixed upper blck r similar cmpnent). The device(s) must prevent such damage/failure at all pints where tw-blcking culd ccur. Exceptin. The requirements d nt apply t such lattice bm equipment when used fr dragline, clamshell (grapple), magnet, drp ball, cntainer handling, cncrete bucket, marine peratins, and pile driving wrk. Special Prcedures A lift prcedure shall be develped by the Cntractr fr the fllwing and submitted t Gilbane prir t the lift taking place: Critical Lift (defined as when lifting a lad where the weights are at r ver 75% f the rated capacity f the crane and rigging as determined by the manufacturer); Multi-Crane Lift; 100 Tns r greater Lift; Any applicatin that deviates frm the manufacturers recmmendatins; When special r unique hazards are under r adjacent t the lad at any time during the lift; When Gilbane determines such a prcedure is necessary. The Lift Prcedure will include a Hazard Analysis develped by the Cntractr and submitted t Gilbane alng with Pre-Lift meetings, which shall be held at 30 days prir t the lift, the day prir t the lift and immediately prir t the lift with the actual wrkfrce ding the lift. All cncerned parties must be present fr the meetings with minutes f the meeting recrded by Gilbane. The Lift Prcedure will include dcumentatin f calculatins which incrprates weight deductins f all rigging equipment, a lad chart fr the crane(s) that will be used, a site plan and layut sheet which will include the path f travel f the lad, swing radius prtectin and any ther necessary factrs. The Gilbane Crane Lift Plan Frm (APPENDICES H1 & H2), Critical Lift Checklist (btain frm Gilbane Reginal Safety Manager), r equivalent, shall be used. The crane peratr(s) shall be prficient in the peratin f the crane(s) and licensed in the State/City where the peratin is being perfrmed. Certificatin by the Natinal Cmmissin fr the Certificatin f Crane Operatrs (NCCO) is preferred. Recrd Keeping All recrds pertaining t crane inspectins shall be kept with the crane r in the trade cntractr s site field ffice. If during any safety inspectin, the peratr r supervisr cannt prduce the required crane inspectin sheets, the crane shall be shut dwn as sn as pssible and shall be inspected. Where crane peratrs are required t be licensed by the State where the prject is being built they shall have a current license and prvide a cpy t Gilbane when requested. Duplicates f Certificatin recrds shall be maintained n prject site by Cntractr and made available t Gilbane upn request. The cntractr shall prvide evidence f cmpetency f the peratr t Gilbane. Rigging A Cmpetent Rigger appinted by the Cntractr shall perfrm all rigging equipment. Rigging equipment shall be inspected by the Cntractrs designated Cmpetent Persn prir t its initial use n the jb site and prir t each shift thereafter t ensure that it is safe. Recrds f all inspectins will be kept n the site and shall be made available t Gilbane upn request. All rigging equipment that is defective r damages shall be immediately remved frm the prject site. The headache ball hk and all ther rigging hks, with the exceptin f shake-ut hks, must be Page 14 f 77

15 equipped with a self-clsing keeper r muse. Chain slings are prhibited frm use fr any lifting peratin unless specifically designed fr a unique applicatin. Tag Lines shall be prvided and used when histing all lads alft. All hks used fr verhead lifting shall be equipped with safety latches r alternate lifting methds such as clamps will be used. Shake-ut/srting hks may nly be used fr unlading materials frm trucks and will nt be used fr verhead lifting. DEMOBILIZATION The Prject Superintendent and each cntractr shall rganize and schedule the rderly remval f their prject site ffices and trailer facilities, the terminatin f temprary utility services, the transfer f telephne services t their ffices, and the frwarding f mail. The site shall be left in the cnditins specified by the cntract dcuments. The Prject Superintendent shall inspect the site with the Owner t verify that all permanent security and safety devices are in place and perfrming their intended functin. DEMOLITION Structural Demlitin An engineering survey shall be cmpleted befre the start f demlitin. It has been determined that underpinning f structures will be required fr this prject. All structural shring shall have stamped drawing and calculatins by a registered Prfessinal Engineer. Areas being demlished must be secured by means f barricades t prevent unauthrized persnnel frm entering the area. Subcntractrs must submit, prir t the start f cnstructin, a detailed demlitin plan t include, means and methds, related drawings, and ther relevant safety plans. Dust Cntrl All debris cntainers must be cvered befre being remved frm the cnstructin area. All temprary partitins that are installed must have a fire rating equal t that which they are replacing and at least 2-hurs in all cases. All temprary partitins shall be installed deck-t-deck and taped t prevent dust transmissin. Walk-ff mats r ther means shall be used at cnstructin entrances t prevent dust and ther freign matter frm being tracked thrughut the existing structure. Drs and entrances shall have bttm flr-sweeps installed. Where slid partitins f plywd r drywall are nt pssible, fire resistant visqueen shall be used r fireresistant tarps. All seams will be duct taped and dust prf entrances used. Apprpriate signage will be psted at cnstructin entrances. Pwered hand tls shall be f the dust cllecting type. All cncrete and blck shall be wet cut. Husekeeping must be perfrmed n a cntinuus basis. Temprary tilet facilities must be prvided with adequate hand washing facilities equipped with twels and hand sap. Interim Life Safety Measures Interim life safety measures shall be crdinated with the schl befre cnstructin starts. This shuld include re-ruting f fire escapes, signage requirements, fire exits, area mapping, and lcal fire marshal apprval f the plan. Page 15 f 77

16 Fire prtectin plan shall include Ht wrk permits, fire watch, prvisins fr prtectin when sprinklers, smke, and heat detectrs are inactive, strage f cmpressed gas cylinders. DISCIPLINE - ENFORCEMENT All cntractrs and suppliers shall participate in the prject safety plan. Shuld an eminent dangerus cnditin be discvered, all wrk in the area f danger will be stpped until crrectins are effected. Shuld Gilbane find cntractr areas f wrk r individuals being r acting in nn-cmpliance with OSHA r the Prject Safety Plan, Gilbane shall have the authrity t rder immediate crrectin f the nncmpliant ccurrence. All csts f crrectin shall be brne by the Trade cntractr deemed respnsible. If mre than ne cntractr is deemed respnsible, Gilbane's divisin f respnsibility shall be final. Nthing cntained herein, hwever, shall serve t relieve the cntractr f their liabilities and/r bligatins under OSHA as well as ther applicable Federal, State and lcal requirements as well as the Prject Safety Plan. Gilbane may withhld payment f any sums due cntractrs fr failure t fllw the Prject Safety Plan plicies and prcedures. Gilbane will issue a written, 24-hur ntice in this regard requiring immediate respnse by the cntractr. Repeated vilatins r lack f cperatin with regard t the Prject Safety Plan by emplyees f a cntractr will indicate nn-cmpliance with prvisins included in the cntract and may be reasn fr the emplyee being barred frm the prject site and/r fr terminatin f the cntractr's cntract. At rientatin, new emplyees are given their first warning: These are the rules; if yu fail t fllw them yu will receive a citatin. 1st Citatin: Ntice is sent t emplyer. Emplyee must cme in and see Gilbane t review vilatin s we can be sure the emplyee knws hw serius this citatin is and what crrective actin must be taken. A fine fr the Cntractr may be impsed. 2nd Citatin: The individual will be remved frm the prperty. A fine n the Cntractr may be impsed. This cnstitutes three (3) warnings. At this pint, this persn will be banned frm further access t the site. Immediate remval frm the prperty Citatins will result when: Any emplyee, supervisr r manager expses themselves r ther emplyees t eminent lss f life r limb. Any emplyee, supervisr r manager penly exhibits disregard, defiance r disrespect fr the safety plan. Any emplyee, supervisr r manager knwingly falsifies any investigative dcument r testimny invlved in an investigatin. Vilent physical encunters (fighting) ccur. All individuals invlved in the incident are subject t remval. Threats are made against any safety persnnel perfrming their duties. Theft r destructin f prperty ccurs. Any emplyee, supervisr r manager cnsumes, pssesses, distributes r is under the influence f alchl/drugs. 17 subject t arbitratin. DISCIPLINE FINES Gilbane Building Cmpany reserves the right t levy fines as part f the discipline prgram. (Refer t Appendix A- Table f Fines) If Fines are instituted as part f the Discipline Prgram, Gilbane Building Cmpany s Prject Manager will cllect them frm the principal f the respective cmpany at the mnthly Safety Meeting. This will be by separate check. The fines are nt t be cllected frm the individual vilatr. These funds will be used t fund a reward/incentive prgram fr thse wh wrk and are safe individuals r grups. Any mney remaining at the cmpletin f the prject will be dnated n behalf f all wrkers and cmpanies emplyed n the prject t a lcal charity. ELECTRIC TEMPORARY All electrical wrk, installatin and wire capacities shall be in accrdance with the pertinent prvisins f the Natinal Electrical Cde (mst current versin), ANSI and OSHA Standards. All 120 vlt, single phase, 15 & 20 amp temprary pwer circuits (with the exceptin f temprary lighting) shall have grund fault circuit interrupters installed. In additin all tls, crds and pwer sets shall have an assured equipment inspectin prgram maintained n quarterly basis. The clr cdes used fr identifying inspected & tested equipment n this prject are: January, February, March White April, May, June July, August, September Octber, Nvember, December Green Red Orange (NOTE: The cycle f clrs is repeated fr the next year) Prtable tls will have the apprpriate clr cde affixed t the male (plug) end fllwing inspectin. Extensin crds will have the apprpriate clr cde affixed t bth ends (plug & receptacle). The previus quarter s clr cde will be remved t avid cnfusin. When utilizing existing building pwer and nce permanent pwer is established fr new cnstructin, inline GFCIs must be used in cnjunctin with the Assured Equipment Grunding Prgram. Each Cntractr will supply in-line GFCIs fr all current carrying devices used by their wrkfrce. All necessary pen wiring must be made inaccessible t unauthrized emplyees r visitrs and nt be subject t damage. Open wiring is NOT acceptable fr temprary lighting circuits. Lighting n barricades, fences, r sidewalk cverings shall be encased in metal raceway. Temprary lighting must have guards t prevent accidental cntact with the bulb except where the bulb is deeply recessed in the reflectr. Temprary lights shall nt be suspended by the crd unless the fixture was specifically designed in that manner. Prtable electric lighting used in mist r ther hazardus lcatins such as drums, tanks, vessels, bins, bunkers, etc., shall be perated at a maximum f 12 vlts (nn-explsive). Extensin crds used with prtable tls must be f a heavy-duty 3-wire type. Flat extensin crds are prhibited. Damaged electrical crds shall nt be used. All extensin crds will be suspended seven feet (7 ) abve finish flr r wrk platfrm. Extensin crds will nt be fastened with staples, hung frm nails, r suspended by nn-insulated wire. All nn-current carrying parts f electrical equipment must be grunded r have an apprved duble- Page 17 f 77

