Profile drawings showing the proposed location of the fiber line installation is attached. Drainage Superintendent Jim Reeve and I will be setting up a meeting with Jeff Soetemans, Operations Manager of Execulink Telecom, to review the proposed profile drawings. ) The Bridge and Culvert Inspection Report, in accordance with the Ontario Ministry of Transportation requirements, as submitted by Spriet Associates, is attached.

Execulink is looking to extend our fiber optics from Poplar Hill into Strathroy and continue to Pik where we co-locate equipment on the communication tower. Attached are the proposed plans where we w bury fiber on your roads. Along the path we would also like to set ourselves up and offer fiber service to the r Buttery Court, and others along the way. I didn't see an application form on the website, so I just thought I'd drawings to you and I can follow up with a phone call. 16 STRUCTURES INSPECTION AND ASSESSMENT REPORT Municipality of Adelaide Metcalfe. INTRODUCTION The Roads Departments has requested an assessment of bridge and culvert structures in the Municipality of Adelaide Metcalfe in accordance with the Ontario Ministry of Transportation requirements. This report will provide documentation with respect to structure conditions, recommended maintenance, repair or replacement of such construction as well as identify the projected costs of such to establish a sustainable work program and b recommendations in accordance with the terms of reference outlined in the Request f Proposal.. OBJECTIVES This report is intended to establish a comprehensive inventory database of all bridges and culverts on municipal roads under the control of the Municipality which span approximately 3 meters or more. This inventory documents the structure condition including material an performance deficiencies. The recommended maintenance, repair, improveme replacement has been established for each structure along with associated costs for such work. In some cases further engineering study may be recommended to more accurately assess the construction and financial needs of that structure. The most important aspect of the report is to provide a short and long term planning aid which the Municipality can use to budget more accurately for capital expenditures on its bridges and culverts. 3. METHOD A detailed element by element visual inspection of each bridge/culvert was made identifying physical geometrics, structure condition, deficiencies, required addi investigations, and recommended rehabilitation/replacement works and associated estimated costs. This data along with historical information and photos was entered into an electronic inspection form in accordance with the Ontario Structure Inspection Manual (OSIM).

17 4. STRUCTURE INSPECTIONS 4. Inventory The current structure inventory was provided on maps and summaries on file at the Municipality Office. These structures have been previously monitored through bi-annual inspections. Each structure is labelled with a structure number. A copy of each map is included in the report for reference purposes. In total the Municipality has 4 bridges and 37 culverts that meet the criteria outlined above and that ar maintained by the Municipality. For the location of each structure please see the map attached to this report. 4. inspection Forms All structures in the inventory were inspected in accordance with the current Ontario Structure Inspection Manual identifying all pertinent data as indicated in Section 3. This data was entered into an electronic inspection form for each structure and a copy of these forms have been provided in bridge and culvert binder. These forms include sections on each key element of th structure and outline the required work and related costs for each element. The form establishes a priority rating for this work by separating it into three groups as follows:. Urgent. Within year 3. One to 5 years

BRIDGES The repair/rehabilitation and replacements recommended in the inspection forms were summarized to provide a summary of work recommendations with respect to bridges. All bridges in the municipality have been upgraded for roadside safety over the last few years and meet with official safety standards. Generally speaking the Adelaide Metcalfe bridges were found to be in excellent structural condition. 5. CULVERTS Adelaide Metcalfe has 37 culverts which have been identified in this report. Twenty-four of them have no deficiencies. The remainder have been found with repair needs and/or traffic safety issues.
