2 ENVIRONMENTAL N/A 1.0 Definitins & Acrnyms Emplyee/Persnnel Unless stated specifically, these terms will refer t all BP emplyees, cntractrs, visitrs, and guests. Enclsed Vehicle - Trucks, cars, cranes, r ther equipment that is clsed n all sides. If a windw r dr is pened the vehicle n lnger meets the definitin f enclsed. Green Zne - Areas defined by Map: E25 (See Attachement A) and des nt require PPE except when wrk is being perfrmed r in regard t the use f winter tractin aids. X-BOX - POD lcated Nrth f the Nrth POD and frmally referred t as Nrth Annex POD and CW POD. FRC - Flame Resistant Clthing. PPE - Persnal Prtective Equipment. Safety Shes - Steel r cmpsite-te wrk shes/bts with an ff-set heel. Spggles - Safety glasses that cntain a fam r rubber piece that prvides a seal t the face intended t reduce the risk f dust and slid particles t the eye. PFD - Persnal Fltatin Device. 2.0 General 2.1 PPE will be required at all times utside f the green zne except as defined in this sectin r 6.0. The Green Zne includes thse areas marked n the attached map (Attachment A) alng with areas utside f the refinery fenceline, such as Lallendrf r the Navarre Training Center PPE will generally nt be required t be wrn inside buildings r enclsed vehicles. This prcedure will detail any vehicles r buildings where PPE must be wrn PPE will generally nt be required t be wrn inside the Green Zne n the attached map. NOTE: The Shift Relief Plicy requires specific PPE fr Operatins Persnnel even if in the Green Zne. 2.2 PPE is nt required when walking t/frm an enclsed mtr vehicle and a building utside the Green Zne prvided the parking space is in clse prximity (<50 ) t the building and nt in a prcess unit. This is intended t prvide emplyees access t their ffices/changing areas at Page 2 f 18

3 the start/end f the wrkday. 2.3 At the POW, parking spaces may be >50 frm the building prvided that the area is: nt a prcess unit, the emplyees are arriving t r departing frm the wrk day, and there were n parking spaces lcated clser than PPE will be required in certain buildings within the Green Zne as defined in sectin PPE may als be required in the green zne based n task. The risk assessment will determine what PPE is necessary and be dcumented in the Task Risk Assessment (TRA) packet r Jb Safety Analysis (JSA). 2.6 PPE is available at the Safety Stre. If PPE is purchased thrugh a vendr ther than the Safety Stre, it must meet the minimum requirements fund in this prcedure. 2.7 PPE shall be in gd wrking rder and replaced when necessary. 2.8 EMS, Fire Dept. & Law Enfrcement are exempt frm standard PPE when respnding t an incident. The PPE requirements will be determined by the n-scene Cmmander/Safety Officer. 2.9 Tled ERT cnducting marine peratins n the water and watercraft are exempt frm standard PPE. The ERT will fllw the ERT standard perating prcedures under the directin f the On-scene Cmmander. 3.0 Hazard Assessment 3.1 PPE standards were develped per the Hazard Assessment fund in Attachment E. All wrk tasks shuld be evaluated fr task specific PPE. The CW prcess will be the primary surce f Hazard Assessment and prvide PPE requirements fr wrk tasks. 3.2 MAINT-E-028 prvides guidance f perfrming Hazard Assessments when the CW prcess des nt apply within BP shps. 3.3 Base level hazard assesments have been cmpleted fr the Heavy Craft shp, lab building, utside green zne area, I&E shp, and the machine shp. The hazard assesments are lcated in Attachment G. 3.4 Other OPNS, SAF, and MAINT prcedures will cntain Hazard Assessments and PPE requirements. When applicable, these prcedures must be adhered t. 4.0 Cntractrs & Visitrs 4.1 The Main Gatehuse has a limited supply f persnal prtective equipment (PPE) fr lan. Upn departure frm the Refinery, the checked-ut PPE will be returned t the Gatehuse. Visitrs wh d nt return PPE will be respnsible fr the replacement cst and their cmpany will be inviced accrdingly. 4.2 Visitrs will be required t meet the PPE standards fr the areas they are visiting. If they cannt, an exemptin may be given based n an Page 3 f 18