18 insulated setup. Grunded circuits must have enugh capability t carry all currents likely t be impsed n it. Cntractr shall determine befre peratins start if there is any energized equipment r electrical circuit in the wrk area that might have risk t the wrker. Equipment and cnductrs that must be deenergized shall be identified t the Gilbane wh will arrange t de-energize the equipment under the Lckut and Tagging prcedure/system. Cntractr shall use the prject Lckut/Tagut prcedure and strictly adhere t the use f this requirement. Gilbane will mnitr adherence t the prcedure n a regular basis. Electrical tie-ins shall be cnducted nly n de-energized (lcked ut and tagged ut) systems. If a cnditin makes this prcedure impssible then a pre-task safety meeting with the Gilbane Rep is required. All such live wrk shall cnfrm t NFPA 70E. All temprary pwer panels shall have cvers installed at all times. All pen r expsed breaker spaces shall be adequately cvered, and labeled. All electrical equipment and wiring in hazardus lcatins must cnfrm t Natinal Electrical Cde standards. The frames f all cutting, welding (arc, heli-arc, gas-plasma-arc) machines shall be grunded. Fish tapes r lines made f metal r any ther cnductive medium are prhibited. Nncnductive tapes and lines will be used in their place. All temprary wiring shall be effectively grunded in accrdance with the Natinal Electrical Cde (Articles 305 and 310). All wiring used fr temprary lighting shall be in accrdance with the mst recent NEC. Defective Electrical Tls and Equipment - All electrical tls and extensin crds fund t be defective (Examples: missing r brken grund pins, expsed internal cnductrs) will immediately be rendered inperative by cutting ff the plug end r by immediately remving frm the prject. ELEVATED WORK Ladders Manufactured ladders n the prject shall cmply with the regulatins f ANSI-A (r mst recent versin), Safety Cde fr Prtable Wd Ladders r ANSI-A (r mst recent versin), Safety Cde fr Prtable Metal Ladders as required by OSHA. All ladders shall be used in the manner and fr the purpses fr which they were designed and cnstructed. All ladders used n Gilbane Prjects in the New England Regin will have a minimum rating f 1A (Extra Heavy Duty). The side rails f extensin ladders shall extend 36 inches abve the landing. When this is nt pssible, grab rails shall be installed. All ladders in use shall be tied, blcked r therwise secured t prevent accidental displacement. When wrking n/frm a ladder at elevatins greater than six (6') feet r mre abve the wrk surface, the ladder must be tied ff r therwise secured against accidental displacement; this includes stepladders. Prtable stepladders shall be secured in place by a helper hlding the ladder until it is tied ff. In additin, wrkers are required t be tied-ff using a safety harness, r utilize a different means f gaining access (i.e., scissr lift, scaffld, etc.). Prtable metal ladders shall nt be used and are prhibited frm Gilbane prjects in New England. Scafflding All emplyees erecting, using and dismantling scafflds shall be trained in the hazards present and the safe prcedures t be fllwed t eliminate expsure t thse hazards and shall be prvided with fall prtectin when 6-feet r mre abve the next lwer level. 19 All scafflds with a wrk platfrm ver fur feet in height and having a minimum hrizntal dimensin in either directin f less than frty-five inches shall have standard guardrails installed n all pen sides and ends f the platfrm. If guardrails are nt feasible, then persnal fall arrest systems shall be prvided by the Cntractr and utilized fr fall prtectin. In additin, any Baker style scaffld having a wrk platfrm in excess f six feet will be prvided with utrigger base frames. Cncrete and Masnry All equipment and materials used in cncrete cnstructin and masnry wrk shall meet the applicable requirements as prescribed in ANSI-A (r mst recent versin)"safety Requirements fr Cncrete Cnstructin and Masnry Wrk." Cncrete (Pre-cast) Prtectin frm falling will be prvided fr all wrkers expsed t a fall ptential ver 6 feet t the lwer surface. Neither a Cntrlled Access Zne nr a mnitr-type system is permitted. Stairways Upn delivery t the prject site all ffice trailers and material strage trailers shall be prvided with stairway access t all drways and shall have landings with railings which allw fr at least 20 inches f clearance in frnt f any dr swing. Stairway placement shall fllw placement f the upper flr deck, as sn as practical. Hists and Elevatrs Temprary persnnel elevatrs and material hists shall be cnstructed, installed and maintained in cmpliance with the manufacturer's instructins and the prvisins f applicable statutes and regulatins f gverning authrities. N elevatrs r hists are t be used fr the mvement f materials and persnnel until the devices have been certified and licensed by a third party inspectr qualified t apprve the equipment. N persn shall be allwed t ride n a material hist except fr the purpses f inspectins and maintenance. ELEVATED WORK - FALL PROTECTION A Fall Prtectin Plan must be develped by the cntractr fr all wrk with a fall expsure greater than 6-feet with a cpy prvided t Gilbane prir t start f wrk. When wire rpe is used as guardrails prviding fall prtectin, all cnnectins & splices shall be lp type cnnectins with a minimum f three (3) wire rpe clamps when used as a guardrail and three (3) wire rpe clamps when used as a hrizntal lifeline. Turnbuckles shall be installed at suitable intervals t maintain the required tautness f the wire rpe but in n instance less than ne per perimeter side. Wire rpe installed as part f a guardrail system shall be 3/8 galvanized wire rpe installed 43 ½ abve the walking / wrking surface. Cables shall be installed and adequately tensined t meet the requirements f OSHA Subpart M. Cld-rlled metal studs are prhibited frm use fr guardrail r handrail systems. The use f safety mnitrs in lieu f cnventinal fall prtectin, i.e. guardrails, and/r 100% tie ff, is prhibited. All anchrages fr wire rpe cable will capable f withstanding a minimum f 200 punds frce if the wire rpe is used as a guardrails system r a minimum f 5,000 punds frce per persn attached if the wire rpe is used as an anchrage fr a persnal fall arrest system. Persnal Fall Prtectin systems shall be wrn and used by all emplyees when wrking six (6') feet r mre abve the grund/flr r whenever wrking in a precarius psitin. Only full-bdy safety harnesses are apprved fr use n this prject as a part f a persnal fall prtectin system. All lanyards are t be as shrt as pssible, but in n event lnger than six (6') feet. 20 lanyards must be used unless a Self-Retracting Lanyard is in use. Wire rpe lanyards are prhibited unless apprved by Gilbane. Safety Harness and lanyard shall als be wrn and attached t the manufacturer s apprved anchrage when wrking ut f all aerial lifts, scissrs lifts, r any ther type f elevating wrk platfrm, and t vertical drp lines when wrking frm suspended scafflding. (12/04) EMERGENCY ACTION PLAN The Gilbane Prject Manager, r his designee will prvide t each Cntractr a Cnstructin Emergency Actin Plan fr evacuatin f wrk areas in the event f any emergency that wuld affect the entire site. This plan will be a separate dcument and specific fr each Prject. Each Cntractr will be respnsible fr advising their emplyees (and sub-tier Cntractrs) f the emergency signals and evacuatin rutes. Telephne numbers and addresses f the physicians, hspital and ambulance shall be cnspicuusly psted. Cntractr shall cmplete and prvide t Gilbane an "Emplyer's First Reprt f Injury" within 24 hurs f any/all incidents invlving wrk activities assciated with the prject. Cntractrs are advised t maintain their wn OSHA 300 Lg as an OSHA requirement. Cntractr shall ensure that each f its lwer-tier cntractrs meet these medical requirements. If the injured emplyee is released by the dctr fr light r restricted wrk duty, the Cntractr shall make available restricted duty wrk fr the injured emplyee. Each ccupatinal illness r injury shall be reprted immediately by Cntractr's emplyee t Cntractr's first aid attendant and Gilbane. Cntractr's first aid attendant r ther cmpetent persn shall treat the injured emplyee as ften as necessary t ensure cmplete recvery, r until a decisin is made t seek medical treatment. If the emplyee is able t return t the prject site the same day, he/she must return with a statement frm the dctr stating same and cntaining such infrmatin as date, emplyee's name, date f return t regular r restricted duty, date he/she is t return t dctr, diagnsis, signature and address f dctr. If the injured emplyee is unable t return t the prject site the same day, the emplyee wh transprted him/her shuld bring this infrmatin back t the prject site and reprt it t Gilbane. If it is necessary t call the utside medical facility, Gilbane Prject Manager shuld make this call while the injured emplyee is being transprted. Each cntractr, at the time f mbilizatin, shall deliver t Gilbane a cmplete list f the cntractr's supervisrs with the cmplete after hur s telephne numbers. The list shall be kept current and shall be updated accrdingly. Each cntractr shall insure that his field trailers and his sub-tier cntractrs' field trailers are anchred in at least three lcatins. Upn ntificatin f a Severe Weather Watch by the U. S. Weather Bureau, the fllwing actins are t be initiated. Each cntractr having n-site generatrs which are fuel-pwered is requested t ntify Gilbane f the numbers and wattage. Generatrs may be needed t prvide temprary pwer fr rescue r clean-up activities. All materials shall be secured t prevent them frm becming air brne during high winds. Particular attentin needs t be given t picking up scrap materials and hauling r cvering trash cntainers. Crawler and mbile cranes shall have bms lwered at the end f the shift. Cranes nt capable f lwering bms shall be permitted t weather-vane r free-swing. Check t assure that swinging bms will nt cntact ther bjects such as pwer lines, structures, etc. Sufficient flashlights, batteries, and bulbs shall be prvided t assigned emergency respnse persnnel. Bmb Threat When a bmb threat is received r if a suspicius article is fund, Gilbane will take the fllwing actins. Wrk shall be stpped immediately and the prject and ffice shall be evacuated f all persnnel. A cunt will be made t assure that all are present. Envirnmental Spill In the event f a spill f envirnmentally damaging materials, immediate respnse is required t prevent r minimize the impact this event will have upn the envirnment and the public welfare. All persnnel shall cntinue t bserve standard precautins fr handling the materials as detailed in the manufacturer's prduct Material Safety Data Sheet (MSDS), including the use f persnal prtective equipment. Where cnditins warrant, the cntractr shall have emergency spill cntainment supplies available fr immediate use. The fllwing general prcedures apply t the immediate respnse which must be initiated: Immediately, all persnnel in the immediate area f the release shall be alerted t the hazardus material and the nature f the immediate danger t themselves and the envirnment. As sn as pssible, Gilbane shall be ntified and requested t initiate emergency cntainment and clean up prcedures. The Lcal Fire Department shall be ntified t mbilize their hazardus materials respnse units and shall be given the necessary infrmatin regarding the materials that were released. If safe t d s, every effrt shall be made t cntain the materials within berms, by absrbent materials, r thrugh ther apprpriate means, until prper handling and dispsal persnnel may be mbilized at the site. Public Demnstratins When a public demnstratin is expected r ccurs, Gilbane will take the fllwing actins. Wrk n the prject site shall cntinue where nt encumbered by the public demnstratin; hwever wrk in the immediate area shall be stpped and all prject emplyees shall be evacuated. ENVIRONMENTAL - ASBESTOS There is a ptential fr incidentally encuntering Asbests Cntaining Materials (ACM) during the curse f the wrk. In the event that suspect ACM is encuntered, wrk shall stp immediately and the Gilbane Prject Superintendent & Manager will be ntified. Wrk shall nt resume until ACM has been abated r suspect material has been psitively identified as nn-cntaining ACM by envirnmental cnsultant. Occupatinal Safety and Health Administratin (OSHA) regulatins have been prmulgated t prtect wrkers frm expsure t airbrne asbests fibers. Under the Asbests Cntrl and Licensing Act, a cntractr must be licensed by the Department f Labr and the State in which the wrk is being perfrmed in rder t remve asbests. Ntificatin - Befre starting asbests remval wrk, the United States Envirnmental Prtectin Agency (USEPA) and the Lcal Department f Envirnmental Management must be ntified in writing by the cntractr and apprpriate permits must be n file. The fllwing Psting is Mandatry: DANGER - ASBESTOS CANCER & LUNG DISEASE HAZARD AUTHORIZED PERSONNEL ONLY Asbests Handling - The encapsulatin, remval and/r dispsal f ACM shall be perfrmed by a Cntractr licensed t d such wrk in the State in which the wrk is being perfrmed and in accrdance with all applicable Federal, State and Lcal Regulatins per apprved abatement plans where required. Wrk Practices - Asbests cntaining materials shall be wrked in a wet state sufficient t prevent the emissin f airbrne fibers in excess f the permissible expsure limits. Wrk areas are t be adequately prtected, thrugh apprpriate type enclsures, s as t ensure that n asbests cntaminated material will be permitted t leave the cntrlled area. Persnal Prtective Equipment - In instances where re-usable clthing is used, the fllwing precautins must be fllwed: Cntaminated clthes must be apprpriately bagged and labeled. Ntificatin and transprtatin t authrized laundries and haulers. All emplyees wrking in asbests remval areas shall wear apprpriate persnal prtective equipment. Cleanup - There shall be n dry sweeping f asbests material. Use flr cverings t prevent debris frm falling t lwer flrs and t speed up huse-keeping. Labeling and Waste Dispsal - Apprpriately wrded labels must be affixed t all materials, waste, debris, etc., cntaining asbests friable materials. Asbests waste and/r asbests cntaminated material must be cllected and discarded in sealed, labeled, impervius cntainers by cntractr. ENVIRONMENTAL LEAD Lead Painted Cmpnents Lead-based paint (LBP) r lead cntaining materials are either assumed r knwn t exist n the interir r exterir surfaces f this facility. As such, it is the respnsibility f any Cntractr impacting any painted surface r lead cntaining materials t prperly prtect their emplyees frm ptential elevated lead expsure by minimally cmplying with all requirements f the Occupatinal Safety & Health Administratin s (OSHA s) Lead in the Cnstructin Industry Standard (29 CFR ). A ptential surce f lead emissin is the disturbing f painted surfaces f structures and cmpnents Page 26 f 77

The primary cnsideratin when specifying wrk methds shall be the requirement t prtect wrkers frm expsure t lead abve the Permissible Expsure Limit (PEL). Further cnsideratins when specifying wrk methds shall be the effrt t reduce the release f lead int the air, water and sil, and t reduce t a minimum the generatin f debris. At all times when activities which disturb paint are in prcess, the Site cmpetent persn fr lead shall have unrestricted access t the wrk area fr inspectin, and shall have the authrity t stp wrk when the cntrl measures being utilized are nt as specified in this sectin r the OSHA Standard, if the cntrl measures are nt adequately cntrlling expsures r if ther hazards are identified which require wrk t be stpped. All air mnitring cnducted by the Site cmpetent persn fr lead r ther qualified representative shall be perfrmed in accrdance with the OSHA Standard. Cpies f all data / sampling recrds will be prvided t Gilbane upn request. Detailed and accurate recrds f all mnitring and ther relevant data used in cnducting emplyee expsure assessments shall be kept and maintained in accrdance with the OSHA Standard. Signs shall be psted in each wrk area where wrk n painted surfaces disturbs the paint in such a way s as t expse persnnel t lead cntaminated dust, debris, r lead fumes. At minimum they shall read: WARNING LEAD WORK AREA POISON NO SMOKING OR EATING All wrker prtectin requirements will, at minimum, meet the current OSHA Standard. These requirements include but are nt limited t: Signage, Barriers & Access Expsure Mnitring including Initial Expsure Respiratry Prtectin Medical Surveillance & Recrds Educatin & Training Decntaminatin & Clearance All wrk invlving lead remval r re-cating shall be cnducted in a manner that minimizes the release f lead and lead cntaining materials int the air, water, and sil. Cntractr shall prvide fr prper dispsal f waste, including EPA identificatin number, ntificatin, certificatin, manifest, etc. All waste cntainers must be leak prf and capable f being securely cvered. All waste cntainers shall be clearly labeled with weather resistant labels using indelible ink t identify Page 27 f 77

