Cancellations & Delays Obviously our club needs snow for our trips, however sometimes Mother Nature throws so much snow that it makes traveling unsafe. If the school cancels all weekend events, then so is our trip. A make up day will be scheduled. If road conditions are bad in the morning, but show signs of improving, then we may delay our trip one or two hours. Make sure you listen very carefully to the announcements on Friday for information about our trips. Also check the website above for up to the most current information

3 2. Dress Warm It is always at least 15 colder on the mountain than at Franklin Regional. (However loose articles of clothing such as scarves are Dangerous) You can also go into any lodge to get some temporary relief from the cold (Tahoe, and Main Lodge). 3. The Proper Equipment is Suggested You must already have your skis or snowboard, Gloves, Coat, Goggles, etc before our first trip. It is now mandatory that club members wear a Helmet for their safety. 4. Storing your Skis and Boards at 7-springs Skis are stolen all the time. The safest place to store your equipment is in one of the two ski/board checks: under the Foggy Goggle or next to the rental shop. It only cost $2. 50 for an IN & OUT pass. This year the cost is FREE for any club member that has purchased a lift ticket. (You can go in and out as many times as you would like and there is always someone there watching your equipment) Ski patrol reported one year that a school bus from Washington DC area was stopped on the way out of the resort with 100 EXTRA pairs of skis. People will take your skis, Poles, and boards if they are left unprotected! 5. The Clocks at 7-Springs are Wrong Always have a watch on so you know the correct time. Don t be late for the Bus! 6. Always follow proper Ski etiquette If you are unfamiliar with these, look for the signs at 7Springs or ask a staff member for a pamphlet on the subject. 7. Only Ski on the Open Slopes & Trails If 7Springs closes a slope, it must be for a good reason, so respect their signs, and follow the warnings. 8. Stay on the Slopes you are Comfortable With Don t go down a slope just because your friends are (this is how accidents occur) Remember you had to learn to crawl before you could walk, and you will someday be ready for these slopes, but don t go until you are ready. 9. Be Respectful of Others ALL Franklin Regional School Rules apply while you are on the slope. Anyone who breaks these rules will not join us on the future trips and forfeit any money already paid. (Please refer to the Student Handbook in your planner) 10. Never Ski Alone We work on the BUDDY system while we are there. You are to NEVER leave your buddy alone! (Pick a buddy (or buddies) with the same skill level as you and you will enjoy your time on the slopes more.) It is always more fun to ski with a group of your friends. If you don t have a buddy you can: Ask a friend to join with you. See Mr. Racchini or Mr. Danny and they will introduce you to some people and pair you up with others around your skill level. 11. If your buddy gets hurt STAY WITH YOUR BUDDY until help arrives. The worse thing you can do is leave to get help. Flag down another skier and ask them to send the Ski Patrol. 12. If you get hurt Tell Ski Patrol that you are with the Franklin Regional Ski & Board Club and have them contact Mr. Racchini. I will check in with Ski Patrol frequently throughout the night for any students, and report there immediately when I am informed of an injury.