4 evaluatin f risk in the areas that will be visited. This exemptin shall be written permissin and shuld be given by a member f the HSSE team r the Shift Manager. The written permissin must be available upn request. NOTE: Written permissin may be in the frm f an Truck drivers entering the refinery fr lading and/r unlading f bulk prducts, such as: LPG, Vac Bttms, Decanted Oil, etc., are expected t furnish their wn PPE. 4.4 FRC is nt required fr truck drivers lading asphalt at gate Third-party entities that wn fixed equipment within the refinery fence line shuld fllw their wn minimum PPE requirements, such as: cmmercial rail rad crews, First Energy and Air Prducts. 5.0 BP Emplyees 5.1 All BP emplyees will receive persnal Fire Resistant Clthing (FRC), an insulated FRC jacket and a fire retardant hard hat liner in accrdance with the fllwing: BP will purchase Nmex clthing fr Tled Refinery emplyees thrugh the Purchasing Department. Emplyees wh wrk utside will be furnished with their chice f ne; parka; insulated bib veralls; r insulated cverall. 6.0 Minimum PPE Requirements and Specificatins 6.1 Minimum PPE cnsists f: Fire Retardant Clthing Safety glasses w/ rigid side shields(may be detachable) Hardhat Glves while perfrming wrk r expsed t cut hazards Steel r cmpsite-te wrk shes/bts Hearing prtectin Persnal H2S meter (see nte belw). NOTE: It is imprtant t nte that H2S mnitrs will nt prtect yu frm a H2S expsure. These devices are warning devices which alert yu t the ptential presence f hazardus cncentratins f H2S. 6.2 Minimum required PPE fr the Water Intake will vary. See Attachment B fr specific PPE requirements. 6.3 Additinal PPE is required when entering r wrking in Aurra Pink Demarcated areas. These requirements are listed in detail in SAF 115 Crrsive Hazard Area Identificatin. 7.0 Fire Resistant Clthing(FRC) 7.1 Refer t SAF 043 Fire Retardant Clthing Prcedure fr mre infrmatin. 8.0 Eye and Face Prtectin 8.1 All persnnel will wear nly apprved safety glasses with side shields, gggles r spggles. Each type will meet r exceed ANSI Z Page 4 f 18

5 basic impact standards. 8.2 Sunglass type safety glasses may be wrn frm dawn t dusk. If indrs, sunglass type safety glasses cannt be wrn while perfrming maintenance tasks. See Attachment C fr images f glasses that fall under the categry f sunglass type. 8.3 Vehicles are n lnger enclsed when windws r drs are pen. Therefre, minimum eye prtectin is required utside the Green Zne. 8.4 Cntact lenses may be wrn under apprved safety glasses. 8.5 Spggles may be used as a frm f increased eye prtectin against dust. Spggles shuld nt be used t prtect against any liquids, chemicals, mists, gases, r vaprs. The fam /rubber liners are nt intended t prvide a cmplete seal and may absrb liquid and hld it against the skin. 8.6 An ANSI Z impact apprved face shield ver safety is required fr the fllwing tasks: Line breaking Rdding ut bleeds Handling r sampling f acids, caustics & ther crrsive chemicals Handling r sampling f ht asphalt Lighting furnace burners Lking int furnaces in peratin Handling and sampling hydrcarbns r chemical prcess streams High pressure water blasting (>100 psi) When using a grinder r pwered wire wheel Chipping r hammering that culd result in flying fragments r debris Entry int areas marked in high visibility aurra pink paint When using high-impact air driven impact guns As determined in the pre-jb risk assessment and specified n the Task Risk Assessment NOTE: In the event that a tight space des nt allw enugh rm t wear a face shield while using high-impact air driven impact guns, a risk assessment must be perfrmed. NOTE: If cntractrs are perfrming tasks that require a face shield fr impact prtectin the face shield shall meet the ANSIZ standard. If the face shield is nt fr impact prtectin, it may meet the basic impact standard. 8.7 Gggles with integrated face shields may be used as an alternative t a face shield fr chemical splash hazards such: Line breaking Rdding ut bleeds Page 5 f 18

