1 NOMINATION FORM Nominations for the 2015 WA Swimmer of the Year Awards are now open. All submissions must be received at Swimming WA by 5pm Tuesday 7th April Conditions of entry The nominee must have attained their achievements whilst a member of Swimming WA and affiliated clubs within the time stipulated in the selection criteria. Nominations that do not clearly address each item in the selection criteria will not be accepted. All nominations must be endorsed by respective Clubs to ensure members are of good standing within the Association. Swimming WA reserves the right to nominate candidates. All nominations must be entered using the official nomination form, via , return mail or in person. All nominees, including finalists, will be subject to potential media exposure. Late nominations will not be accepted. Finalists will need to produce two photographs for Swimming WA to use at the event closer to the date. Winners are selected by an independent judging panel. PLEASE NOTE: Nominations can be made for Technical Official of the Year, Coach of the Year and Volunteer of the Year only. All other awards will be selected.

2 Awards and criteria Lyn McClements Swimmer of the Year Outstanding achievement(s) in performances at an International, National, State or Club level. Age Swimmer of the Year Outstanding achievement(s) in performances at an International, National, State or Club level Nominees must be between years of age. Junior Swimmer of the Year Outstanding achievement(s) in performances at a State or Club level. Nominees must be 12 years of age and under during the specified time period. Open Water Swimmer of the Year Outstanding achievement(s) in Open Water Swimming competitions at an International, National and/or State level. Multi-Class Swimmer of the Year Outstanding achievement(s) at an International, National and/or State level. Technical Official of the Year Outstanding achievement to Swimming officiating at an International, National, State and/or Club level. Coach of the Year Outstanding swimming coaching performance with athletes at an International, National and/ or State level. Volunteer of the Year Outstanding contributions to swimming at Club, Regional and/or State Level. Also presented on the morning Metropolitan and Country Club Shields A, B, C, D, E Club Premierships Dive and Try Awards Presidents Award - Swimmer and Non-Swimmer Perpetual Trophy Winners Swimmers Swimmer of the Year* Target Time Swimmer for contribution to swimming either at National or State level. *Voted by Swimming WA Target Time Swimmers
