VEHICLE 2 WAS TRAVELING WESTBOUND ON CEDAR LAKE ROAD AND CONTINUED STRAIGHT THROUGH THE INTERSECTION O CEDAR LAKE ROAD AND LINDBERGH DR. 1 ADVISED HER SEAPHORE WAS YELLOW AND THE VEHICLES TRAVELING WESTBOUND LOOKED AS THOUGH THEY WERE SLOWING DOWN TO STOP. 2 ADVISED THE SEPHAORE WAS GREEN SO HE CONTINUED THROUGH THE INTERSECTION. VEHICLE 1 HAD HEAVY PASSENGER SIDE RONT END DAAGE. VEHICLE 2 HAD HEAVY SIDE RONT END DAAGE. NO INJURIES WERE REPORTED. THE WITNESS ADVISED SHE WAS AT A RED LIGHT ON SOUTHBOUND LINDBERGH DRIVE WAITING TO TURN ONTO CEDAR LAKE RD. SHE SAW THE VEHICLES TRAVELING WESTBOUND ON CEDAR LAKE RD WERE ALL DRIVING THROUGH THE INTERSECTION. HE THOUGHT THAT THE AXEL WAS BROKEN SO HE OVED TOWARD THE RIGHT SHOULDER APPLYING THE BRAKES. THE TRAILER THEN BECAE DETACHED AND CROSSED ALL LANES O TRAIC, STRIKING TWO VEHICLES IN THE LET LANE, THEN CROSSED BACK TO THE RIGHT SHOULDER COING TO A STOP IN THE GRASS O O THE RIGHT SHOULDER JUST NORTH O HIGHWAY 494. O VEH 2 (JONCZYK) STATED: HE WAS IN THE IDDLE LANE O 494 WHEN HE OBSERVED A TRAILER COE LOOSE RO VEH 1. HE OVED INTO THE LET LANE TO AVOID A COLLISION, HOWEVER THE TRAILER STRUCK HIS VEHICLE THEN OVED ACROSS THE LANES O TRAIC COING TO A STOP O THE RIGHT SHOULDER. O VEH 3 (EHUS) STATED: HE WAS IN THE LET LANE O TRAIC BEHIND VEH 2, HE OBSERVED THE TRAILER COE LOOSE RO VEH 1 AND IT STRUCK THE PASSENGER SIDE O HIS VEHICLE. 6 Remarks Crash Narrative: On 11/07/2018 I responded to a personal injury accident at southbound HWY 169 and innetonka Blvd. I arrived on scene and observed a Red Oldsmobile Bravada approximately 20 feet off the roadway. I observed the vehicle had the right rear axle broke and damage to the driver side of the vehicle. I observed the vehicle had hit the N DOT traffic camera pole and appeared to have knocked the camera from the pole. I spoke to the driver Cindy Neukircher and she advised she was driving south on HWY 169 when she hit a patch of ice and lost control of her vehicle. Neukircher advised she spun off the road. Neukircher did not know what she had struck after losing control. Neukircher complained of head, neck and back pain. Neukircher was transported to North emorial by HCC paramedics (Run# )accident report was completed. State was notified of the damaged camera. Date of Print: 11/13/ :24 A Page 2 of 2

10 Remarks Vehicle 1 was stopped behind a vehicle that had skidded into the curb due to the icy roadway on westbound Wayzata Blvd at Clarendon Dr. Vehicle 2 was westbound on Wayzata Blvd behind Vehicle 1. Vehicle 2 could not stop, struck the left rear of Vehicle 1, crossed the centerline, and struck the left side of Vehicle 3, which was eastbound on Wayzata Blvd. Driver 2 complained of an injury to her wrist.