28 the type f waste they cntain. ENVIRONMENTAL - ON-SITE HAZARDS Material that is designated as a hazardus substance requires special attentin by the Cntractr and wrkers t minimize the expsure. A plan addressing the prper handling, strage and dispsal f hazardus material must be develped. Gilbane and the Owner, must be immediately ntified f any hazardus material leak r spill. Any Cntractr-caused il spills must be reprted immediately t Gilbane. There is a ptential fr existing light ballasts encuntered during the curse f the wrk t cntain plychlrinated biphenyls (PCBs). The cntractr will be required t submit a plan as stated abve fr the prper handling & dispsal f these materials if they are cnfirmed t cntain PCBs. ENVIRONMENTAL - SILICA Cntractrs shall submit their silica prtectin prgram fr review by Gilbane prir t the pre-cnstructin cnference. As a minimum the cntractr s silica prtectin prgram shall cmply with OSHA regulatins and shall address the fllwing items: Statement f the cntractr's cmmitment t prevent silicsis and t cmply with OSHA's standards. Descriptin f air mnitring t determine the silica levels generated by tasks t prvide a basis fr: Selecting engineering cntrls, Selecting respiratry prtectin, Selecting wrk practices t reduce dust, and Determining if a medical surveillance prgram is necessary. Descriptin f engineering cntrls which are prpsed fr the prject t eliminate r reduce the amunt f silica in the air and the build-up f dust n equipment and surfaces. Descriptin f persnal prtective equipment and clthing t be prvided t emplyees and changing facilities if necessitated by the level f silica dust expsure. Certificatin f training prvided t emplyees abut health effects f silica expsure, engineering cntrls and wrk practices that reduce dust, the imprtance f maintenance and gd husekeeping, as well as the prper type and fitting f respiratrs; and include a statement that the emplyee is r is nt enrlled in a medical surveillance prgram. ENVIRONMENTAL - POWERED EQUIPMENT INSIDE ENCLOSED STRUCTURES If internal cmbustin engines are used n pwered equipment in enclsed areas, the cntractr is respnsible fr mnitring the quality f breathing air fr harmful cntaminants and adequate xygen and is respnsible fr prviding adequate ventilatin. Recrds f air quality sampling will be retained n site by the Cntractr and made available t Gilbane upn request. EXCAVATION The cntractr must designate a cmpetent persn trained in sil classificatin and the recgnitin f trenching and excavatin hazards. All trenching requiring shring and thse in excess f 20 shall require a Prfessinal Engineer s review f the shring design. The Engineer will inspect the trench(s) daily and prvide written certificatin that all shring and/r trench cnstructin is safe fr wrkers t enter. Where prtective systems as defined in 29 CFR are designed by a licensed Prfessinal Engineer, wh is nt a regular Gilbane emplyee, the resulting design dcuments must be reviewed by Gilbane prir t the cmmencement f the wrk t assure that the dcuments set frth the accurate and cmplete assumptins (as set frth in the current, applicable cntract specificatins) upn which the design is based. Trenches 4 feet and ver in depth r presenting a hazard t the wrker shall be shred r walls cut back t prtect emplyees frm cave-in. All trenches and excavatins shall be prperly barricaded t prevent persns frm walking int them. When an excavatin will remain pen lnger than ne wrk shift, a barrier sufficient t prtect peple frm falling int the excavatin r erected at a minimum f 6-feet frm the excavatin in rder t warn f the fall hazard must be erected and maintained fr the time duratin that the excavatin remains pen. Apprpriate dcumentatin t meet the OSHA trenching and excavatin standards is t be maintained n site. EYE AND FACE PROTECTION Apprpriate eye prtectin meeting the requirements f ANSI Z87 (mst recent versin) with side shields are required t be wrn in a manner t prtect the eyes while in cnstructin areas at all times. In additin, apprved eye and face prtectin is required as fllws: Gggles, welding hds and shields, r face shields will be required t be prperly wrn at all times when in the area f peratins, such as when welding, burning, grinding, chipping, chemical handling, crrsive liquids r mlten materials, drilling, sawing, driving nails, pwer actuated tls, cncrete puring, tampers and gasline fueled hand perated equipment (i.e. chain saws). This sectin will als apply t thse emplyees f Cntractrs wh are assisting any wrker as an apprentice r helper. Prescriptin glasses must meet the requirements f ANSI Z87 (mst recent versin), r be cvered with ver-the-glass safety glasses r face shield. FIRE PROTECTION Cntractr shall be respnsible fr fire prtectin in its wrk and peratinal areas, including ffices, tl rms, and strage areas, twenty fur (24) hurs per day, seven days per week thrugh the duratin f this Cntract. The cntractr is respnsible fr btaining a ht wrk permit n a daily basis frm the Gilbane superintendent. The cntractr, as required by OSHA and the lcal fire prtectin cde, must prvide apprpriate fire suppressin equipment. A minimum size f 20 punds fr multi-purpse ABC chemical extinguishers is required n the prject. A mnthly Fire Extinguisher Inspectin Prgram will be implemented t insure cmpliance. A fire watch and at least ne fire extinguisher f apprpriate size & type will be prvided by the cntractr fr all Ht Wrk Operatins. Chemical extinguishers must nt be used t attempt t cl Ht Wrk. 30 Only metal safety cntainers apprved by UL and the Lcal Fire Marshall, and prperly labeled as t their cntents, are t be used fr handling and/r strage f flammable liquids in quantities mre than ne galln. A flammable liquid strage area will be maintained by the Cntractr in an area apprved by the wner & Gilbane site management. All tarpaulins and plastic used fr temprary cvers shall be f fire resistance manufacture. Cntractr must prvide a cmpetent fire watch fr after-hurs peratin f any / all temprary heaters. HAND PROTECTION Hand prtectin will be mandatry n this prject. General requirements: Emplyers shall select and require emplyees t use apprpriate hand prtectin when emplyees' hands are expsed t hazards such as thse frm skin absrptin f harmful substances; severe cuts r laceratins; severe abrasins; punctures; chemical burns; thermal burns; and harmful temperature extremes, r as deemed necessary by the Gilbane Safety Department. Selectin: Emplyers shall base the selectin f the apprpriate hand prtectin n an evaluatin f the perfrmance characteristics f the hand prtectin relative t the task(s) t be perfrmed, cnditins present, duratin f use, and the hazards and ptential hazards identified. Refer t APPENDIX C Hand Prtectin Reference fr additinal infrmatin. HAZARD COMMUNICATION PROGRAM The Occupatinal Safety and Health Act (OSHA) mandates that each emplyee ptentially expsed t hazardus chemicals must be advised f the ptential hazards and hw t guard against thse hazards. Each cntractr whse emplyees are ptentially expsed t hazardus chemicals must develp a list f all such chemicals used n the prject; gather material safety data sheets (MSDSs) fr thse materials; develp a labeling system fr all materials; and train all ptentially expsed persnnel in the hazards and their cntrls fr all listed cmpunds. These steps are utlined in detail in the fllwing material. Emplyee training fr this requirement will be dcumented and acknwledged by signatures fllwing each sessin using the acknwledgment statement fund at the end f this sectin. Material Safety Data Sheets (MSDSs) Every cntractr will be respnsible fr develpment and maintenance f a list f hazardus chemicals utilized within the prject peratins and will be further respnsible fr btaining and maintaining MSDSs fr all such hazardus chemicals. Emplyees will be allwed access t this infrmatin and the specific MSDSs fr chemicals utilized in their wrk areas. All questins relating t the prgram shuld be directed t the cntractr's superintendent r safety representative. A cpy f each MSDS will be delivered t Gilbane prir t wrk starting invlving that substance. This Prject is a multi-emplyer wrk site. A central library will be prvided fr each Cntractrs Hazard Cmmunicatin Prgram, Chemical Infrmatin List, and MSDSs fr prducts utilized n the Prject. This library will be lcated in the Gilbane Prject Office. Each Cntractr is required t update their infrmatin lcated in this ffice as sn as new infrmatin is available. Emplyee Infrmatin and Training It is the respnsibility f emplyers t train their emplyees in the requirements utlined in 29CFR Hazard Cmmunicatin. Training must be dcumented and cpies prvided t Gilbane upn request. All new and present emplyees will be given infrmatin regarding the requirements f the Chemical Hazard Cmmunicatin Prgram; the hazardus chemicals present in their wrk place; and the physical and health risks f these chemicals. This requirement may be met thrugh rientatin sessins fr new emplyees, and refreshers fr all during tlbx talks. The infrmatin and training will als include the Page 30 f 77

31 fllwing elements: The symptms f verexpsure t the chemicals. Hw t determine the hazardus presence r release f a chemical in the wrk place. Methds t reduce r prevent the expsure t hazardus chemicals, such as cntrl prcedures, wrk practices, r persnal prtective equipment. Prcedures t fllw in the event f an expsure t hazardus chemicals. The lcatin f the lg cntaining the MSDSs that applies t their wrk place and the lcatin f the written Chemical Hazard Cmmunicatin Prgram. Hw t review MSDSs t btain the hazard infrmatin fr the chemical, and hw t read the labels that are required n the chemical cntainers. When a new hazardus chemical is btained fr use, each emplyee wh culd be expsed will be given the infrmatin and training as described abve, and a cpy f the MSDSs fr the chemical will be btained and distributed t thse wh actually use the chemical in the wrk place. The MSDSs will be available t all emplyees during each wrk shift. Prper dispsal prcedures f waste materials shall be enfrced. Labeling f waste cntainers and dispsal f all hazardus materials by a licensed dispsal facility is required. Cntainer Labeling All chemical cntainers at the site must be clearly labeled as t the cntents, the hazards invlved, and the name and address f the manufacturer. All secndary cntainers f hazardus chemicals are t be clearly labeled with the same infrmatin as the riginal cntainer. Each cntractr's superintendent r safety representative shall perfrm the abve respnsibilities fr all their materials. Hazardus Nn-Rutine Tasks and Nearby Wrk In the event an emplyee is assigned t perfrm, r is assigned t wrk in an area where a hazardus task, nn-rutine t their wrk, the emplyee will be given the additinal infrmatin and training related t the hazardus chemicals which may be encuntered in the nn-rutine task. The first-line freman, cntractr superintendent, r cntractr safety representative will prvide this infrmatin and training. The infrmatin will include the specific chemical hazards f the task, the cntrls and prtective measures required, the types f persnal prtective equipment required, hw t use the equipment, the nature f ther wrk being perfrmed in r near the nn-rutine task, and what emergency prcedures are invlved with the task. Chemicals in Unlabeled Pipes, Vessels and Cntainers T ensure that emplyees wh wrk n unlabeled pipes, vessels r cntainers have been infrmed as t the hazardus materials cntained within, the fllwing plicy has been established: Prir t starting wrk n unlabeled pipes, vessels r cntainers, emplyees are t cntact their freman fr the fllwing infrmatin: Type f chemical in the pipe, vessel r cntainer. Ptential hazards. Safety precautins that shuld be taken. Audit and Review It will be the respnsibility f each cntractr's superintendent and safety representative t review the entire Hazard Cmmunicatin Prgram, and t revise and update the material cntained herein t reflect all changes in the purchase, use, strage, and handling f hazardus chemicals at the prject site. It will be the further respnsibility f the superintendent and safety representative t peridically audit that prcedures in the use f the hazardus chemicals meet the requirements as set frth in the MSDS s. HAZARD ANALYSIS Page 31 f 77

