SAFE WORK METHOD STATEMENT Ensure all employees and contractors are trained and competent for the tasks to be completed. Ensure all persons have undertaken HSC and Site WH&S induction. Operators and Drivers hold relevant licence for Plant/Vehicle to be operated Add others (if necessary):. Hazard identification Hazards not identified or adequately assessed. Supervisor or person in control of work site will assess worksite in order to identify hazards. Supervisor or person in control of work site will identify any unusual or specific hazards associated with the work, such as other workers or people in the immediate vicinity Site occupancy restrictions and conditions, including access and egress, work schedules and emergency management arrangements, including emergency communication and evacuation plans will be identified. If in doubt as to safety for the task, a second opinion may be required with an experienced operator, safety representative or WHS personnel. 2

Noise Dust Lack of Vision Blind Spots All site personnel understand any safety zone or exclusion zone that applies. Personnel are not to approach the mobile plant while it is operating. Persons other than the operator do NOT ride on the mobile plant. Mobile plant is operated at appropriate speeds for the terrain. Blades and attachments are lowered before the operator leaves the mobile plant. Operator is alert to other plant and equipment movements on the site. Speed is reduced before brakes are applied. All persons to wear high visibility clothing when on site. Water truck to damp down work area when excess dust generated Hearing protection to be worn where necessary (be careful not to over-protect). Operator must be aware that there is restricted visibility to the rear of mobile plant due to height of cabin, engine size, air cleaner and exhaust location. Spotters to be used to monitor blind spots Ensure the P.P.E is rated or capable of the environment. Public Safety / Pedestrians Public may be at risk from injury or death from work. Injury or death from contact with moving plant/vehicles Keep unauthorised person out of the site. Pedestrian signage is to be installed. Install safety barriers where necessary. Instigate traffic management plans if necessary. Reduce vehicle speed around the work site. Where required, clearly signed alternative safe laneways shall be made available for pedestrians. Laneways crossing kerb and channelling shall have non slip ramps suitable for wheelchairs, baby strollers, pedestrian scooters and aged or disabled persons. Plan and use safe vehicle access. Schedule working hours so as to limit exposure to the public. 3

6 Driving to and from work site (What are the hazards and Vehicle Interaction Loose or unrestrained loads Equipment Damage Comply with QLD Road Rules Ensure all loads are restrained as per Load Restraint Guidelines Ensure windscreens and all glass is clean and there are no obstructions to vision Ensure the slasher and other attachments are at maximum height when travelling at road speed as the slasher could strike the road or other objects and cause the tractor to veer off course. Any power transmission equipment attached to a tractor for the purpose of driving another machine and every joint by which such equipment is linked, MUST be fitted with guards. Always disable/isolate hydraulics when disembarking from tractor Pre start check - complete prestart checklist and ensure that: Slasher and other attachments are checked prior to use and are in good working condition. Check the oil level and top up if necessary. Check the blade, skid plates, safety chains, etc. Ensure that all parts are safely secured to prevent them coming loose and striking the operator or other people. Check PTO (Power Take Off) cover and replace if damaged to prevent operator s clothing becoming caught Ensure PTO shaft and universal joints are in good condition (universal joints could collapse and PTO shaft break and parts strike operator). All spur and other toothed friction gearing of machinery must be effectively guarded so as to prevent danger or must be situated safely, as if it were guarded. Any transmission machinery, wheel or pulley, must be effectively guarded or be situated safely, as if it were guarded. Use correct manual handling techniques, team lifting or mechanical aids. 6

Clear the area to be slashed of any obvious hazards, eg large stones, large twigs/ boughs from trees, wire. Select the correct height adjustment for the slasher for the task at hand. Fasten the seat belt Before starting motor, ensure transmission is in neutral and check throttle for proper operation. Ensure that all attachments are at a safe travelling height - NEVER move off with the bucket raised above eye level. Make another visual check to ensure that no people or obstacles are in the path of travel. Check the slope of the land, if applicable Check that all gauges, indicators and warning lights are operational. Sound the horn. Select the correct gear and move off. Proceed at a safe speed. A collision or rollover can occur quickly - even during routine manoeuvres such as turning and driving on hills or over obstacles In difficult terrain, ensure 4WD is engaged prior to moving off and that all equipment is at the lowest possible setting and the tractor is in the most appropriate gear. If hilly, the terrain must be traversed carefully. Proceed up and down and not across the side of the hill unless necessary. Where the tractor is only 2WD, brakes should NOT be applied when going down steep, slippery hills, as a rollover can occur. Should wire or other objects become caught in the blades - apply the brakes to tractor and switch off the motor. When the slasher is lifted to remove objects around blades, tractor motor must be off and slasher supported by jacks or stands Continually perform visual checks of the area and the slasher throughout the entire operation, as there could be a possibility of starting a fire. Use correct manual Handling Techniques, Team Lifting, and Mechanical Aids. 7

8 Working Adjacent to a Road or Railway Motor Vehicle Traffic Compliance with Manual of Uniform Traffic Control Devices (What are the hazards and Risk of injury or death from contact with motor vehicles. All work will comply with the Manual of Traffic Control Devices some of but not all are listed below SAFE WORK METHOD STATEMENT Ensure any control device applied does not become a potential hazard to workers or pedestrians or vehicles. Correct signage will be identified and displayed before work begins. Signage devices will be set up at correct distances. Signage devices will be set up at correct heights. Signage devices shall be clean and in good condition. Local speed shall be reduced to the required level (40 kph when clearance to traffic is less than 1.2m). Vehicle travelled path shall be clearly indicated Only accredited traffic controllers shall be used for traffic control. Traffic controllers shall have their tickets available on site. Traffic controllers shall be given 15 minute break every 2 hours. Only trained persons will be given responsibility for worksite signage. A Traffic Management Level 2 person shall authorise the site signage. Records should be kept for all work site signing and delineation identified and maintained. 8