4 Bus Transportation 1. Getting on the bus When you get on the bus a chaperone or club officer will ask for your last name and record that you are on the bus. Once you are on the bus, you are ON it until the bus arrives at either the school or 7- Springs. 2. Get to the Bus on Time Everyone needs to be on the bus by 8:50 pm. This means there should be no more skiing or boarding on the North Face after 8:00 pm. Finish your day on the Front Face. 3. Passengers on the Bus: According to a Franklin Regional School District Policy, only Franklin Regional Middle School students and Pre-approved chaperones may ride the bus. Parents are still welcome to buy lift tickets from the club at a reduced rate, but they will have to provide their own transportation. Contact Mr. Racchini if you are interested in purchasing parent/ family reduced lift tickets. 4. Food and Drink is Allowed on the Bus This is a privilege which can be taken away if it is abused. When we get on the bus it is clean, and I expect it to be clean when we get off. Make sure you plan some time during the trip for lunch and/or dinner. All drinks should have a lid. (No open containers of Hot Chocolate). 5. ipods, Cameras and Cell Phones These are all great things to have to help pass the time on the long bus ride there and back. However, the light from the display screen and flash can be VERY distracting to the driver especially on those snowy roads. Please try to keep the light as low as possible. If road conditions are bad when we leave, students will be asked to not use these items to limit the number of distractions. 6. Loading and Unloading the Bus I will ask a few students or chaperones to be loaders on the trip. They will be the last ones on the bus and will make sure that all equipment is loaded under the bus. Loading at School: When you are dismissed from class, we will ask you to first get changed as quickly as possible (boots and coats can wait until we are almost there). After you are dressed we will need to unload the skis from the ticket booth and place the equipment on the sidewalk at least three feet away from the curb. All Skis, Ski Poles, and snowboards will be placed under the bus by the loaders. Make sure you bring your boots IN the bus with you. They will stay warmer that way. Unloading: When we arrive at 7-springs, ONLY loaders will exit the bus while the ski patrol greets the rest of the students. They will be unloading and just stacking the skies and boards somewhere near the bus. Once Mr. Racchini returns with the ski passes, all other students may exit the bus. Loading at 7-Springs: When you return to the bus, please put your snowboard or skis in your bag or ski strap and then neatly place it under the bus. Ski Straps or bags: These are great for keeping your poles with your skis and increase the likelihood that someone else will not mistakenly take any of your equipment. 5 Middle School Ski & Board Club Sponsor: Joe Racchini (724) (x2205) The Franklin Regional Middle School WEDNESDAY Ski & Board Club will be going on a total of three ski trips to Seven Springs in the months of January and February. Below is some important information to consider before signing up to join the Wednesday Ski & Board Club, so please read it carefully. Students: We will need to have a total of 36 Students to participate in the club. Buses fill up fast and seats are first come first served. Bus number one can only hold 50 students. (In order to add a second bus, an additional 36 students are needed. If we get less than 36 extra students beyond the first 50 we will have to turn them away from club) Authorization: Your parent must complete the Ski Club Authorization Form (THE LAST PAGE OF THIS PACKET) so you can receive treatment in case of an emergency. It must be completed, NOTARIZED, and returned in the envelope with your payment. Trip Dates: January 10, 2018 January 17, 2018 January 24, 2018 Make-up Trip/Extra Trip January 31, 2018 (If ski conditions are bad in January, February Wednesdays may be used.) Time: The Buses will leave the Middle School at 2:40 pm (You will be dismissed from class at 2:15 so you can get dressed and be ready to go by 2:35) and will return to the school by 10:00 pm (Tell your parents to be 15 minutes early to pick you up) The building is closed and you will be cold waiting for your ride outside. It is easier for your parents to wait for you in a WARM car, than it is for you to wait outside; cold, damp, tired and hungry. Cost: $ Cash or checks are acceptable. (Exact Cash only or Make Checks payable to FRSD - MS Ski Club and indicate on the check memo that it is for the Student s 1 st & last name) The money will pay for All three ski passes, Chaperones expenses, and Transportation to and from the ski resort. Food, Lessons, and Equipment are not included in this price. (Students will need to bring money for dinner and/or snacks.) Place the money and notarized Authorization form in an envelope with the Students name, Grade, and include the title Wednesday Ski & Board Club. Return to Mr. Racchini (Room 205) or Mr. Danny (Room 114) by Thursday, November 16, Any missed trips, due to family or personal reasons will result in a loss of only $23.34 for that trip s bus cost and all other fees for that trip ($ Lift Ticket) will be refunded upon written request at the end of the season.

6 Season Pass Holders (Seven Springs) Students that already have a season pass will be invited to join us. However we must have 20-committed Non-season pass holders in order to receive the group rated tickets. The cost for season pass students is just $ for the transportation for all three trips. Equipment: ALL PARTICIPANTS MUST HAVE THEIR OWN EQUIPMENT (Skis, Boots, Poles, Snowboards, etc). If you need to rent your equipment, do not plan on renting it at 7-springs (This takes away a lot of your valuable time on the slopes). Instead GOOD, QUALITY equipment can be rented for the entire season from: Peak Ski & Snowboard 640 Center Road Pittsburgh, PA Located Near Boyce Park Call (412) for more details. 1. Travel west on US 22 towards Monroeville 2. Turn Right onto Mosside Boulevard (1 st Light after Turnpike) 3. Continue straight through one light and one stop sign. 4. Make the next right onto Center Road. (Also the Orange Belt) 5. After about 1 mile turn Right into the Parking lot for Peak Ski Rental / Center Ski and Snowboard after crossing over the turnpike Students may store their equipment in a secured room during the day of the trip. Just come in to the office lobby BEFORE 8:00 AM and we will store all of your equipment. Cancellations: If we need to cancel a trip, we will try to make it up on a make-up day. If the trip cannot be rescheduled all money for that trip will be refunded. Students will be notified on the morning or afternoon announcements as early as possible if a trip is canceled due to poor ski or road conditions. Please make sure that you pay attention to these announcements. You can also check for this information on the day of the trip at the Ski club s Web Page. Information will be posted as soon as possible. Remember: All participants are representing Franklin Regional Middle School. Students must be mature and have good grades to remain in the club. Any misconduct or bad behavior on the slopes, on the bus, in the lodge, or even at school, will result in the removal of that student from the club and forfeiture of fees paid.