32 Prir t beginning wrk, each cntractr shall prepare a hazard analysis that defines the activities t be perfrmed and identifies the sequence f the wrk, the specific hazards, and the methds t be used t eliminate r minimize each hazard. The hazard analysis shall be submitted prir t, and will be reviewed during the pre-cnstructin meeting by Gilbane, and the cntractr s supervisrs and safety representative. The hazard analysis shall be written in a frm acceptable t Gilbane. Hazard Analysis shall be dne when the scpe f the wrk r cnditins change. Each Cntractr Freman will infrm their wrk crew f the Hazard Analysis fr their wrk activity each day prir t start f wrk r when cnditins change. Each cntractr shall submit fr review by Gilbane a site specific safety prgram which addresses all the elements f this safety plan as they will be implemented by the cntractr, its cntractrs, vendrs and suppliers. The hazard analysis will be included as an appendix t the cntractr s site-specific safety prgram. See Appendix B Sample Hazard Analysis HOUSEKEEPING Gd husekeeping practices designed t prevent fires & injuries arund prject sites are t be fllwed. During the curse f cnstructin, all debris and scrap material shall be remved frm the wrk area. Debris and ther lse materials shall nt be allwed t accumulate in stairwells. Cntainers shall be prvided fr the cllectin and separatin f waste, trash, ily and used rags and ther refuse. Metal (dumpster type) cntainers must be used and emptied prmptly. Garbage and ther waste shall be dispsed f at frequent r mre regular intervals in a manner apprved by Gilbane. Cntractr shall ntify Gilbane f any hazardus waste it will generate during perfrmance f the Wrk. Cntractr has the direct respnsibility f maintaining prper strage f these wastes while n site and will verify t Gilbane in writing that the wastes have been dispsed f in a legal manner. A cpy f the haulers manifest must be prvided t Gilbane. Cntractr shall nt pur, bury, burn, nr in any way dispse f a chemical n the wrk prject site. Cntractr shall clear all cmbustible debris t a slid waste dispsal prject site prperly licensed under the laws f the State having jurisdictin. NO OPEN BURNING OF DEBRIS, OR RUBBISH WILL BE PERMITTED ANYWHERE ON THE PROJECT SITE. Materials and supplies shall be stred in lcatins that will nt blck access-ways, and arranged t permit easy cleaning f the area. In areas where equipment might drip il r cause ther damage t the flr surface, a prtective cver f heavy gauge, flame resistant, il prf sheeting shall be prvided between the equipment and the flr surface sheeting s that n il r grease cntacts the cncrete. This requirement is applicable t bth finished and unfinished flrs. All hses, cables, extensin crds, and similar materials shall be lcated, arranged and gruped s that they will nt blck any access-way and will permit easy cleaning and maintenance. Whenever pssible, crds, leads, and hses shall be kept at least 7 ff f the grund r at whatever height is necessary t be prtected frm traffic and t eliminate tripping hazards. INCENTIVES AND AWARDS Safety awareness and recgnitin campaigns during cnstructin will include the psting f banners, psters and signs emphasizing safety awareness, the prper use f safety equipment and safe wrk practices. Recgnizing the imprtance f teamwrk in the preventin f accidents, special awards may be made t individuals wh: Page 32 f 77

33 Stp wrk f anther individual t prevent injury r crrect an unsafe cnditin fr activities utside their area f respnsibility. Submit safety imprvement suggestins related t the Prject's activities, especially thse utside their area f respnsibility. Such awards may include autmbile first-aid kits, hme size fire extinguishers, r ther useful safety theme items. A hardhat sticker may be awarded fr stpping unsafe wrk. INSPECTION AND AUDITING Purpse and Scpe T establish a basic inspectin/audit prgram fr the eliminatin f unsafe practices by emplyees and t establish a hazard free wrk envirnment fr all emplyees n the prject. Objectives T reaffirm the Trade Cntractr's basic respnsibility fr the actins f the emplyees as riginally assigned under the General Prvisin f the Occupatinal Safety and Health Act f 1970 (revised). The exercise f these respnsibilities by all prject trade cntractrs will be the effective deterrent t accidents arising frm unsafe practices and physical cnditins, that will materially enhance the cnstructin efficiency f this prject. Prcedures Cntrl will be achieved nly when each trade cntractr fulfills their cntractual and statutry respnsibilities and applies all practical steps t maintain safe and healthful wrk practices and cnditins. Prject Cntrls Cntinued mnitring/audit f the perfrmance f the Cntractr and their supervisin under this sectin will be made by Gilbane. Cntractrs will be ntified f any unsafe practices bserved. The Cntractr's safety supervisr, the Prject Safety representative and the Cnstructin Manager's field staff shall utilize the "Cnstructin Safety Survey". Selected Gilbane persnnel will als utilize the DBO2 Safety Net audit system n a nce weekly minimum basis. All unsafe cnditins will be crrected immediately. Supervisry Cntrl Cntractr Each Cntractr will be respnsible fr cnducting cntinuus daily surveys f their peratins t insure they are aware f the prbable surces f ptential injury r lss due t unsafe acts f prcedures. Planning Cntractrs must extensively plan the prcedures t be fllwed fr each peratin using Hazard Analysis prcedures and submit such plans t Gilbane. Persnnel chsen t perfrm any such planned peratin shall be thrughly briefed in all aspects f the prcedure, including emergency actins t be taken in the event f a mishap. Inspectins In additin t inspectins cnducted by Gilbane, Insurance Representatives, and each Cntractr, cnstructin activities are subject t peridic inspectin by OSHA Cmpliance Officers. Each Cntractr is required t ntify Gilbane in writing prir t starting wrk if they, by their Cmpany plicy, will require a warrant fr OSHA t inspect their wrk. Gilbane des nt require a warrant. Cntractrs shall frward cpies f any and all inspectin reprts and/r citatins received by the Page 33 f 77

34 Cntractr frm OSHA t Gilbane. All infrmatin will remain cnfidential. In the event a OSHA Cmpliance Officer visits the site, he/she will be directed t the Gilbane ffice. The apprpriate Cntractrs will then be ntified s that an Opening Cnference may be cnducted. Gilbane will rganize an inspectin party, cnsisting f bth emplyer and emplyee representatives. Ntificatin f Hazards Each Cntractr shall ntify Gilbane verbally r in writing f the existence f any hazardus cnditins, prperty, r equipment at the wrk site that are nt under the Cntractr's cntrl. Hwever, it is the Cntractr's respnsibility t take all necessary precautins against injury until crrected by the respnsible party. Equipment and Facilities All Cntractrs perating equipment and facilities used shall be, inspected, and maintained as directed by this manual; as dictated by the applicable Federal and State safety and health regulatins. In the event f cnflict, the mre stringent requirement will take precedence. INTERIM LIFE SAFETY MATTERS FOR OCCUPIED FACILITIES Specific Measures Whenever cnstructin affects the facility's ability t accmmdate ccupants (either because f disruptin f services, interruptin f nrmal peratins, r when hazards are present), it will becme necessary t implement interim life safety measures, as fllws: Ensure that all exits are clear. This includes areas directly affected as well as all ther exits. Ensure that there is free access t emergency services, that vehicles, material, etc. are nt blcking the access rute. Disabling f fire prtectin systems. A small disaster culd escalate if the fire prtectin system is nt functinal. Care shuld be given t prvide an alternate system while the primary system is ff-line. This includes scheduled maintenance, upgrade, repairs, r adding f cverage resulting in disabling system, and disabling system t allw maintenance r repairs t be cmpleted n ther systems (e.g. ht wrk). Fire alarm, detectin, and suppressin systems must nt be impaired. A temprary (but equivalent) system shall be used if the system is impaired. These temprary systems must be tested mnthly. Temprary cnstructin partitins shall be smke tight and nncmbustible. Adequate signage shall discurage casual bservers frm pening r entering the partitins. Additinal (duble) fire-fighting equipment must be prvided, as well as persnnel trained in its use. Smking is prhibited n campus, in and adjacent t all cnstructin areas. Strict enfrcement must ccur. Cnstructin site shall be kept clean and rderly. This includes material piles, debris, platfrms, and break areas. Fire drills / evacuatin drills may be cnducted at the discretin f Gilbane and the Owner. Cntractrs are required t participate in these drills. Any csts incurred by the cntractrs related t these drills are the respnsibility f the cntractrs. Hazard surveillance f sites shall be increased and dcumented. Attentin is t be given t evacuatin rutes, cnstructin areas, strage, ffice/lunch areas, and fuel strage. Whenever the safety f adjacent areas is cmprmised because f cnstructin, staff shall be infrmed. Alternate exit rutes shall be identified. Facility-wide educatin prgrams are cnducted explaining interim life safety matters and current life safety deficiencies. Page 34 f 77

35 The cnstructin site must be restricted frm all but authrized staff. Adequate signage shall be prvided. Alternate access must be prvided fr public and emergency traffic whenever disruptin ccurs. Plicy and prcedures must ensure that rads and pathways are clear f mud, debris, materials, etc. Prper ntificatin must be made t lcal authrities (fire, plice, ther) whenever life safety is diminished. Gverning bdy shall be kept apprised f status f life safety during prject. Cnstructin wrkers must be made aware f egress rutes. Cnstructin wrkers egress rutes must be inspected daily t ensure n bstacles. Effective strage, husekeeping, and debris-remval plicies and prcedures must be in place t reduce cllectin f cmbustibles in cnstructin areas. Whenever fire znes are altered, the wner s staff will be infrmed in regard t new r different life safety measures regarding their changed cmpartmentatin and fire safety. LINE BREAK Plicy - Any entry int an perating Prcess System under installatin, testing, r perating cnditins is subject t the prcedures fr line breaking. All emplyees are t be infrmed f the inherent dangers f wrking n perating prcess systems. Entries can be made nly with apprval f the Owner and Gilbane. Added hazard ptential exists when cling ccurs, vacuums that may be hlding liquids in pckets ften break withut warning and liquid is released t run t the lwest pint. Plugs (particularly slidified prcess materials) can mve and release materials after the first cnnectin has been brken. The Owner and Gilbane must agree n the lcatin f first breaks All systems must be cnsidered as having the ptential t discharge cntained energy/material frm pen ends f lines r brken flanges at any time even after the line has been drained and vented. Cautins N Cntractr may enter an perating piping system r equipment until the requirements f this prcedure are met. Systems activated fr testing purpses fall under this prcedure. Under n circumstances will any line/system be vilated ther than via the lck and tag prcedure. LOCKOUT/TAGOUT PROCEDURES The cntractr must adhere and strictly fllw either the Prject Lckut and Tagut requirements, the wner s requirements r the cntractrs wn requirements, whichever is the mst stringent. N emplyee is permitted t wrk n any energized circuit. All circuit discnnects must be lcked in the pen psitin r therwise apprpriately identified with affixed tags stating "DANGER - DO NOT ENERGIZE" r ther equivalent wrding prir t wrking n the system r equipment. Lckut Devices Only individually keyed padlcks shall be used. Padlcks are t be painted per the craft clr cde fr easier detectin and craft identificatin. A lckut device f the standard scissr type that will allw the placing f mre than ne padlck is required, when mre than ne craft is wrking n a circuit r mechanical prcess. A piece f chain r cable may be necessary t cmplete a lckut n sme valves r cntrls and shall be used wherever needed. Danger Tags Page 35 f 77

36 'Danger Tags' are nt 'Danger Signs', and shall nt be used where a sign is needed. Tw standardized Danger Tags shall be used n this prject. They are described as fllws: "DANGER - DO NOT USE": This tag must be attached t each padlck n a lckut. "UNSAFE - DO NOT USE": This tag des nt require an attachment t a padlck, but may be used if needed. This tag shall be used t identify tls, equipment, vehicles, etc. Prcedure If device, valve, switch, r piece f equipment is lcked ut, a "Danger Tag" shall be attached. N device, valve, switch r piece f equipment shall be perated with a "Danger Tag" and/r lckut attached regardless f circumstances!!! Systems cnsisting f electrical cmpnents will be checked, lcked and tagged first by electrical craft emplyee wrking n the circuit. The electrical craft will be the first lck n, and the last lck ff. Where placing f lck is nt feasible, the circuit cnductr will be discnnected frm the breaker and tagged ut. The panel cver must be f the type that will cver all breakers when clsed and must be equipped with a hasp in rder t secure a lck t prevent the panel dr frm being pened. If panel cver is f a type that cannt be lcked clsed, a cver must be secured ver the panel cver and be lcked clsed and tagged while any wrk is being perfrmed n any f thse circuits. If the abve cannt be accmplished, each circuit will be tagged ut as prescribed and an electrician will stand by the panel bard t prevent breakers frm being tampered with. This physical presence will cntinue daily until the wrk is cmplete. All "Danger Tags" must be dated and signed. Als n tag, must be the intended wrk and equipment fr which tag has been placed. If emplyees f mre than ne craft r crew are t wrk n a system, circuit, machinery, r cmpnent, the supervisr frm that craft shall place his individual lck and tag; and verify that the system, circuit, machinery r cmpnent being tagged, is indeed the system that is t be wrked n. Only the persn that placed the lck and tag shall remve them withut special authrizatin frm the Prject Manager, Cnstructin Manager r Craft Superintendent. Padlcks, Lckut Devices and "Danger Tags" shall be made available as specified abve. Padlcks shall be clr cded fr craft identificatin and shall nly be used by that craft fr lckut purpses, i.e. valves, switches, electrical cmpnents, etc. Padlcks shall be issued frm the cntractr respnsible where a sign in/ut lg will be maintained. Lcks and tags shall be issued t the fremen r supervisr respnsible fr the craft perfrming the wrk. The cntractr f each craft discipline will be respnsible fr assuring all padlcks are persnally identified, that will be used fr lck and tag purpses. The Cntractr Superintendent(s) will be respnsible fr rdering their wn craft's padlck. A master key will als be prvided. Any emplyee(s) r persn(s) fund t have remved anther's lck and/r tag will be subject t disciplinary actin up t and including dismissal frm the prject. Special Situatins When due t the nature f wrk, a supervisr wh has emplyees assigned t wrk n systems that are between cnstructin and client turnver that is t be lcked and tagged ut in rder t perfrm wrk, the belw shall be applied: Prir t the electrical freman de-energizing the system, the freman will ascertain whether system r device has been turned ver and accepted by the client; If system is signed ff, the client shall assume respnsibility fr de-energizing system and becming the tagging authrity. 37 Cntractr Electrical freman/craft jurneyman places lck and tag and tries t engage the equipment. The electrical jurneyman r lead man will meter the tagged equipment t verify that it is de-energized. Operating Facilities and Equipment All systems cvered under this sectin whether electrical, mechanical r ther, are cnsidered thse systems where n future cnstructin activity is warranted. Electrically Operated Systems Client representative r designee de-energizes system demnstrating accuracy t cnstructin electrical supervisr, then lcks and tags. Cnstructin electrical freman/jurneyman ascertains that fuses, breakers r thrws have been remved, when applicable, tags, lcks and tries system. Electrical freman/jurneyman, meters the side f the system t be wrked n t verify it is de-energized and safe. Upn cmpletin f wrk, the jurneyman remves their lck/tag and advises the cnstructin electrical supervisr. Client representative r designee clears system, remves lck and tag and re-energizes if necessary. Other Systems Plant engineer r designee de-energizes system and makes system safe. Client mechanics r designee(s) makes first break in flanges, places blanks, blinds r valves, and demnstrates that the system is empty and decntaminated. Cnstructin (Client) Crdinatr r designee verifies that the system is de-energized and tagged. Cnstructin Craft supervisr lcks, tags and tries system, surrenders the key t the jurneyman wh will then perfrm the assigned task. Upn cmpletin f wrk, the jurneyman will return the key t the assigned supervisr and tag and lck are remved. Equipment r circuits that are de-energized shall be maintained inperative at their main pwer surce and shall have lcks and tags attached t prevent accidental turn n. A staff member shall be designated frm the electrical department (Superintendent r General Freman), t assume the respnsibility, fr the remval f lcks and tags, and activatin f pwer frm the main switchgear thrugh end line cmpnent. MEETING - PRE-CONSTRUCTION The Cntractr, befre starting wrk at the prject site, shall attend a pre-cnstructin award meeting with Gilbane t understand the prject cnditins and safety requirements. A prject site tur shall be made t cnfirm the Cntractr's awareness f ptential safety hazards. Apprpriate methds, equipment, devices and material shall be prvided by the cntractr t assure a safe wrk place. The Cntractr shall prvide r develp his wn prject specific safety prgram and submit it t Gilbane fr review prir t starting wrk at the prject site. Such review shall nt relieve the Cntractr f respnsibility fr safety, nr shall such reviews be cnstrued as limiting in any manner. It is the Cntractr's bligatin t undertake any actin that may be required t establish and maintain safe Page 37 f 77

