2 SPECIAL OLYMPICS INFORMATION In order to participate in Special Olympic sports, athletes must complete an Athlete Application for Participation. All sections must be completed, signed, require a doctor s signature, physical and background check. Forms will not be accepted if not completed fully. Once completed, the application must be turned into the LEEP Office and LEEP will submit the form to the Special Olympics Minnesota s (SOMN) office. This application does not guarantee acceptance into Special Olympics program until official word is received from SOMN. Forms must be updated every three years to continue participation. Athletes are not allowed to participate in Special Olympics practices or tournaments without a current completed form. If you are in question as to whether you have this form completed, please call LEEP at and we can verify it for you. This form must be updated every three years, if it is not up to date, the athlete will not be able to practice or compete until cleared by Special Olympics Minnesota. All athletes must be a paid LEEP Member before participating in any LEEP activities. After the athletes have registered for their sports, LEEP will mail out specific information regarding practice times, tournament dates, what to wear, etc. Special Olympics is available for anyone over the age of 8 years old.