7 What to expect on Wednesday Ski & Board Trips How to Check the Status of the trip: 1. Listen to the morning and afternoon announcements the day before and the day of the trip for updated information about snow and slope conditions. 2. Check the website in the morning of the trip for the most updated information (Most of the time information will be posted by 7:15 that morning) 3. Sometimes it becomes necessary to cancel a trip at the last minute. These will only be announced during the Afternoon Announcements, so students must make sure they listen very carefully to these announcements. What to expect the day of the Trip: 7:20 7:40 Check the Middle School Ski & Board club website for the latest update before you leave your house. 7:35 8:00 Arrive at school and drop of equipment and ski clothes. Equipment and ski bags will be stored (and locked) in the Little Theater ticket booth during the school day. Please have your equipment there before the start of Homeroom (8:00 AM) IMPORTANT: All students that are attending the trip MUST come to the Ticket booth in the morning to Check in and also to indicate whether or not they are purchasing a lesson. PLEASE make sure that you have ALL of your equipment before the first trip. It is best to have your equipment labeled with your name and tied together (Ski straps are available for purchase from Peak Ski & Snowboard. Ski or Snowboard bags and bungee cords also work fine). This keeps your equipment from getting separated during the loading and unloading process. It is also a good idea to label your snowboard, skis and poles with your name. If you plan on purchasing LESSONS for a trip, you must PAY Mr. Racchini when you drop off your skis or snowboard in the morning. He has to tell 7-Springs the final number of tickets and lessons before we leave. Parents and family members that will be traveling to seven springs on the day of trip may purchase discounted tickets through the club. Please read the page for buying extra tickets in this packet. If you are interested then Mr. Racchini or call x2205 before noon of the day of the trip. 1:55 2:10 Volunteers will unload all of the equipment from the ticket booth and costume room. Skis and snowboards will be placed on the sidewalk by the bus loading area. Bags with ski clothes will be placed in the main lobby along the walls. 2:15 Students will be dismissed from class. They should quickly go to their lockers then report to the main lobby to change into their ski clothes. (It is best to put on your under layers at school, but don t over dress for yet. Too many layers on the bus can cause you to sweat, and the sweat will make you cold on the slopes.) 2:15 2:35 Use the Bathrooms to change into ski clothes (DO NOT put on your ski boots at school. You can take these on the bus with you and put them on when we get closer to 7-springs. This will make your bus ride a little more comfortable.) After you are changed, you will be expected to SIT DOWN IN THE LOBBY and wait for instructions from Mr. Racchini.