38 wrking cnditins at the prject site. MEETINGS A prject start safety cnference will be held with the superintendents, safety representative, and fremen f each new Cntractr prir t cming n the site. Gilbane will issue the prject start package infrmatin and he will issue special instructins t the Cntractrs in supprt f the Prject Safety Plan when needed. Gilbane will cnduct regularly scheduled meetings with the Superintendents, safety representatives, and fremen f new Cntractrs cming n the site and explain safety gals, cntents f this manual and therwise prvide site rientatin, safety activities and infrmatin. All Supervisrs will be required t attend this rientatin after cming n the site. Cntractr meetings will be held as necessary and as directed by Gilbane. All Cntractrs actually wrking n the Prject will have a representative at the safety meeting t maintain all safety requirements fr their trade. Gilbane will cnduct safety Meetings n a regularly scheduled basis. Minutes f the meeting will be a tpic f all scheduling and prgress meetings. All Cntractrs are required t hld weekly minutes "Tl Bx" safety meetings fr all emplyees. Tpics related t wrk assigned, and current safety prblems will be discussed. Mnthly meetings fr supervisry and clerical emplyees will be held. Gilbane will mnitr these "Tl Bx" meetings thrugh persnal attendance r by reviewing a cpy f the meeting reprt. Prir t starting any majr peratin, which wuld invlve lcking and tagging prcedures, a meeting must be set up invlving Gilbane, and every Cntractr Superintendent and every Cntractr Safety representative affected by the wrk. Specific prcedures must be adpted and reviewed by all cncerned with the peratin prir t cmmencement f the wrk. In an effrt t prmte an Incident and Injury Free envirnment peridic Principals Meetings will be cnducted at the prject t discuss safety and health issues. Prject walkarunds and wrker feedback interviews will be part f these meetings. Cntractr principals are required t attend and participate in these meetings. MOTOR VEHICLES AND EQUIPMENT All equipment must be inspected daily befre use by Cntractr's peratr. Cntractr must als make dcumented and cmplete inspectins at 30-day intervals with prper dcumentatin maintained at the prject site by Cntractr and cpies shall be made available t Gilbane upn request. Defective equipment shall be repaired r remved frm service immediately. All Cntractrs peratrs f cnstructin equipment shuld be prperly licensed and certified by a cmpetent persn. Cpies f the certificatins shall be maintained n prject site by Cntractr and made available t Gilbane upn request. Operatrs f Pwered Industrial Trucks (including LULL type frklifts) must be able t present certificatin f cmpetency f site-specific training upn request. Vehicles used t transprt emplyees shall have seats firmly secured and adequate fr the number f emplyees t be carried and all passengers shall be prperly seated with seat belt used. Standing/kneeling n the back f mving vehicles is prhibited. Lcatins fr strage f all fuels, lubricants, starting fluids, etc., shall be reviewed by Gilbane prir t use by Cntractr fr strage and shall cnfrm t the requirements f the NFPA as well as the lcal Fire Marshal. Where required, cntractrs shall prvide equipment diapers t prtect frm envirnmental spills. Drivers f mtr vehicles shall have a valid state drivers license (CDL when applicable) and be instructed t exercise judgment as well as bserve psted speed limits. Page 38 f 77

39 All cntractrs means f ingress and egress shall be adequately marked and kept clear f stred material, debris and equipment. The maximum speed limit n the prject site is 10 mph. Pedestrians always have right-f-way ver mtrized traffic. Hrns shall be sunded at blind crners, when passing, and/r fr warning. Established hand signals r turn signals are t be used. Reckless driving r ther nn-bservance f these instructins will be cause fr withdrawal f driving privileges n the prject. Any ATV's used n the prject shall be "fur"- wheeled, nt three-wheeled. All vehicles permitted access t the site must display an apprpriate vehicle identificatin badge frm the rear view mirrr r ther cnspicuus lcatin at all times while n the prject. All emplyees perating mtr vehicles and any equipment with rllver prtectin structures during perfrmance f wrk shall wear seat belts. Prperly trained and equipped flag persns shall be used whenever cnstructin traffic accesses r exits frm public highways as well as when cnstructin traffic and deliveries interfere with the planned flw f traffic n public highways. OSHA REQUIRED TRAINING Instructin and training f emplyees is a requirement f OSHA and will be enfrced n this prject. Training f cntractr persnnel is the respnsibility f the cntractr. All cntractr persnnel must attend the Gilbane New Emplyee Orientatin prir t their starting wrk n their first day n the prject. All persnnel wrking n the prject must be able t prvide prf f cmpleting the OSHA 10 hur curse n cnstructin safety. Persnnel withut a valid OSHA 10 hur card will nt be allwed n site. OSHA - INSPECTION It is Gilbane plicy t allw OSHA t cnduct an inspectin f the prject (subject t review by Gilbane Crprate Legal if necessary). If a cntractr wishes t assert their rights under the U.S. Cnstitutin regarding inspectin by OSHA, then the cntractr must s ntify OSHA prir t the start f an inspectin. Gilbane will accmpany the OSHA inspectin party at all times and will make arrangements fr the necessary meetings between OSHA, cntractrs and rganized labr representatives (if any). Gilbane des nt assume liability r respnsibility fr the presence f any alleged hazards r their crrectin. Cntractrs will infrm Gilbane f the issuance f any OSHA citatins and prvide a cpy when requested. PROJECT - CODE OF SAFE PRACTICES Each individual wrking n this prject will be required t attend a safety rientatin meeting at the start f his r her assignment. At the cnclusin f the meeting, each will be required t sign a Cde f Safe Practices (APPENDIX E), indicating their agreement t fllw that Cde while n the Prject. This des nt relieve the trade cntractr f any respnsibility t prperly rient and train their emplyees fr the specifics f their wrk. PROJECT - SAFETY RULES All persnnel n this prject, including the emplyees f Cntractr, will be required t cmply with these rules. Cntractr shall ensure and indicate that all its emplyees have read these rules and understd its cntents. The emplyee must sign a declaratin, which shall then be retained by Cntractr with the Page 39 f 77

40 emplyee's persnnel file. In additin, Cntractr shall cmply with the fllwing: Lng r shrt sleeve shirts shall be wrn at all times. All shirts shall be tucked in trusers at all times. All shirts shall be hemmed at neck, sleeve and tail. Muscle Shirts are prhibited. Lng pants are required. Shrts are prhibited. A well-cnstructed bt/she that prvides ankle prtectin with a substantial, flexible sle shall be wrn. Expsure t hazard dictates whether r nt a prtective te guard will be required. Sandals, tennis shes, r any ther street type shes, will nt be permitted. Lse fitting clthes r dangling jewelry shall nt be wrn arund mving machinery, grinding peratins, welding, r ther hazardus peratins. Hair, which culd cme in cntact with, r be caught in machinery, shall be prtected by a hardhat r hair net, as apprpriate. Apprved hard hats meeting specificatins cntained in the mst current additin f the American Natinal Standards Institute (ANSI), Z89.1 and/r Z89.2 are required. Reflective vests r clthing will be wrn by all persnnel during site wrk & excavatin phases f the prject r when deemed necessary by Gilbane. All cntractrs means f ingress and egress shall be adequately marked and kept clear f stred material, debris and equipment. The use f stilts is prhibited n this Prject. N firearms are allwed n the prject site. Practical jkes, hrseplay, scuffling, wrestling and/r fighting are prhibited and may be grunds fr immediate dismissal. Drinking and/r pssessin f intxicants n The Owner's prperty are frbidden. The use f narctics, unless authrized by a physician, and the Prject Manager/Superintendent ntified, is frbidden. Vilatin(s) f the abve will result in immediate dismissal. Gilbane Site Management reserves the right t deny access r remve frm the Prject any emplyee wh is suspected f being under the influence f alchl r ther cntrlled substance. PROTECTION OF THE PUBLIC Access t the Site N wrk shall be perfrmed in any area ccupied by the students and/r public unless specifically reviewed and permitted by Gilbane. In that the prject interfaces with the public, precautins t be taken include, but are nt limited t: Each Cntractr shall take such necessary actin as is needed t prtect and maintain public use f sidewalks, entrances t buildings, lbbies, crridrs, aisles, drs, exits and vehicular radways. The Cntractr shall prtect the students and public with apprpriate sidewalk sheds, canpies, catch platfrms, fences, guardrails, barricades, shields, and adequate visibility as required by laws and regulatins f gverning authrities. Such prtectin shall guard against flying materials, falling r mving materials and equipment, ht r pisnus materials, flammable r txic liquids and gases, pen flames, energized electric circuits r ther harmful expsures. Guardrails shall be made f rigid materials cmplying with the requirements fr standard guardrails as defined by OSHA and the Prject Safety Plan. Temprary sidewalks, ramps r stairs shall be prvided with guardrails n bth sides whenever permanent sidewalks, ramps r stairs are bstructed by the wrk. Gilbane may authrize barricades, secured against accidental displacement, meeting the requirements f lcal authrities, where fences, sheds, walkways and/r guardrails are impractical. During the perid when any barricade, fence, shed, walkway, r guardrail is remved fr the purpse f wrk, a watchman shall be placed at all penings. Page 40 f 77