6 Handling r sampling f acids, caustics & ther crrsive chemicals Handling and sampling hydrcarbns r chemical prcess streams 8.8 ANSI Z basic impact apprved gggles r spggles shall be wrn fr the fllwing tasks: Wrking/cleaning in the FCC Regeneratr until dust free (spggles are nt acceptable fr this task) Mixing refractry, mrtar r ther dusty materials Handling r wrking arund materials that generate excessive dust As determined in the pre-jb risk assessment and specified n the Task Risk Assessment. 8.9 A full face respiratr mask can be used as substitutin fr face shields, spggles, and gggles as lng as the face piece f the respiratr meets ANSI Z (2010+, if being used fr impact prtectin) When welding, welding shields/hds ver safety glasses shall be wrn See Attachment D fr specific requirements relating t selectin f filter lenses fr welding and cutting Thse standing in clse prximity t welding/cutting tasks shall als utilize dark green plastic cver gggles r tinted shield/hd that will prvide equal r greater eye prtectin. NOTE: Gggles/spggles cannt replace a face shield fr thse tasks that specifically require a face shield (like line breaking) The refinery will furnish prescriptin safety glasses in apprved frames fr BP emplyees per the terms and cnditins f the Prescriptin Eyewear Prgram. See the Health Center fr futher infrmatin. 9.0 Head Prtectin 9.1 All head prtectin must meet ANSI Z , 2009 r Type 1, Class G apprved prtective helmets are t be wrn as minimum head prtectin. 9.3 Type 1, Class E apprved prtective helmet are required t be wrn by affected emplyees when near expsed electrical cnductrs which culd cntact the head. 9.4 Each BP emplyee will be furnished a prtective helmet by the cmpany. 9.5 Baseball style hats shall nt be wrn under a hard hat. 9.6 The emplyee must make sure the fit is prper and the suspensin shuld be inspected befre use. 9.7 Prtective helmets can be equipped with chin strap and/r fire retardant Page 6 f 18

7 winter liner. 9.8 Hard hats must be wrn with the bill frward unless under a welding helmet. 9.9 Inspect yur prtective helmet fr any damage (cracks, dents etc.) and replace if damaged r fllwing a significant impact. It is recmmended t replace the suspensin annually and the shell every 5 years Hard hats shall nt be mdified (making hles, mdifying suspensin, painting, etc.) Hard hats must be wrn in cnjunctin with face shields, welding helmets and welding hds during field welding activities. If the hard hat is nt capable f being wrn in the welding zne based upn tight wrking cnditins, accmmdatins must be made t ensure that verhead hazards in the area are understd and cntrlled during the welding peratin In cases where the wrk requires use f SCBA s, Supplied Air Masks and Full Face respiratrs, hard hats may be remved. Overhead hazards shuld be remved r cntrlled in these situatins. This must be dcumented n the Permit t Wrk r Task Risk Assessment The the wner f this prcedure will apprve hard hat styles fr use at the Tled Refinery Hand Prtectin 10.1 Glves shall be wrn when perfrming wrk. Glves shall als be wrn when there is a ptential fr hand injury, such as climbing ladders, clsing tlbxes, picking up trash, etc. Peple are encuraged t carry glves with them any time they wear standard PPE Glves shuld be selected n the basis f the material being handled, the particular hazard invlved, and their suitability fr the peratin being cnducted. One type f glve will nt wrk in all situatins. Fr general wrk, the minimum specificatin is cttn general duty wrk glve t prtect the hand frm cuts. See Appendix F fr the glve selectin chart It is acceptable t wrk withut glves n jbs requiring a greater amunt f dexterity than a glved hand. Hwever a pre-task risk assessment must be cmpleted prir t beginning wrk Sme examples f specialized prtective glves are: Insulated Winter Chemical Prtective Thermal Prtective Abrasin/Cut Resistant Vibratin Reducing Fire Fighting Impact Prtectin 10.5 Glves may create a hazard when wrn arund revlving tls r rtating Page 7 f 18

8 machinery. The pre-task risk assessment shuld cnsider whether glves are apprpriate Ft Prtectin 11.1 BP emplyees receive a she certificate t aid in the purchase f apprved wrk shes. This prgram is administered by the Human Resurces Department Ftwear shall have an ff-set heel. This helps prevent slipping when climbing ladders Ftwear shall have a prtective safety-te (such as steel ted r nnmetallic te cap) meeting the requirements f ASTM F Additinally reinfrced insles/sles t prevent puncture hazards are recmmended Wear chemical prtective ft prtectin when wrking with chemicals The fllwing ftwear cannt be wrn in any prcess, maintenance, warehuse, r tank field area: Athletic style shes, flat sled shes withut an ffset heel, pen ted shes, crepe sled shes, tennis shes, sneakers, canvas type shes, sandals, high heeled shes, clgs r shes with metal taps Safety shes need t be replaced when they lse their effectiveness. NOTE: Steel te shes shuld be replaced if leather te wears ut expsing steel te cap. Winter Tractin Aids: Winter tractin aids are required by all site BP and cntract emplyees when walking/wrking n snw and/r ice cvered surfaces Any persn escrting a visitr n-site is required t prvide them with tractin aids if they will be in areas that require their use Emplyees may determine that fr specific tasks the tractin aids create a greater slip hazard and remving the tractin aids is the apprpriate mitigatin. The expectatin remains that all tasks are fully risk assessed t determine the apprpriate mitigatin fr the individual task at hand. If wrking under a JSA r wrk permit, a waiver f the tractin aids needs t be dcumented Refer t the Q&A lcated in SAF-026-RF-02 fr further guidance n winter tractin aids Hearing Prtectin 12.1 The minimum standard fr hearing prtectin devices allwed fr general use within Tled Refinery shall prvide an attenuatin f 24 db Page 8 f 18