8 Since there are so many skis and snowboards, we ask that only designated loaders actually place the equipment on and off the bus. 2:35 2:40 Load the Bus 1. Mr. Racchini, Mr. Danny, or another appointed person will be the FIRST person on the bus. You cannot enter the bus until Mr. Racchini or Mr. Danny gives the ok. 2. As you enter the bus simply tell this person your LAST name and you will be checked in. (Once you are checked in, you will not be permitted to leave the bus) 2:45 3:50 Travel to 7-Springs 3:45 4:00 Arrive at 7-Springs 1. When we arrive at 7-springs Mr. Racchini will exit the bus to purchase the Lift Tickets. 2. The bus will then park and only the Pre-Appointed ski loaders will be permitted off the bus to help unload all of the equipment. ALL OTHER STUDENTS MUST REMAIN ON THE BUS UNTIL MR. RACCHINI RETURNS TO YOUR BUS! 3. While Mr. Racchini is buying the tickets, a 7-Springs representative might enter the bus to share information about the slopes and the night s ski conditions. You are expected to listen to these people, for the information they are sharing is very important to your safety. 4. When Mr. Racchini returns to the bus, he will hand you three things as you exit the bus: a. Your lift ticket (Please do not take a one if you already have a season pass) b. L.E.D. FRMS Ski & Board Club Member ID Light (All members of the trip including students and chaperones, must wear this while at 7-springs. Its purpose is to help students & chaperones identity other members of our club.) c. Wicket (Only take one if you need one. If this is your first time, someone will show you how to use it) 5. As you exit the bus, please gather your equipment, (If you have a snowboard or ski bag, put it back under the bus) meet your Ski BUDDIES and start your night of skiing. Those students that have paid for a lesson should wait at the bus until Mr. Racchini, Mr. Danny, or another appointed person escorts them to the lesson area. The Chaperones will help those students get started until their ski instruction begins 4:00 8:15 Open Slope Time. 8:15 8:45 Finish Skiing - Please make sure you are back to the bus NO LATER than 8:45. Do NOT think that you can get one more run in before 9:00, because you will be late. 8:45 9:00 Check in at bus and Leave 7-Springs - Again as you enter the bus simply tell the appointed person at the front of your bus your last name and you will be checked in. (Once you are checked in, you will not be permitted to leave the bus.) Remove your L.E.D. Member Identification Light and place it in the appointed container BEFORE going back to your seat. 9:00 10:00 Travel to School How to Contact Mr. Racchini while at 7-Springs in case of an emergency: 1. In case of an emergency on the slopes, find a member of the ski patrol (Red and Blue jackets with 7- Springs on the back) and they will contact Mr. Racchini through the chair lift message boards or radio. Mr. Racchini or the closest chaperone will arrive at your location within a few minutes. 2. If you are in the lodge, ask someone at the information desk (Lower level) to contact the Franklin Regional Middle School Ski club sponsor and they will contact Mr. Racchini through the chair lift message boards or radio. Mr. Racchini or the closest chaperone will arrive at your location within a few minutes. 3. Mr. Racchini and the other Chaperones normally stop to eat in the main ski-lodge around 6:00. You should be able to find one of the chaperones here at this time. 4. ALL Ski and Board Club members that will have an Android or ios phone with them at 7springs are REQUIRED to sign up for and use CELLY. (See CELLY Instruction page towards the end of this packet.)

Any member interested in ordering shirts must complete the form below and return it with payment to Mr. Racchini or Mr. Danny by Thursday November 16, No Late orders will be accepted. 10 If this is your first Time: Skiing and snowboarding are lifetime sports. It s never too early or too late to start. In order to get the most enjoyment out of the sport, it s important that you take a lesson from a professional instructor. We will offer group lessons this year to any skier or snowboarder. Your first lesson will help you learn basic turning and speed control. You may want to give skiing or snowboarding a try before buying your own equipment. We recommend that you rent your equipment from Peak Ski & Snowboard in Plum. That way the shop staff can help you select the proper equipment and make any adjustments when needed. If you have skied before and you want to get better: The level II or Level III lesson is just as important as the first in developing skills that will help you safely enjoy the mountain. If you ve already skied or boarded before, then you re off to a good start. The basic turning skills you already know will be developed to give you even more control so that you can smoothly navigate the network of trails. What to do: Tell Mr. Racchini when you check in at the ticket booth in the morning that you would like a lesson for that trip. Lessons are a separate cost from the club cost. You will need to pay Mr. Racchini for each lesson when you check in. Place your money in an envelope and write on it Your Name, Grade Level, and what Type of Lesson (Ski or Snowboard). When we arrive at Seven Springs, all students that are getting lessons will be allowed to get off the bus first, gather their equipment and follow one of the Chaperones (or student officers) to the lesson area. Lessons begin at 4:00pm If the bus runs late, Mr. Racchini and Mr. Danny will help all of the first timers until the next lesson session begins (6:00pm) After the lesson, students are free to spend the rest of the day practicing the skills that they have learned on the slopes with their ski buddy. If their buddy is not taking lessons they will need to arrange a meeting place for one hour after the lesson. Group Ski or Group Snowboard Lesson 1 Hour Lesson Lessons will be given to students by 7-springs ski and snowboard instructors shortly after we arrive. (4:00pm) $17.00 per Lesson Please bring exact cash on the day of the lesson.