41 Apprpriate warnings, signs and instructinal safety signs shall be cnspicuusly psted where necessary. In additin, a signalman shall cntrl the mving f mtrized equipment in areas where the public might be endangered. Warning lights, including lantern, trches, flares and electric lights, meeting the requirements f gverning authrities shall be prvided and maintained frm dusk t sunrise alng guardrails, barricades, temprary sidewalks and at every bstructin t the public. These warning signs and lights shall be placed at bth ends f such prtectin r bstructin and nt ver 20 feet apart alngside f such prtectin r bstructins. With respect t peratins being perfrmed n public radways, all DOT and/r municipality requirements twards public safety will be strictly bserved. Access t the site is limited t the entrance designated fr cnstructin traffic as indicated n the site plans issued with the cnstructin dcuments. At n time shuld Cntractr persnnel r vehicles t bstruct traffic n public streets r Owner entry driveways. All material deliveries shall be scheduled in advance with the Prject Superintendent and shall be cmpleted within the time segment allcated fr the specific delivery. A temprary six-ft high fence, in cmpliance with laws and regulatins f gverning authrities, shall be prvided and maintained arund the perimeter f peratins n the prject site t cntrl access t the wrk by emplyees, t prtect the public, and t restrict access by unauthrized individuals. Authrized Visitrs All visitrs t the site are required t register with Gilbane upn arrival. Each Cntractr will be expected t regulate their visitrs accrdingly. All visitr passes expire upn departure frm the site and are t be surrendered t the gate security guard. Parking Parking shall be in designated areas nly. All vehicles delivering materials t the Prject shall be authrized t d s by Gilbane. Unauthrized vehicles may be remved at the directin f Gilbane and all twing charges will be the respnsibility f the vehicle Owner. Fire hydrants and all designated fire lanes shall remain clear at all times fr the use f emergency vehicles. Emplyee Identificatin All prject site emplyees will be issued an identificatin badge and hardhat sticker upn cmpletin f their initial safety rientatin and after having passed their alchl and drug test. All persns withut a hardhat identificatin sticker shall reprt t Gilbane s ffice fr verificatin f emplyment status, attendance at an rientatin sessin, r issuance f a single day visitr pass. This identificatin badge will remain the prperty f Gilbane and the Owner. The identificatin badge shall be maintained in gd cnditin and n the persn t whm it is issued. The identificatin badge shall be returned t Gilbane r the Owner when emplyment n the Prject is terminated r when requested by Gilbane, r ther authrized and designated persn. All lst r stlen identificatin cards shall be immediately reprted t Gilbane r the Owner. In the event that an identificatin badge is lst, a replacement badge will be issued t the emplyee. The cst f a replacement badge is $10.00 which will be charged t the cntractr. Turs It is f the utmst imprtance that a high degree f prtectin be affrded all persns turing the prject site. The fllwing guidelines shall be cmplied with by persnnel wh are respnsible fr the rganizatin, directin and safe cnduct f the turs: All grup turs will be cleared thrugh the Owner's representative and Gilbane, allwing fr maximum ntice. 42 All turs will be crdinated by Gilbane t accmmdate the Prject schedule, t make necessary preparatins, and t assure safety precautins are bserved. Gilbane will review the fllwing items with the persn requesting the tur: Number f Visitrs Individual tur grups in nn-hazardus areas shuld be limited t n mre that 10 persns per tur guide (i.e. a tur grup f 20 will require at least tw tur guides). Clthing Tur grups will be required t wear apprpriate clthing (i.e. slack and lw-heeled shes). Children Children under the age f 12 will nt be permitted t accmpany turs. Each child age 12 t 15 must be accmpanied by an adult. Only thse 18 years f age and lder are permitted t wrk n the prject. Prtective equipment Hard hats, bts, raincats, eye prtectin, etc., will be supplied as required. Release and Hld Harmless Agreement Each visitr will be required t sign this frm prir t the start f the tur. In the case f children, an adult must sign fr them, preferably a parent. Immediately prir t entering the prject site, all visitrs shall be briefed abut the need fr careful and rderly cnduct, including mentin f any special hazards that may be encuntered. Technical and fficial visitr turs will be cnducted in accrdance with the abve safety precautins. Since technical turs are ften cnducted thrugh areas f mre hazardus wrk, it is recmmended that the number f peple n such turs be prprtinate t the degree f hazard invlved. RECORD KEEPING It is the Cntractrs respnsibility t maintain all recrds required by OSHA. This includes but is nt limited t the Accident Lg (Frm 300), Annual Summary (Frm 300A) and the prvisin f all psters required by current Law. A lg f safety activities including but nt limited t emplyee instructin & training, tl bx meetings, accident investigatins, etc. will be maintained n the jb site by each Cntractr and cpies shall be prmptly prvided t Gilbane upn request. SAFETY TASK ASSIGNMENT PROCESS The STA prcess must be cmpleted daily fr each task. Each supervisr will review the task t be accmplished, the expected hazards, and hw each hazard will be avided. This will be recrded n the STA by the supervisr. Each crew-member invlved with the task must sign the STA. Pst the STA in a cnspicuus lcatin thrughut the length f the task and at the end f the task, give the STA t the Prject Manager. If deviatin frm knwn safe wrk practice/prcedure ccurs, wrk must be stpped. The STA frm is available frm Gilbane site management. SANITATION Husekeeping The site, wrk areas, and all premises ccupied by Gilbane's and cntractr's persnnel will be maintained in a clean, healthy and sanitary cnditin. Wrk areas, passageways and stairs, in and arund buildings and structures, shall be kept clear f debris. Cnstructin materials shall be stred in an rderly manner. Strage areas and walkways n the site shall be maintained free f dangerus depressins, bstructins, and debris. Cnstructin equipment shall be stred r placed in an rderly manner. Page 42 f 77

43 Gd husekeeping n the prject is mandatry and every emplyee must d his part daily t minimize dust and t clean up his wrk area t keep the prject clean fr safety and efficiency. Cntrls shall be bserved which keep dirt frm being tracked int areas utside the wrkspace. Immediate cleanup is required when dust, dirt r debris may affect the wner s peratins. Eating within the cnstructin prject shall be cnfined t areas designated by Gilbane fr such purpses. Emplyees shall prperly dispse f all lunch refuse and drink cntainers in trash receptacles Failure t maintain adequate husekeeping and t perfrm daily clean-up will result in the fllwing actins: Written Ntice: Upn receipt, the cntractr shall take immediate actin t perfrm husekeeping and clean up. If having been given sufficient ntice, the cntractr fails t prvide clean up, the wrk will be perfrmed by thers, and the errant cntractr back charged fr all related csts. Daily and final clean up must be perfrmed in accrdance with cntract dcuments. Facilities The lcatins f lunch areas and emplyee tilet facilities will be designated by Gilbane and apprved by the Owner. Refuse and Garbage Each cntractr will prvide an adequate number f cvered garbage cntainers. The site will be cleaned and garbage and refuse will be cllected at least daily and remved frm the building. Ptable Water Each cntractr shall prvide ptable water at the wrk site and test it at least weekly if delivery is frm ther than municipal supplies. Sanitary facilities shall be prvided fr persnal hygiene. SCAFFOLDING The Cntractr s designated Cmpetent Persn shall inspect all scafflds prir t each wrk shift with written dcumentatin prvided t Gilbane n a daily basis. Any cntractr using scafflding shall prvide t Gilbane the name f their Cmpetent Persn alng with the cntent f the Cmpetent Persn s training prgram and prf f Scaffld User Training fr all emplyees wh may wrk n scafflding. All scaffld planking shall cmpletely cver the wrk platfrm. Only planking that has been inspected and clr-cded GREEN n the ends is permissible fr use in platfrm cnstructin. Un-inspected r damaged planking shall be clr-cded RED n the ends and cannt be used fr platfrm cnstructin. Nminal grade lumber is nt allwed fr scaffld planking. An apprved scaffld inspectin and tagging system shall be implemented and maintained. All scafflds shall bear a tag, signed and dated by the cntractr s cmpetent persn, denting that the scaffld has been inspected and is safe t use prir t any emplyee utilizing that scaffld that day. Ladder Jack Scafflds & Lean-T Scafflds are nt permitted n Gilbane Prjects. The use f the prduct referred t by many different names such as: Mini rlling scaffld, fld-up unit, fld-up scaffld, mud cart, wrkman, wrkman scaffld, painters scaffld, acustical scaffld, utility scaffld rll & fld, multi-purpse scaffld, r quick step is prhibited frm use n New England prjects. SIGNS, SIGNALS, BARRICADES AND LIGHTS (MOTOR VEHICLE EXPOSURE) Signs, signals and barricades shall be visible at all times where a hazard exists and will be in cmpliance Page 43 f 77

STEEL AND PRECAST CONCRETE ERECTION Erectin Plan A Site Specific erectin plan will be prepared by the Steel Erectin Cntractr s Qualified Persn and reviewed with the Gilbane Prject Safety Manager and Gilbane Prject Superintendent prir t start f wrk. Erectin f structural members will nt begin until the erectin plan has been submitted and reviewed. The Erectin Cntractr s Qualified Persn shall apprve all changes in the safety erectin plan. A cpy f the erectin plan shall be maintained at the prject site shwing all apprved changes with a cpy prvided t Gilbane. The implementatin f the erectin plan shall be under the supervisin f a cmpetent persn. A safe means f access t the level being wrked shall be maintained. Climbing and sliding n clumns r diagnals, is nt allwed. Cntainers, buckets, bags, etc. shall be prvided fr string r carrying blts r rivets. When blts, driftpins, r rivet heads are being remved, a means shall be prvided t prevent accidental displacement. Tls shall be secured in such a manner t prevent accidental falling. Lifeline attachments, dynamic fall restraints and ther fall prtectin prvisins shall be cnsidered during shp drawing preparatin, shall be incrprated in fabricated pieces, and shall have safety lines r devices attached prir t erectin wherever pssible. A tag line shall be used t cntrl all lads. Fr the prtectin f ther crafts n the prject, signs shall be psted in the erectin area, "Danger Men Wrking Overhead" and nly irnwrkers allwed in this area. This will include shakeut areas, erectin areas and the lad travel path frm the strage area t the erectin area. When lads are being histed, all persnnel are t be prevented frm walking under the lift. N ne shall be permitted t ride a lifting lad under any circumstances. Crane Persnnel Platfrms will nt be used fr any purpse withut the written apprval f Gilbane. Material shall nt be histed t a structure unless it is ready t be put int place and secured. Bundles f metal decking r small material shall be s secured as t prevent their falling ut frm the rigging. All metal decking will be secured in place as sn as practical but in n instance will decking remain unsecured at the end f the wrkday. Christmas treeing (multiple lifts) f n mre than three pieces is allwed. Fall Prtectin (See Elevated Wrk - Fall Prtectin) All emplyees engaged in all erectin activities including cnnecting, blting-up, decking, welding r any ther activity that expses them t a fall f 6 feet r greater shall be prvided with and use fall prtectin. Neither Cntrlled Access Znes nr Safety-mnitr systems are permitted. The exceptin cntained within OSHA Standard (b) (12) allwing fr a written fall prtectin prgram in lieu f ther requirements is nt acceptable fr NENG Prjects and is prhibited. These requirements apply t bth steel & precast cncrete erectin. Fall prtectin shall be rigrusly enfrced during steel & precast cncrete erectin. Permanent Flrs Permanent flrs shall be installed as sn as practical fllwing the erectin f structural members. 45 fundatin r uppermst-secured flr. Temprary Flring The erectin flr shall be slidly planked r decked ver its entire surface except fr access penings. Planking shall be nt less than 2 inches thick, full size undressed, and shall be laid tight and secured against mvement. Perimeter Prtectin A guardrail system f tw (2) 3/8 inch diameter 7x19 galvanized new aircraft cable shall be installed. Tp rail f the wire rpe cables shall be installed at 43 ½ inches abve the finished flr and the midrail f the wire rpe cable shall be installed apprximately halfway between the finished flr and tp guardrail apprx. 22 inches. Wire rpe guardrails shall be tensined t 2,400 punds f frce, initially, and maintained t cmply with OSHA fall prtectin requirements. Wire rpe guardrails shall be installed immediately fllwing the erectin f beams and clumns. The length f cable shall nt exceed 120 feet withut being terminated. Cables shall be terminated at all 90 degree turns and shall be lped cnnectins with 3 wire rpe clips used at all cnnectins (line splicing is nt permitted). Steel angle stanchins shall be installed and spacing n perimeter bays shall be as fllws: In bays with clumn spacing greater than 30 feet, at least tw intermediate stanchins are required. In bays with clumn spacing less than 30 feet, at least ne intermediate stanchin is required. Steel stanchins used at crners shall diagnal supprts installed t at least 80% f the height f the stanchin. Turnbuckles shall be installed n tp and midrail wire rpe cables at each perimeter side, and at intervals nt t exceed 120 feet, r as directed by Gilbane. Steel erectr must ensure that the turnbuckles are rated t at least the same strength r greater as the wire rpe cables. Lading bays shall have separate guardrail and turnbuckle assemblies installed. Interir Prtectin Installatin f guardrails at interir flr penings, i.e. stair r mechanical shafts, shall cnfrm t ne, r a cmbinatin f the fllwing: Optin 1 Install new 3/8 galvanized aircraft cable thrugh stanchins at 43 ½ inches abve the finished flr. Terminate cables at all 90 degree turns. Optin 2 Blt 2 ½ x 2 ½ x ¼ steel angles nt stanchins. A mid-stanchin / pst is required fr spans greater than 8 feet. Optin 3 Secure 2 x4 cnstructin grade lumber t steel stanchins. A mid-stanchin / pst is required every 8 feet. Guardrails shall nt be used as a hrizntal lifeline as part f a persnal fall arrest system unless designed by a Registered Prfessinal Engineer and installed under the supervisin f the steel erectr s cmpetent persn. All exterir and interir perimeter prtectin is t remain in place fr the prtectin f ther trades and will be maintained by the Erectr until it is n lnger required. Precast Leading Edge Prtectin During the erectin f precast decking, the Erectr shall maintain a barricade system a minimum f ne bay back frm the leading edge t insure ther trades will nt enter the erectin area. Stretch and Flex Prgram Purpse Sft tissue injuries are a majr surce f disabling injuries t ur wrkfrce, and result in significant csts and lst prductivity t ur industry. Warm up stretches befre wrk begins can reduce the incidence and severity f sft tissue injuries. Therefre, all cntractrs f every tier shall ensure that all emplyees participate in stretching exercises at the beginning f each wrkday. Prgram Requirements All cntractrs and tradesmen are required t design and implement a Stretch and Flex Prgram fr their emplyees. The purpse f the prgram is t gently cnditin the muscles and tendns f the wrkers befre they engage in their duties in rder t avid injury. A Stretch and Flex Prgram shall be develped by each Cntractr and submitted t Gilbane prir t cmmencing activities n site. Stretch and Flex activities shall be perfrmed every day wrk activities are scheduled and they shall be perfrmed befre the wrk activities begin. Everyne is required t participate. Recmmendatins Cnsult with a licensed Physician/Physical Trainer/Stretching Instructr/Yga Instructr fr the mst suitable stretches fr yur wrk crew. Incrprate incentives fr active participants. See Appendix E fr an example f a Stretch and Flex Prgram. Check with yur Cmpany s insurance carrier. They may prvide services, suggestins and guidance fr yur cmpany s prgram. SUBSTANCE ABUSE PROGRAM A. General 1. The CM (Cnstructin Manager) shall establish a Substance Abuse Preventin Prgram ( Prgram ) in rder t implement the drug-free wrkplace plicy enacted by the Owner. The Cntractr shall cmply with the CM Prgram as described belw. 2. The CM s Substance Abuse Preventin Prgram shall prvide fr limited testing f Cntractr persnnel fr the fllwing substances: marijuana, ccaine, piates, phencyclidine, amphetamines and alchl. 3. The CM shall administer the Prgram. The CM shall establish a drug testing sample facility nsite and/r arrange with a lcal clinic r hspital t prvide testing services. This facility must be apprved by the Carpenters Assistance Prgram. Page 46 f 77