9 Nise Reductin Rating (NRR) as determined in cmpliance with 40 CFR Part 211, Subpart B See SAF 081 Hearing Prtectin and Attachment B fr mre infrmatin n hearing prtectin devices, and when they are needed Respiratry Prtectin 13.1 Respiratry prtectin, air-purifying r supplied air, may be required by the pre-task risk assessment. This will be cmmunicated by the Task Risk Assessment Respiratry prtectin may be required at any time fr peratrs r ther persnnel depending upn the state f refinery peratins. Emergencies r ther prcess upsets may require the use f airpurifying r supplied air respiratrs fr unit respnse and/r evacuatin Respiratry prtectin shall be selected, used, and maintained in accrdance with SAF 109: Respiratry Prtectin Prgram Entanglement Hazards 14.1 These rules apply in all cases where PPE is required (areas utside f the Green Zne and inside the Green Zne, based n task): When wrking with rtating machinery r ther equipment that has a ptential t entangle an emplyee, the task shuld be risk assessed and include cnsideratin fr remving glves, lse clthing, jewelry, and rlling up/securing sleeves. NOTE: Hair can nly be tucked under a hardhat if the hardhat still fits securely n the head Water/Drwning Hazards 15.1 Emplyees wrking ver r within 6 feet f water at the lcatins listed belw, where the danger f drwning exists, shall wear a U.S. Cast Guard apprved life jacket r buyant wrk vest: Marine Dck Water Intake WWTU Bi Pnds Fire Water Pnd On any watercraft Other lcatins as necessary EXCEPTION: Persnal Flatatin Devices(PFD) are nt needed fr wrk near water that is surrunded by hard barricades, r if persnnel use 100% fall prtectin that is cnfigured t remve the drwning hazard, r if a Level 2 Task Risk Assessment deems that PFDs are nt necessary Ring buys with at least 90 feet f line shall be prvided and readily available fr emergency rescue peratins at the lcatins listed belw. These requirements d nt apply t break/ffice areas as lng as they are divided frm wrk areas by walls r drs, unless wrk is taking place. Warehuse 16.2 Minimum PPE Includes: Steel r cmpsite-te wrk shes/bts (see 11.0) Safety glasses with side shields (see 8.0) Labratry 16.3 Standard Labratry PPE includes: FRC lng-sleeved uter wear (i.e., cveralls, lab cats, r pants & shirt), Steel r cmpsite-te wrk shes/bts (see 11.0) and cnventinal r athletic style all leather flat sle shes are acceptable inside the lab. Safety glasses w/side shields (see 8) Additinal PPE may be required fr specialized tasks r by prcedures and include: splash shields/gggles, aprns, glves, and earplugs Sectin 14: Entanglement Hazards applies t this wrk area Maintenance Wrkshps 16.5 The fllwing is minimum PPE fr all maintenance wrkshps during nrmal wrking hurs r when wrk is taking place in the shps utside f nrmal wrking hurs: Steel r cmpsite-te wrk shes/bts (see 11) Safety glasses w/side shields (see 8) Hardhats and FRC are jb specific requirements that shuld be assessed per task Sectin 14: Entanglement Hazards applies t this wrk area 16.7 Exceptins: I&E Shp: The suth side f the yellw PPE line is cnsidered a PPE exempt area. Heavy Craft Shp: The west side f the green PPE line is cnsidered a PPE exempt area. Page 10 f 18

