1 May 1, 2017 Dear Parents and Swimmers, Attached please find a copy of the Nashville Aquatic Club (NAC) BridgeAthletic Summer Sizzler Itinerary, Permission Slip and Payment Form, Medical Release Form, and WEST Swim Club s Travel Trip Rules and Regulations. NOTE THAT THE BUS/HOTEL TRAVEL TRIP OPTION IS AVAILABLE FOR THOSE SWIMMERS ENTERING HIGH SCHOOL IN THE FALL AND OLDER! YOUNGER SWIMMERS ARE ENCOURAGED TO ATTEND THE MEET WITH THEIR PARENTS. This year we have a huge group of eligible and interested swimmers for the bus/hotel travel trip option, so putting down a deposit to secure your spot is strongly suggested. If a swimmer is eligible for the bus/hotel travel trip option, and would like to exercise that option, please RSVP by MONDAY, May 15 TH by ing the coaching staff at westcoach@comcast.net. The NAC BridgeAthletic Summer Sizzler runs from Thursday, June 22, 2017 through Saturday, June 24, The bus travel group will be leaving from the West Superior Training lot promptly at 8:00 am on Wednesday, June 21, 2017 and will be returning on Sunday, June 25, 2017 between 11:00 pm and midnight. Not only will the trip include some fast WEST swimming, but we are working on planning some Nashville summer country fun for both before and after the meet as detailed below: Shortly after arriving in Nashville on Wednesday, June 21, 2017, the team will head to the MARTIN S BBQ JOINT on Fourth for a night of dinner, games and team fun. ALL SWIMMERS and PARENTS are invited to be part of the fun! Following the finals session on Saturday, June 24, 2017, ALL SWIMMERS and PARENTS are invited to a Southern family style meal at MONELL s AT THE MANOR. And then before heading back to the Chicagoland area on Sunday, June 25, 2017, the team will spend 4-5 hours at the NASHVILLE SHORES WATERPARK, where we will have a WEST picnic area (with plenty of food) set aside for the group in addition to the water ride fun in the sun. More details on these entertainment activities (including the cost breakdowns) are included in the attachment entitled WEST 2017 Summer Nashville Travel Meet Entertainment.
2 Eligible swimmers electing for the bus/hotel travel trip option will travel with the team to ALL of the swimming and entertainment activities. Swimmers will not be allowed to stay back at the hotel during such activities unless a responsible parent/guardian is present and the coaching staff has been notified ahead of time. It will be assumed that swimmers taking the bus/hotel travel trip option will attend ALL activities, both swimming and entertainment, unless individually discussed with the coaching staff in advance. Families without swimmers eligible for or electing out of the bus/hotel travel trip option, are still invited to the entertainment activities. If these swimmers/families would like to attend the entertainment activities, please indicate so by WEDNESDAY, MAY 31, 2017, by completing this document and placing in the LUNDGREN FOLDER at the pool with your payment. If swimmers elect for the bus/hotel travel trip option, a deposit of $250 (in the form of a check) for the trip is due by WEDNESDAY, MAY 31, Each swimmer is responsible for their own spending money for meals not catered or included with the entertainment activities. It is recommended that your swimmer bring $75 - $100 in spending money for the trip. Daily breakfast, a late morning snack and dinners are included in the travel costs for all swimmers travelling on the bus and offered to all 13 & Older swimmers for an extra fee. The team will be staying at the Holiday Inn Vanderbilt-Nashville 2613 West End Avenue - Nashville (phone # ). Rooms for the swimmers taking the bus/hotel travel trip option have already been reserved and will be booked by the team. The coaching staff would love to have 12 & Under swimmers with their parents and the parents of bus riders join us at the meet to cheer on all of our swimmers, but parents of bus riders are not obligated to attend. The WEST coaching staff assures all parents that the swimmers electing for the bus/hotel travel trip option will be chaperoned, monitored, and well taken care of if the parents are not able to attend. In recent communications with the Holiday Inn Vanderbilt-Nashville, we were notified that the all of the available rooms in the WEST team block for attending parents are already spoken for, though we continue to try to find more. The NAC meet packet has also been attached as a guide to the scheduling and format of the meet. If you have any questions, please contact the coaching staff at westcoach@comcast.net.
3 WEST Swim Club NAC Nike Summer Sizzler Invitational Our team travel trip is June 21 25, 2017, to the NAC Nike Summer Sizzler Invitational in Nashville, TN. This is a great opportunity to swim against some of the fastest clubs in the country, and definitely some of the best from the Midwest. There is a bus/hotel travel trip option, for swimmers entering high school in the fall and older. The trip will be chaperoned by the coaching staff. The costs are as follows: Estimated Cost: $550 ($250 Check Deposit due Wednesday, May 31st) Includes: Transportation: Hotel: An air-conditioned motor coach with bathroom and video capabilities and a 15-passenger cargo van. 8 WEST SWIM CLUB TRAVEL TRIP RULES AND REGULATIONS The following rules and regulations will be in effect June 21-25, 2017 for the team travel trip to Nashville, TN for the NAC BridgeAthletic Speedo Sizzler. Anyone who, in the opinion of the coaching staff, acts in a manner that would interfere with the travel objectives listed below, will be subject to immediate return home (at the expense of the parent and/or swimmer) and other punishments including barring from future travel meets or other competition, suspension from practice, or dismissal from the team. 1. All WEST Swim Club team members should be respectful of their peers, coaches, and parents. Any WEST team member not acting in such a manner will be asked to change or stop that behavior. If that team member fails to comply, they will be asked to leave the meet. 2. WEST will travel as a team and as individual families in order to compete with the best and further strive to better competition. Everyone is expected to behave in an exemplary manner. The reputation of WEST, as well as the other athletes with you, is dependent on your behavior. 3. The coaching staff holds the final word on any rules, regulations, or disciplinary action. 4. The consumption or purchase of alcohol, smoking or chewing tobacco, or use of any other illegal drugs, or USOC banned substance of any kind will not be allowed. In addition any team member found or suspected to be in the presence of others (regardless of team affiliation) partaking in any of the above activities will be subject to disciplinary action as outlined above. 5. At no time will male and female athletes be in the same room together unless the door is open. This applies to WEST members or any other team. 6. Any damages or thievery incurred at the hotel will be at the expense of the swimmers assigned to that room and further disciplinary action will be taken. No loud behavior will be tolerated in the hallways or public areas and such behavior should be kept to a minimum in your rooms. I recognize my responsibility to abide by the rules and requirements of the WEST Swim Club that I am representing and I acknowledge that I have received and read such. 9 MEDICAL RELEASE FORM 2017 Name of Swimmer Date PARENTAL CONSENT - This medical release form must be signed by a parent or legal guardian for EACH SWIMMER of the WEST Swim Club. If the swimmer is 18 years of age or older, the swimmer may sign this form themselves. MEDICAL RELEASE - I certify that, to the best of my knowledge and belief, (name of swimmer) is in good physical condition and has no condition which would impair participation in the program. In case of injury, I hereby give the WEST Swim Club and it s coaching staff permission to act on my behalf in seeking medical treatment from any licensed physician, hospital, or clinic for my child in the event that such treatment is deemed necessary. I give permission to those administering medical treatment to do so using methods deemed necessary. I absolve WEST Swim Club and its coaching staff from all liability while acting on my behalf in this regard. We have a private area reserved for our WEST family-style meal of pulled pork, brisket and chicken with fantastic Southern sides. We will have the outside garden for ping pong, yard darts and shuffleboard and indoors for WEST Bingo, Trivia and fun! Awesome prizes, too!