47 The test results shall be sent frm the labratry t the Medical Review Officer (MRO) wh is a nn-cntractr emplyee. The MRO shall frward cpies f the test results t the emplyee and t the CM. If the test results are psitive, the MRO shall first hld an interview with the emplyee t determine if a legitimate cause fr the psitive result exists. If ne exits, the results shall be dcumented as negative, nt psitive. 5. The Cntractr shall identify an individual as a cntact fr the CM s crdinatr. The CM s crdinatr shall be respnsible fr frwarding the test result t that individual. All Cntractr emplyees shall execute a cnsent frm t release the reprt f the test results t the CM s crdinatr. Failure t sign the apprpriate release frms r t therwise cmply with testing prcedures shall result in a permanent bar frm the Prject. 6. The CM s prgram includes an appeal prcedure t reslve any disputes invlving the applicatin f the prgram. Each individual subject t substance abuse testing n the Prject has the right t appeal a psitive alchl r drug determinatin. The individual denied access: A. may request a re-analysis f the riginal aliqut within five (5) days after being ntified f the cnfirmed [psitive test by the Medical Review Officer (MRO). Qualificatin fr a test is nt subject t a specific cut ff requirement, but must cnfirm presence f metablite. Cst fr re-analysis is the respnsibility f the appellate, B. has five (5) wrking days t initiate the appeal after receipt f ntificatin f a cnfirmed psitive drug r alchl test, C. shall ntify the CM substance abuse preventin prgram representative f the intentin t appeal, D. must prvide a written basis fr appeal, E. may be invited t meet with the CM substance abuse preventin prgram representative and/r MRO, F. may have access t any recrds relating t his r her substance abuse test results. After denial f access, reinstatement privilege may be made after sixty (60) days, subject t CM Substance Abuse Preventin Prgram. B. Cnditinal Emplyees 1. All cnditinal emplyees fr Prject psitins shall be required t satisfactrily cmplete a drug test befre achieving status as a nn-cnditinal emplyee. Specimens shall be cllected n site during the first day f wrk. All cnditinal emplyees shall be cnsidered n the clck and paid the applicable straight time Page 47 f 77

48 rate during the specimen cllectin prcess. All cnditinal emplyees shall be cnditinally permitted access t the Prject pending receipt f final test results. 2. All cnditinal emplyees shall be ntified f test results (psitive and negative) in sealed envelpes. If test results are cnfirmed psitive fr illegal drugs r prescriptin drugs withut a valid prescriptin, the emplyee shall be barred frm the Prject immediately. Such cnditinal emplyees shall be paid fr all time wrked. NERC (New England Cuncil f Carpenters) members shall be referred t the Carpenters Assistance prgram by their Steward. 3. Any cnditinal emplyee s barred may be eligible fr re-assignment n the Prject after a perid f nt less than sixty (60) days prvided the emplyee receives a clinical evaluatin frm an apprved clinic r his/her assistance prgram and cmpletes the recmmendatins f the clinic r prgram, if any, and submits t a drug test cnducted by a Substance Abuse and Mental Health Services Administratin (SAMSHA) certified labratry at the emplyees expense. A cpy f the recrd dcumenting clinical evaluatin and recmmendatin fr treatment, if any, and stating cmpletin f the recmmendatin f the clinic alng with drug test results, must be brught t the Prject by the emplyee when returning t wrk. Upn the successful cmpletin f these requirements, the cnditinal emplyee shall be eligible fr assignment n the Prject. 4. Any cnditinal emplyee s barred fr a secnd time may be eligible fr reassignment n the Prject after a perid f nt less than ne hundred twenty (120) days prvided the emplyee receives a clinical evaluatin frm an apprved clinic and cmpletes the recmmendatins f the clinic, if any, and submits t a drug test cnducted by a Substance Abuse and Mental Health Services Administratin (SAMHSA) certified labratry at the emplyees expense. A cpy f the recrd dcumenting clinical evaluatin and recmmendatin fr treatment, if any, and stating cmpletin f the recmmendatin f the clinic alng with drug test results, must be brught t the Prject by the emplyee when returning t wrk. Upn the successful cmpletin f these requirements, the cnditinal emplyee shall be eligible fr assignment n the Prject. 5. Any cnditinal emplyee s barred fr a third time will result in a permanent bar frm the Prject. 6. Refusal n the part f any applicant t cmply with the testing prcedure shall disqualify the applicant frm cnsideratin fr emplyment n the Prject. 7. Any nn-cnditinal status emplyee wh rehires n the Prject within thirty (30) days f leaving their frmer Prject psitin shall, nt be required t re-test upn their return. All ther rehired emplyees returning t the Prject after thirty (30) days shall be required t satisfactrily cmplete a drug test befre achieving Page 48 f 77

49 status as a nn-cnditinal emplyee. C. Pst Accident Testing 1. Any emplyee wh is injured in an accident in the curse f jb duties which invlves use f vehicles, heavy equipment, pwer tls r ther dangerus instrumentalities r wrking cnditins and which results in a lst time injury may be tested fr drugs r alchl if there is reasnable cause t cnclude that drug r alchl use caused the accident. Such testing n an injured emplyee, shall be cnducted after required medical care is administered. 2. Any emplyee wh is tested after an injury shall be cnsidered n the clck at the applicable straight time r vertime rate during the specimen cllectin prcess nly. Immediately fllwing the specimen cllectin prcess, such emplyee shall be remved frm the Prject pending the results f the test. If the test results are negative, the emplyee shall be re-emplyed and prvided with full back pay. 3. If either an applicant r an injured emplyee tests psitively, such persn shall be barred frm the Prject effective the date and time f the specimen cllectin fr a perid f at least 60 days. After 60 days, such persn shall be eligible fr reemplyment, if the persn prvides prf that he/she has successfully cmpleted a drug test cnducted by Substance Abuse and Mental Health Services Administratin (SAMHSA) certified lab and a clinical evaluatin frm an apprved clinic f the Carpenters Assistance Prgram. 4. There shall be n randm testing f active emplyees. D. Test Prcedures 1. Testing prcedures, including drugs t be tested, specimen cllectin, chain f custdy and threshld and cnfirmatin test levels shall cmprt with the mandatry Guidelines fr Federal Wrkplace Testing Prgrams established by the U.S. Department f Health and Human Services, as amended. Drug tests shall be cnducted nly by labratries licensed and apprved by SAMHSA. Any alchl testing shall be cnducted under prcedures apprved by Massachusetts law. Currently, tests shall nly be cnducted fr marijuana, ccaine, piates, phencyclidine, amphetamines and alchl. Befre submitting t specimen cllectin, emplyees shall be given the pprtunity, in cnfidence, t reprt any prir recent use f ver-the-cunter r prescriptin drugs. Drug tests shall be by urinalysis and shall cnsist f tw prcedures, a screen test (EMIT r equivalent) and if that is psitive, a cnfirmatin test (GC/MS r equivalent). If the screen is negative, there shall be n secnd test. 2. A designated Unin fficial assciated with the Carpenters Assistance Prgram shall have the right t visit specimen cllectin and testing facilities t determine if it cmplies with the freging prvisins. 3. Tests fr alchl shall be perfrmed using a breathalyzer test. Any test revealing a bld alchl cntent level equal t r greater than.1 percent shall be psitive. All persnnel perfrming breathalyzer tests will be certified in the peratin f such instrument/equipment. E. Test Results Page 49 f 77

50 1. Any psitive test fr drugs shall be reprted t a Medical Review Officer (MRO). The MRO shall review the test results and any disclsure made by the emplyee and shall attempt t interview the emplyee t determine if there is any physilgical r medical reasn why the result shuld nt be deemed psitive. If the MRO declares the test psitive, ntificatin shall be prvided, in writing, t the emplyee and simultaneusly t the CM Substance Abuse Prgram. The CM shall keep test results in medical cnfidence. A limited ntificatin shall be prvided the emplying Cntractr, by the crdinatr, slely reprting psitive/negative results. The emplying Cntractr shall nt have access t individual drug test files. F. Re-tests 1. In the event f a psitive drug screen, there shall be an autmatic cnfirmatin test perfrmed by the drug testing labratry at n cst t the emplyee r Cntractr. In additin, the drug test labratry shall preserve a sufficient specimen t permit independent re-testing at the request f the emplyee at his/her expense, r by the Cntractr at its expense. Retests may be cnducted by the same r any ther apprved labratry. The emplyee r Cntractr, may request a re-test within five wrking days frm ntice f a psitive test result by the MRO. Csts f retests shall be paid in advance by the requesting party. In the event the initial test is prven t be a false psitive, csts fr any retests shall be reimbursed. In any event, if the results f the retest are psitive, any further tests wuld be at the expense f the emplyee r Cntractr. G. Cnsent Frms 1. Emplyees shall execute a cnsent frm t release the reprt f test results t the CM Substance Prgram. Failure t sign the apprpriate release frms r t therwise cmply with testing prcedures shall result in a bar frm the Prject. Signing the cnsent frm shall nt waive any individual rights available under federal r state law. H. Payments 1. The Cntractr shuld assume that all cnditinal emplyees shall be paid by the Cntractr fr time required during nrmal shift times fr testing. 2. All cnditinal emplyees shall be paid straight time rates during the specimen cllectin prcess. 3. Cntractr shall give twenty-fur (24) hur ntice t the CM s that the CM can arrange appintments fr all persnnel that shall be tested under this prgram. If 24 hur ntice is nt prvided t the CM fr a wrker, that wrker shall nt be allwed t wrk n the Prject until a drug test is perfrmed. If a wrker fails t reprt fr a drug test appintment, the Cntractr shall be charged fr the cst f the appintment. I. Disciplinary Rules 1. All cntractr s disciplinary rules shall cmprt t the CM s Prject Rules and Safety Regulatins which establish the minimum disciplinary standards fr the Prject. The Cntractr s disciplinary rules fr drug related cnduct shall be as fllws: a. pssessin f illegal drugs n the jb r in a jb status results in permanent bar frm the Page 50 f 77

Eligibility fr rehire after a minimum f 60 days (1st ffense) and 120 days (2 nd ffense) is dependent upn certificatin as drug r alchl free as cnfirmed by an apprved labratry. d. any third ffense invlving an n-the-jb presence f illegal drugs r alchl results in permanent bar frm the Prject; e. pssessin, use r system presence f prescriptin drugs withut a valid prescriptin n the jb r in a jb status may result in permanent bar frm the Prject. The severity f the sanctin shall be made n a case-by-case basis Fllwing an investigatin cncerning the use f the drug. The Cntractr shall treat the recreatinal use r abuse f prescriptin drugs the same as use f illegal drugs. 2. All Cntractrs shall recgnize that emplyees have a right t privacy and that any adverse actin taken against an emplyee fr ff-duty cnduct shall take int accunt the emplyee s right t privacy and the impact f the emplyee s cnduct n his/her jb perfrmance and jb site safety. J. Testing Facility The designated Testing facility fr this Prject is: Salem Occupatinal Acute Care K. Cllectin Lcatin Salem Occupatinal Acute Care 13 Red Rf Lane, Suite 2 Salem, NH Tests will be held n TO BE DETERMINED. TEMPORARY HEAT Heater supplier and Cntractr shall prvide their cnstructin heating plan t the Gilbane Prject Manager and Reginal Safety Manager fr review and apprval. This Plan must include specificatins fr the heaters, and heater and fuel placement and strage, as well as heater maintenance, service and inspectin schedule and cmpetent persns t implement the Plan. Whenever heaters are perating during nn-wrking hurs, the Cntractr may be required t prvide a fire watch t supervise the equipment. If a fire watch is required by the client, lcal fire fficials, r by Gilbane, the cntractr fire watch persnnel must be trained by the manufacture r heater supplier in the safe peratin f the heaters. All heaters specified shall be an apprved appliance meeting r exceeding safety features utlined in apprpriate specificatins (see belw) Installatin f an appliance meeting the fllwing specificatins shall be made by a qualified technician accrding t safety measures as utlined in ANSI 10.10, NFPA 58, NFPA 54, U.L., the peratrs/manufacturers manual, and lcal cdes Page 51 f 77