11 Machine Shp: There is n PPE exempt area (ther than ffices and break rms) 17.0 Training 17.1 Each emplyee wh is required t wear PPE under this prcedure shall receive training in the fllwing: When PPE is necessary. What PPE is necessary. Hw t prperly put n, take ff, adjust, and wear PPE. The limitatins f the PPE. The prper care, maintenance, useful life, and dispsal f PPE Re-training shall be given when: Changes in the wrkplace make the previus training bslete. Changes in the type f PPE used makes the previus training bslete. Inadequacies in the emplyee s knwledge r use f PPE indicate that the emplyee has nt retained the requisite understanding r skill Self-Verificatin 18.1 An annual review f SAF-026 will be cnducted by the prcedure wner Deviatins 19.1 Deviatins t this prcedure may be requested and authrized using the Persnal Prtective Equipment Requirement Amendment Request Frm (SAF-26-FM01) 19.2 This frm shall nly be used fr deviatins that will last less than a year, are nt task specific, and cannt be captured as part f a Level 1 r 2 Task Risk Assessment Deviatins are t be returned t and retained by the Senir Safety Advisr A deviatin frm minimum PPE requirements may be made when the PPE prevents the task frm being perfrmed. The variance shall be agreed upn by the Reliability Crdinatr, Perfrming Authrity, and BP Safety Advisr. This variance shall be dcumented as part f the Task Risk Assessment package. Page 11 f 18

12 Appendix A: Fire Retardant Clthing SCOPE This prcedure describes that each emplyee shall wear the apprpriate fire retardant clthing. Fire retardant clthing will help prtect all emplyees frm the effects f a flash fire. HEALTH N/A Special PPE & Special Hazards SAFETY Prtectin frm burns assciated with flash fires. 13 OVERVIEW The purpse f this prcedure is t prtect emplyees, cntractrs and visitrs frm ptential expsure t hydrcarbn flash fires. It is mandatry fr all BP Tled Refinery emplyees, cntractrs and visitrs t wear lng sleeved fire retardant clthing (sleeves dwn; zipped up) as specified in ur PPE Prcedure (SAF 026). FRC is required whenever utside f the designated Green Znes. Nmex IIIA is the fabric f preference fr BP emplyees. At a minimum, FRC fabrics shall be the equivalent f 6 z./yard per Nmex IIIA r an arc rating f 5.7 ATPV (Arc Thermal Perfrmance Value) r meet the requirements f NFPA 2112 Standard n Flame Resistant Garments fr Prtectin f Industrial Persnnel Against Flash Fire and have an arc rating f 5.7 ATPV (Arc Thermal Perfrmance Value). Fire retardant clthing is mst effective if it is wrn utside all ther clthing. The use f uter clthing ther than fire retardant clthing, except as nted in this prcedure & the PPE Prcedure, will nt be permitted. Layering f clthing under the fire retardant clthing shuld prvide cmfrt in all climates. Wearing f fire retardant clthing is mandatry and is a cnditin f wrking within BP Tled Refinery. This plicy benefits all emplyees, cntractrs and visitrs wh may be expsed t flash fire. 1.0 Exceptins 1.1 BP emplyees, cntractrs and visitrs have the ptin t adjust their fire retardant clthing (unzipped, sleeves rlled up, tied arund waist, etc.) t enhance their cmfrt level in ffices and cntrl rms (Satellites), as permitted by the supervisr fr that building. 1.2 When emplyees are required t wear special persnal prtectin equipment, fire retardant clthing will be wrn beneath the special persnal prtectin equipment. Examples f special equipment are as fllws: rain gear, dispsable cveralls which may be wrn during shutdwns t enter extremely dirty areas r vessels, welders leathers, etc. 2.0 Cntractrs & Visitrs 2.1 Cntractr emplyees and visitrs within the Refinery r ther refinery facilities are required t wear fire retardant clthing in the same circumstances as BP emplyees. 2.2 Visitr FRC shall be requested thrugh the FRC SharePint Site. NOTE: A limited supply f fire retardant cveralls are available at bth the Lbby and Main Gate fr visitrs in the event arrangements have nt been made thrugh the visitr s BP cntact. 2.2 It is the respnsibility f each individual cntract cmpany t supply their emplyees with prper fire retardant clthing. Any cntractr wrking at the Marine Dck r Water Intake will be required t wear fire retardant clthing. 2.3 Truck drivers entering the Refinery fr lading and/r unlading f bulk prducts, such as: LPG, Vac BTMS, Decanted Oil, etc., are expected t furnish their wn PPE which meets BP requirements. 3.0 BP Emplyees 3.1 Fire retardant clthing shuld have emplyees first name, nickname r initials and last name. All emplyees will receive fire retardant clthing, an insulated jacket and a fire retardant hard hat liner in accrdance with the fllwing: BP will prvide Nmex clthing fr Tled Page 13 f 18

14 Refinery emplyees. 4.0 Replacement 4.1 Emplyee FRC Requests shall be made thrugh the FRC SharePint Site.