11 1. Always stays in control, so you can stop or avoid other people or objects. 2. People ahead of you have the right-of-way. It is your responsibility to avoid them in a courteous and responsible manner. 3. You must NOT stop where you obstruct a trail, lift line, ramps or are not visible from above. 4. Whenever starting downhill or merging into a trail, look uphill and yield to others. 5. Always use devices (ski brakes/leashes) to help prevent runaway equipment. 6. Observe all posted signs and warnings, especially slow and closed trail signs. 7. Never jump, ollie or slide objects (tables, fences, trash cans, etc.) other than those in the snowboard park. 8. Prior to using any lift, you must know how to load, ride and unload safely. 9. Profanity is not acceptable. The Challenge of Safe Skiing, Boarding and Snow Tubing 1. Alpine sports are recreational activities that continually challenge the human spirit. Natural and man-made obstacles are part of that challenge of man against the mountain and the elements, but collisions with other people and objects can cause permanent, catastrophic injury or death. 2. Please be aware that Easier does not necessarily mean Beginner at Seven Springs. As a beginner, you should ski/ride the trails marked with a Double Green Circle. As you get better you can then work your way up the other levels (Green Circle, Blue Square, Black Diamond, Double Black Diamond) 3. To enjoy alpine sports, you must know and be willing to accept the limits of your ability. Use caution and have fun. Changing weather, snow conditions and natural and man-made obstacles exist and are part of the challenge. It is important to realize that trail conditions do change during the day through grooming, snowmaking, usage and weather. Please keep this in mind during the day and adjust your choice of terrain accordingly. Your safety will be directly affected by your judgment. 4. Alpine sports also challenge your physical condition and may cause stress. Recognize your limitations. If you are tired or cold, stop for a while. Those who are tired and exceed the limits of their ability are more vulnerable to mishaps. You will reduce the risks of injury and enjoy yourself more if you are in good shape and good health.

12 During the school year, the Middle School Ski and Board club implemented the use of the flashing safety light reflectors. We will continue to use these items during our trips. Each member will be given a light reflector as the exit the bus. This must be clipped on the member s jacket. When members reboard the bus at the end of the trip, they will return their light reflector to a chaperon. The light reflectors help the chaperones identify members of the club from a distance; and in case of an emergency they help ski patrol quickly identify from which group the injured skier or snowboarder belongs to. During the school year, the Middle School Ski and Board club made helmets mandatory. Some research states that skiers and snowboarders can cut the risk of brain injury dramatically by wearing helmets on the slopes. Dr. Stewart Levy, chief of neurosurgery and neurotrauma at St. Anthony Central Hospital in Denver, Colorado, has done a study - that looked at cases from 1998 through He says helmets reduced the risk of brain injury by 75 percent. Other studies show a reduction in brain injuries of 50 percent to 80 percent. But he says a helmet is just part of the recipe for a safe day on the slopes. "A helmet does not prevent all brain injuries and other types of injuries," Levy cautions. "So you still have to ski responsibly. A helmet is not a license to ski recklessly." If Members do not own a helmet, they can Purchase one or rent one from Peek Ski and Snowboard in Plum. ( ).

13 Any Parent that would like to purchase extra lift tickets for themselves and/or other family members may do so through the club. Extra Wednesday Night Lift tickets (4pm 9pm) can be purchased the day of our trips at a cost of $25.00 per ticket. (Regular ticket price at 7-Springs is $59.00 for Adults and $48.00 for kids 6-11)) If you are interested, please let Mr. Racchini know the morning of the trip or before noon on the day of the trip. ALL EXTRA TICKETS MUST BE PAID FOR BEFORE THE BUS LEAVES THE SCHOOL! Please write Your Name, Franklin Regional Middle School, and the Number of extra Lift Tickets Needed on an envelope that contains your payment. Give this envelope to Mr. Racchini before the bus leaves the school. (or you may choose to send it in with your child in the morning when they are checking in) Once the tickets are bought, they will be placed back into the envelope. You may pick up your envelope at the bus at 7-springs after all of the students have been helped and released. (If you arrive after everyone has left the Bus area, your envelope with your tickets will be at the Group Ticket Office) A VERY Limited number of Friday Saturday Sunday Day / Twilight Lift Tickets Might Be Available Extra Friday, Saturday or Sunday Day / Twilight Session Lift tickets (9am 7pm or 1pm 10pm) can be purchased at a cost of $60.00 per ticket. (Regular ticket price at 7-Springs is $85.00). Mr. Racchini has only a limited number of these tickets for this year, so they are on a first come first serve basis. Each ticket is good on any Friday, Saturday, or Sunday in the Ski season. Cash only for all extra tickets please

14 On occasion, there are students that ask to be excused from the bus ride home. (Their parents are at 7-springs and either wish to bring the student home, or they are staying at 7-springs for a longer period of time.) In order for this to happen, the following are required: 1. Mr. Racchini must have a dated note written and signed by the student s parent, which indicates their intent to take over the responsibility of that student. This note must be in Mr. Racchini s hand before the student is allowed to leave with their parent. An is NOT a valid note. Likewise, a phone call or text from a parent is not an acceptable request. We must have a written and signed note from the student s parent or legal guardian (not a brother/sister or a friend s parent) for our records. 2. Any student that has requested to not return with the club must report to the bus at 8:30 9:00 and report only to Mr. Racchini or Mr. Danny. 3. Any Parent that has requested to take responsibility for their child must also report to the bus at 8:30 9:00 to officially take over the responsibility. (We can not take just your word that you will be at 7-springs. We need to see you and your child together at the bus at 8:30) These requirements are mandatory and not up for any kind of negotiation! If any of the three requirements are not met, the student will be riding the bus home with the rest of the club.