THIRD PARTY INSPECTIONS In additin t visits and safety inspectins by its wn crprate r insurance representatives, Cntractr is advised that authrized third parties may inspect the Prject frm time t time. Amng thers s authrized are representatives f the Owner and/r its agent, insurance cmpanies and OSHA. Upn their prper identificatin and clearance thrugh security, they are entitled t access and curteus cnsideratin. Gilbane must be made aware f their presence upn arrival, and in any case as sn as pssible, f the purpse and results f such visits which relate t safety. TRAINING & RECORDS Instructin and training f emplyees is an OSHA requirement and, as such, will be required n this prject. Examples f such required training t be prvided by Cntractr are: Newly emplyed, prmted and/r transferred persnnel shall be verbally instructed in the safety practices required by their wrk assignments. All wrk assignments must include specific attentin t safety. "Fllw-up" mnitring is required in rder t prevent accidents. OSHA requires that emplyees perfrming specific nn-rutine tasks r perating specific equipment be trained in its usage. Training f cntractr persnnel is the respnsibility f the cntractr. Cnduct Tl Bx safety meetings fr all emplyees at least nce a week. Maintain an attendance recrd by having emplyees sign the reverse side f the Tlbx Safety Meeting Reprt, r equivalent frm. Cmplete the Reprt and submit it t the Gilbane Office within 24 hurs after each meeting. File all tlbx meeting reprts and summaries s that they are available fr review at any time during prject peratins r fr a perid f five years fllwing terminatin f the prject. It is the respnsibility f Trade Cntractr supervisin t explain the hazards invlved in an assignment t all emplyees, either individually r in a grup befre they actually begin an assigned task. This may nly require a few wrds, but in many cases it will require the actual demnstratin f hw the task can be dne safely and the pinting ut f the hazards that may be r will be encuntered in any assignment. TOWER CRANE SAFETY COORDINATION MEETING Prir t the planned erecting, dismantling r jumping f twer cranes, a Safety Crdinatin Meeting shall be cnducted with Gilbane Building Cmpany and the fllwing stakehlders as applicable. The engineer f recrd fr the crane must submit written plans and specificatins t Gilbane and the applicable state r federal agency that detail the erectin, jumping and dismantling prcedure fr the crane that is t be erected, jumped r dismantled at the site. These plans must be prepared by the licensed engineer and in cnjunctin with the licensed rigger and must be received prir t the safety crdinatin meeting. During the safety crdinatin meeting the Plan fr the Erectin, Dismantling, and Raising & Lwering f the Twer Crane ( The Plan ) shall be reviewed. TOWER CRANE ERECTION & DISMANTLING The Plan fr Erectin, Dismantling, Raising & Lwering f the Twer Crane shall include: A written jb plan which describes the intended peratin f the subject crane including specific uses f the crane and the nature and weight f anticipated lads. A site specific Jb Hazards Analysis describing the steps invlved in twer crane erectin, jumping, dismantling and peratin, the related hazards, and the cntrls t be implemented t mitigate these hazards. Verificatin by the crane emplyer that during the time perids f erectin, climbing and dismantling f the twer crane, a qualified representative f the licensed crane certifier, r safety representative fr the distributr r manufacturer will be present n site t assure that such prcesses and peratins are perfrmed in accrdance with the manufacturer s recmmendatins and any applicable state and federal safety regulatins. Verificatin that the erectin, dismantling, raising and lwering f the twer crane will be cnducted in cmpliance with the manufacturer s recmmendatin fr the specific crane. A dedicated fire watch shall be present at all welding peratins and remain fr at least 1 hur after the ht wrk has halted. Electric Arc Welding Electric welding equipment, including cables, shall meet the requirements f the Natinal Electric Cde. All arc welding and cutting cables shall be f the cmpletely insulated flexible type capable f handling the maximum current requirements f the wrk. Cables in need f repair shall nt be used. The frames f all arc welding and cutting machines shall be grunded either thrugh a third wire in the cable cnnecting the circuit cnnectr r thrugh a separate wire which is grunded at the surce f the current. All grund cnnectins shall be inspected t insure that they are mechanically strng and electrically adequate fr the required current. Welding practices shall cmply with all applicable regulatins. Wrk permits shall be btained daily, prir t any welding peratins n the site. Gas Welding r Cutting When gas cylinders are stred, mved, r transprted, the valve prtectin cap shall be in place. 82 WIRE ROPE GUARDRAILS INSTALLATION AND MAINTENANCE January 4, 2010 Gal: Install and maintain fall prtectin guardrail cables at building perimeters and interir flr penings which meet the requirements f OSHA Subpart M in a cst effective manner. Backgrund and Executive Summary: Fr many years wire rpe cables have been used as guardrails at building perimeters and interir penings. They are generally installed by the steel erectr and left in place fr the prtectin f ther trades until cnstructin prgress dictates their remval. The mst cmmn type f wire rpe used fr perimeter prtectin has been ½ 6x19 cable. The tp and mid-rail f the guardrail cables are typically run thrugh punched hles in the webs f clumns r thrugh welded nuts n tube steel clumns. In sme instances the steel erectr installs stanchins at midpints between clumn spans and in ther cases this is nt dne. It is cmmn fr a single guardrail cable t run tw hundred feet r mre and make several ninety degree turns befre being terminated. The cmmn methd f inspecting wire rpe guardrails has been t ensure that the cables are cnnected t adequate anchrages, i.e. clumns and/r stanchins with the crrect number f wire rpe clips. An inspectr wuld then lk fr any visible deflectin in the cables, and t push dwn n the cable by hand with mderate frce. If the cable did nt deflect r deflected nly minimally, it was cnsidered t be in cmpliance. Recently, OSHA has begun inspecting cables in a mre aggressive fashin by utilizing scales and applying significantly mre dwnward frce n the cables than had been part f previus standard cable inspectins. The results f these mre aggressive cable inspectins indicate that many wire rpe guardrail systems d nt meet the minimum requirements f OSHA Subpart M. Gilbane cntracted Odeh Engineers t cnduct an engineering study and structural analysis f wire rpe guardrail assemblies in an effrt t develp a safe, cst effective and OSHA cmpliant installatin f guardrail cable assemblies. Initial calculatins cnducted by Odeh Engineers Inc. determined that ½ 6x19 wire rpe has a high mdulus f elasticity which allws the cable t stretch. As a result, a tremendus amunt f tensin must be applied during installatin, s much s that it is extremely difficult t install using standard cnstructin tls and methds. This prblem is cmpunded when the cable is installed in lng runs r makes 90 degree turns. It was als determined that ½ wire rpe is mre susceptible t expansin and cntractin due t temperature fluctuatins which can lead t increased maintenance f the cables. Fr these reasns 3/8 galvanized aircraft cable, which has a lwer mdulus f elasticity and is less susceptible t temperature fluctuatins, is preferred fr perimeter fall prtectin and is the fcus f the Structural Analysis and Field Test Reprt. 1

83 The Structural Analysis and Field Test Reprt detail the initial tensin frces and installatin methds that must be applied t the cables t achieve cmpliance with OSHA Subpart M. OSHA Requirements f particular cncern: (b) (1) Guardrail systems: Tp edge height f tp rails, r equivalent guardrail system members shall be 42 inches plus r minus 3 inches abve the wrking / walking level. When cnditins warrant the height f the tp edge may exceed the 45 inch height, prvided the guardrail system meets all ther criteria f this paragraph (b) (3) Guardrail systems shall be capable f withstanding, withut failure, a frce f at least 200 punds applied within 2 inches f the tp edge, in any utward r dwnward directin, at any pint alng the tp edge (b) (4) When the 200 pund test lad specified in paragraph (b) (3) f this sectin is applied in a dwnward directin, the tp edge f the guardrail shall nt deflect t a height f less than 39 inches abve the walking / wrking level. Executin Plan: The wire rpe installatin and maintenance requirements shuld be reviewed with and purchased frm the structural steel fabricatr and/r erectr if the erectr is a separate cntract with Gilbane. The requirements utlined in the Installatin Plan shall be reviewed with the cntractr during scpe review and pre-mbilizatin meetings. Installatin Plan: Wire rpe used fr guardrails and mid-rails shall be new 3/8 7x19 galvanized aircraft cable. The tp rail f the wire rpe guardrail system shall be installed 43 ½ inches abve the finished flr. The mid-rail f the wire rpe guardrail system shall be installed apprximately halfway between the finished flr and the tp guardrail cable, i.e. 22 inches. The length f cable used shall nt exceed 120 feet withut being terminated. Cables shall be terminated at all 90 degree turns. All terminatins shall be lped cnnectins cnnected with a minimum f three wire rpe clamps. Line splices are nt permitted. 2

84 In bays with clumn spacing less than 30 feet a minimum f ne intermediate stanchin is required. In bays with clumn spacing greater than 30 feet a minimum f tw intermediate stanchins are required. If steel angles are used as stanchins at crners, they must have diagnal supprts ( knee braces / kickers ) installed t at least 80% f the height f the stanchin. Field tests have indicated that a significant pretensin lad must be applied t the cables during the initial installatin. Irnwrkers installing the cables will need t apply apprximately 2,400 punds f tensin frce t the cable during the initial installatin. Field tests have shwn that a significant amunt f tensin can be lst during the installatin f the wire rpe clips and the release f the cable puller / cme-alng device. Installatin f guardrails at interir flr penings, i.e. stair r mechanical shafts, shall cnfrm t ne, r a cmbinatin f the fllwing: Optin 1 Install 3/8 galvanized air craft cable thrugh stanchins at 43 ½ inches abve the finished flr. Terminate cables at 90 degree turns. Optin 2 Blt 2 ½ x 2 ½ x ¼ steel angles nt stanchins. A mid-stanchin / pst is required fr spans greater than 8 feet. Optin 3 Secure 2 x 4 cnstructin grade lumber t steel stanchins. A midstanchin / pst is required every 8 Nte: A mid-rail is required at 22 abve the finished flr in all three ptins. Maintenance Plan: The steel fabricatr and/r erectr shall prvide and install turnbuckles at guardrails and mid-rails at a spacing nt t exceed 120 feet. Turnbuckles shall have a breaking strength equal t r greater than the 3/8 galvanized 7x19 aircraft cable (typically 14,400 punds). The steel erectr shall prvide a minimum f turnbuckles per flr t be installed at lcatins directed by Gilbane fr the lading f materials. The cable assemblies used at lading prts shall be separate assemblies frm the rest f the perimeter prtectin n the flr. (# f turnbuckles per flr and lading prts 3

85 shuld be determined by prject staff based n the size and lgistics f the building) The steel erectr is respnsible fr the maintenance f the guardrail systems until Gilbane directs the steel erectr t remve the system. (Unless ther arrangements are made fr anther cntractr t take ver the respnsibility after initial installatin and maintenance by the steel cntractr). It is recmmend that each prject purchase a cme-alng and Havens grip t allw fr maintenance f the cables by the general cnditins trade cntractr in the event that guardrail cable maintenance is needed and the steel erectr has dembilized frm the site. Inspectin Plan: Inspectin f perimeter and exterir guardrail systems shuld be part f every DBO2 inspectin while such cnditins exist. Any deficiencies nted during the inspectin shuld be prmptly cmmunicated t the party respnsible fr maintenance fr immediate crrectin. Applying a 200 pund test lad t guardrails in field cnditins is extremely time cnsuming, cumbersme, and impractical t cnduct n a frequent basis. Since guardrail deflectin is directly related t cable tensin, the use f a Quick Check Tensin Meter prvides a relatively simple and expedient way f determining if guardrail cables have been installed with adequate tensin t meet the requirements f OSHA (b)(4). The Dilln Quick Check Tensin Meter brchure and price list is attached (prices may vary). Each Gilbane District Office shall have ne tensin meter fr use by any jbsite. Larger prjects are recmmended t purchase their wn fr use at their prject. At the end f the jb the tensin meter shall be either transferred t anther jb r turned int the Reginal/District Safety Manager. Peridic calibratin checks shall be cnducted per the manufacturer s recmmendatins. Field tests have shwn that when inspecting guardrail tensin using a tensin meter, cable tensin shall be a minimum f 1,700 punds. With the cable height at 43 ½ inches, this will maintain the cable height abve the OSHA requirement f 39 inches when a 200 pund frce is applied per OSHA Subpart M. Randm verificatin is recmmended. (it is imprtant t ensure that the cable at the tp rail is installed at 43 1/2 inches abve the finished flr). 4
