In granting this sanction number, it is understood and agreed USA Swimming shall be free from any and all liabilities or claims for damages arising by reason of injury to anyone during the conduct of the event. Whether entries are submitted electronically or on paper, by the action of entering the swimmer in the meet the submitter whether the swimmer, coach, parent or team representative, certifies that the swimmer is a registered member of USA Swimming and that all times stated in/on the entry are true and correct. The submitter further assumes responsibility for any false or incorrect times, and understands that all listed entry times are subject to Utah Swimming Time Verification Procedures, and that a fine of $20.00 shall be paid to Utah Swimming, Inc. for each incorrect time, unless corrected before the entry deadline. It is understood that an automatic call before the review section shall be the result for a third entry time that cannot be verified. The submitter further agrees that a penalty of $ shall be assessed to anyone entering a swimmer in a meet sanctioned by Utah Swimming, Inc., who is not registered with USA Swimming at the time of entry upon it being proved that the swimmer was not so registered. The competition course has not been certified in accordance with C(4). No on deck registration will be accepted. Swimmers must be registered at the time of entry. No swimmer will be permitted to compete unless the swimmer is a member as provided in Article 302. The swimmer s age as of

2 December 7th, 2012 shall determine the age for the entire meet. No times standard will apply. All Times and No Times will be accepted in ALL EVENTS. Meet Format: Check In: Meet will be Timed Final format. Deck entries submitted up to 30 minutes prior to the first session will be accepted. No Time Trials will be offered. The 500 Freestyle, 1000 Freestyle, 1650 Freestyle and the 400 IM will be seeded fastest to slowest alternating women and men. If a swimmer misses their event, they will not be allowed to swim that event but will not be scratched from their next event. Session #1: Positive check-in required by 12:30 PM. Swimmers who do not check in by this time will not be allowed to swim in this session. Sessions #2, 3 and 4: Negative check-in only required. Scratch Procedure: Entries: Scratches are due 30 minutes before the beginning of each session. Please submit all entries either on a disk or by using Hy-tek software, or use the enclosed entry form. Teams entering on a disk or must submit a signed hardcopy with verification of times. Entries will not be accepted without time verification and one uniform entry sheet signed by the person responsible for entry submission. Said person will be responsible for all fines and penalties resulting from inaccurate times and unregistered swimmers. Teams using the official entry form must make sure that each form is signed and time verification is included on each entry. If sending entries by , please include all team information and contact information with the e- mail. Entries should not be considered received/official until receipt of this confirmation. Please contact the Meet Director if you have not received confirmation within 24 hours of the entry deadline. Teams should bring a printed hard copy or electronic copy of all entries and s with entry problems to the meet for reference if a problem arises. Individual Entries: Entry Fees: Entry Deadline: Individual swimmers may enter up to EIGHT (8) individual events, swimming no more than FIVE (5) individual events per day. No Times (NT s) are accepted. The 1650 and 1000 freestyle events will be limited to the first 16 girls and 16 boys (2 heats each) in each event. Surcharge, per participating swimmer: $4.00; Individual Event Entry: $2.50 for electronic submissions. If entering manually (paper entries): Individual Event Entries are $5.00. All fees must accompany entries and are nonrefundable. One team check is recommended. Make checks payable to DART. 3 Warm-up: All warm-ups must be done under the supervision of a USA-S official. Coaches have the responsibility of insuring that all their swimmers are instructed in warm-up procedures and that the swimmers follow all Utah Swimming safety procedures. All swimmers will enter the pool from a 3 point or sitting position. No pushing, splashing, running or any other general horseplay. Swimmers engaging in such activities will be asked to leave the pool. During the last 15 minutes of warm-up lanes 2 and 7 will be opened for diving starts. Four Hour Limit: A mandatory scratch down may apply to this meet if the time line exceeds 4 hours for 12-Under swimmers, subject to the provisions of USA Swimming Rules and Regulations ( F). If a scratch down is necessary, teams will be reimbursed for said scratched events. Unaccompanied Swimmers: Diving Certifications: Audio/Visual Recording: Results: Scoring: IMX Awards: As per USA Swimming rule , all USA Swimming athletes must be under the supervision of a USA Swimming member coach during warm-up, competition, and warm-down. If you do not have a coach with you, please check with the meet director or referee before entering the pool. A coach will be assigned. Per rule A of the 2012 rule book, any swimmer entered in the meet must be certified by a USA Swimming member-coach as being proficient in performing a racing start or must start each race within the water. When un-accompanied by a member-coach, it is the responsibility of the swimmer or swimmer s legal guardian to ensure compliance with this requirement. **Use of audio or visual recording devices including a cell phone, is not permitted in changing areas, rest rooms or locker rooms.* Results will be ed electronically following the meet. Using the USA swimming/hy-tek s Single-Year Age Group Power Point Calculator, all athletes will earn an IMX Score per IMX event. Overall meet score will be the cumulative IMX events points score (combination of all IMX events below) for the Age Group. 1 st 3 rd overall highest IMX point totals for each Age Group will be awarded trophies. Ribbon awards will be given for 4 th 8 th places. To qualify for an IMX award, a swimmer must compete in all of the IMX events in their age group. 10 & Under 100 Fly, 100 Back, 100 Breast, 200 IM, and 200 Free 11 & Fly, 100 Back, 100 Breast, 200 IM, and 500 Free 13 & Fly, 200 Back, 200 Breast, 200 IM, 400 IM, and 500 Free 15 & older 200 Fly, 200 Back, 200 Breast, 200 IM, 400 IM, and 500 Free Event Awards: Ribbon awards for the top 8 positions per event. If an 8 & Under wants to compete for the IMX awards, they need to compete in the 10 & Under category, in events 7/8, 35/36, 41/42, 57/58 and 67/68.

Guests will have until November 15, 2012 to call, at which point unreserved rooms will be released back into inventory. Rate will remain available but not held after that.