15 If you are interested in assisting the students of the club during our trips, then please contact Mr. Racchini. The club is looking to have several chaperones this year. As the club grows, so will the number of chaperone that are needed. 17 Once again this year I am requiring all students with a Smart Phone to sign up and use the Celly App. Celly will be our primary source of communication while we are at 7springs and a means to send reminders home to Ski Club Members. Celly brings the power of mobile social networking to everyday life in a private and secure way. Family, friends, neighbors, classmates, coworkers, and teammates can use social building blocks called CELLS for everyday collaboration, knowledge sharing, and group communication on any Smart phone or Tablet device. Use the App to Join in seconds Group messaging with moderation Works on ios and Android systems App available for free download. PHONE NUMBERS ARE KEPT PRIVATE Easily communicate with anyone from the Cell without giving away personal information. All students that have a smart phone are being required to join the following CELL: FRMS Ski and Board CELL (Ski Club Members only) HOW TO SIGN-UP: Works with iphone or Android app only. 1. If you are new to Celly, you must first use a computer or smart device, and go to 2. Login to your Celly account or Click the Signup button and choose one of the 3 ways listed (With , with Facebook, or with Twitter) a. Please choose option b. Create a username using the following as your example: i. Username = JoeSmith (Only use your name, No crazy usernames like CoolBaby34 ) ii. And then complete all other require fields 3. Using your smart device, click on the Apple or Android button to download the Free Celly App. a. Open the App and Log in. b. Using the search bar, search for the CELL you would like to join: (you will need to type it in exactly as it appears below and then press + Join cell or add friend You should now see the Cell listed. Click Join Enter the Cell s password = {The password will be ed to all members after the club s first meeting} If you do not own a smartphone and cannot use CELLY, please make sure that someone in your group (ONE of YOUR BUDDIES) has a phone with them.

18 This cell is currently set to Curated Chat which means only Mr. Racchini or other Chaperones will be able to send out messages to the group about important events and this will allow you send a private message to Mr. Racchini and the other Chaperones and they must approve it before releasing it to the whole group if they choose to do so. You will always be able to send a private message to anyone listed in the conversation (including Mr. Racchini and the other chaperones.) See the section on How to Use CELLY. HOW TO Use CELLY APP: To read message: 1) Open APP and tap the CELL that you wish to open. To send message to CELL (NOT Available in Alert Mode, and must be approved in Curated Chat Mode) 1) Open APP and tap the CELL that you wish to send a message. 2) Type your message in the message line at the bottom of the screen and tap send. (For more options for your message [Photos and attachments] click the up arrow next to the message bar.) Submit the form and follow the directions on the conformation page. 3. The AUTHORIZATION FORM will be ed to the parent that you typed into the google form. You should expect this within a few minutes after submitting the form. (If you do not, then try resubmitting the form. If you continue to have trouble, then see Mr. Racchini in room 205) 4. Print a copy of the Authorization form from the . (See Mr. Racchini if you are having trouble printing this form.) 5. Fill out the Authorization form, and have a parent sign it in front of a Notary. 6. Place the form and payment into an envelope with your Name, Grade level, and Homeroom #. 7. Hand this to Mr. Racchini (205) or Mr. Danny (114) no later than NOVEMBER 17 th. ONLY those students that have (1) COMPLETED THE ONLINE FORM, (2) COMPLETED A NOTARIZED AUTHORIZATION FORM, and (3) TURNED IN THE PAYMENT WITH THE AUTHORIZATION FORM BEFORE THE DEADLINE will be officially on the list of club members. REMEMBER: there are a limited number of seats on the bus. So the seats are on a first come first served basis. If you want to guarantee a seat on the bus, plan on turning in your authorization form and payment in as soon as possible. Bus one can hold 50 students. (The second bus will not be officially added until we reach a total of 75 students. If only 64 students sign up by the deadline, money will be returned to the last 14 students that turned in their paper and they will not be able to join the club.
