Pre-start checklist. 3 2 M5 Labels 5 4 E9 Fire Collision Maintain clean control labels on levers, buttons etc Fire extinguisher installed on plant, serviced and tested and tagged Phones must not be used whilst using plant. Left hand drive label must be legible at all 3 2 M5 Crushing loading and unloading Follow correct SWMS and training 2 3 M5 Crushing load restraint Follow correct SWMS and training. Never use faulty or damaged restraining equipment 2 3 M5 passenger seat label No passengers are to be carried at any time, the label must be legible at all ROPS label The warning label must not be damaged. It must legible at all ROPS seatbelt label Seatbelts must be worn when operating machinery, the label must legible at all
3 Electrical approach distances Safe distances must be adhered to strictly. Correct training needed for plant. Labels must be legible at all. Spotters are required when working within 5 metres of the minimum approach distance of any live electrical apparatus electrocution and explosion Hearing loss Never break ground without DBYD plans Correct PPE must be worn 4 1 M5 Hearing protection label for bystanders crushing, pinching. Swing boom label for bystanders burns, entanglement and shearing. Engine guard label crushing, poor signage, boom lifting point table burns from open cabin warning device (horn) Table must be legible at all. All operators must be properly trained Correct PPE usage Pre-start checklist making sure horn works and reversing alarm - lifting Lifting point must be 'closed eye' type. Must have rated capacity level adjacent to lifting point. Must have lifting chart adjacent to operator. Lifting slings must be tested and in date

Seat belt must be worn at all burns, striking Hydraulic hoses must be inspected daily before each use though pre-start checklists. Never use any body parts to check for leaks. Always use cardboard to check for leaks 3 2 M5 entanglement incorrect operation. Intuitive controls Guards must be fully functional Maintain plant at all slipping. Access/Egress Safe access/egress to work area must be maintained at all incorrect operation, slipping levers and foot controls must be kept non-slip
5 burns, electric shock The constraint and cover must be present and fully functional at all crushing FOPs Risk assessment must be conducted burns striking The guard between hydraulic hoses must be in place at all poor visibility Lights must be fully functional. If they fail work must cease immediately Maintenance incorrect operation Manufacturers Manual must be available at all. JSEA must be completed daily Maintenance operational malfunction Maintain service and maintenance records. Conduct regular inspections Maintenance ROPS damage ROPS must be free from damage Maintenance burns, striking, hydraulic damage Hoses must be free from damage. Daily inspection of hoses Maintenance operational malfunction Minor leaks must be fixed within 14 days. Pre start checklists must be completed to avoid major and minor leaks 2 3 M5 Transport Transport - Moving the excavator off the transporter Always remain at a safe distance from plant. Do not enter a work area unless they are in full view of the plant operator. Use a spotter where required. 1 5 M6
6 Transport Transport - Crushing injury or body injury from working near plant or machinery Ensure correct machinery unloading process is followed and that it is undertaken by a competent person. Ensure that ramps are secured in place and that spotter is engaged 1 5 M6 Transport Transport - excavator falling off transporter Inspect transporting lugs for wear and tear. Check chains are tight and dog secured whenever loading machinery onto transporter 1 5 M6
