2 Oklahoma Self Insurers Conference & Exhibits Hard Rock Casino & Hotel Terms of the Contract We agree to abide by the regulations affecting the 2016 Oklahoma Self Insurers Conference and Exhibition. 1. No exhibit space shall be sublet or offered for use by any exhibitor without the approval of the convention management of the Oklahoma Self Insurers Association 2. No exhibits permitted that do not have at least one person in attendance at all times. Solicitation or distribution of literature is not allowed except in the exhibitor s leased area, and only by paid vendors/ sponsors. Please respect your fellow vendors! 3. Exhibit booth personnel shall wear an identifying badge at all times on the exhibit floor. 4. It is expressly understood and agreed, and the exhibitor agrees by accepting these contract conditions, that he will make no claim of any kind against the Oklahoma Self Insurers Association, or any of their employees or members for any cause whatever prior, during, or subsequent to, or in connection with said trade show, nor for any injury that may occur to himself or his employees while in the Hard Rock Hotel & Casino, Catoosa,, nor for any damages of any nature of character whatsoever, and such exhibitors will hold Oklahoma Self Insurers Association harmless from any claim of any nature whatsoever, including any function sponsored by Oklahoma Self Insurers Association during the period of September 21st & 22nd, This contract is subject to all conditions under which the Hard Rock Hotel & Casino, Catoosa, has made available to the Oklahoma Self Insurers Association, and the right is reserved to cancel the exhibit space assignment of any exhibitor. All points not covered herein are subject to settlement by the Oklahoma Self Insurers Association and the Association reserves the right to make such changes, amendments and additions to the rules or floor plan as shall be necessary. 6. Exhibits are to be left in place through the closing hours of the day of the Exhibition, Thursday 22nd, 2016 All exhibit material must be removed from the Exhibit Hall after the close of the show at 5:00 pm, Thursday Sept No breakdown of booths is allowed until AFTER the completion of the last break on Thursday September 22, Set up is Wednesday September 21st from 12:00 pm to 5:30 pm and 6:30-7:30 am Thursday September 22nd, It is expressly understood that being an exhibitor at the Oklahoma Self Insurers Exposition does not guarantee product/service exclusively or endorsement on behalf of the sponsoring or managing entities. 9. 3 2016 Booth Rental Agreement Oklahoma Self Insurers Association The Oklahoma Self Insurers Association agrees to provide the following to the undersigned exhibitor: 8 x 10 foot exhibit space skirted and draped, 1 trashcan and 2 chairs Background drape with 36 high side divider draping Electric is on request and must be noted. There is a $35.00 additional charge for electric Payment: Payment is due with 15 days of receipt of agreement Cancellation: Refunds will be paid on a cancellations rate of 75% of amount paid prior to July 15th, 2016 No refunds will be paid after this date. Attendees: Each booth receives 2 attendees. Additional attendee is $75.00 each Breakdown: Booths may be torn down AFTER the Friday afternoon break, unless approved prior by OSIA. 9 The Oklahoma Self Insurers Association Annual Scramble Golf Tournament Wednesday September 21, 216 The OSIA Annual Golf Tournament has become a terrific way to spend time with Your clients and associates in the industry. This will be our 6th tournament, the third time here at Cherokee Hills Golf Club. Each year it continues to grow and the more fun we have! Cherokee Hills is designed by renowned golf architect Perry Maxwell in 1924, the course at Cherokee Hills Golf Club is rich in Oklahoma history. The course was recently redesigned by Tripp Davis and has proven to be one of the most beautiful and challenging courses in the state. Four teeing areas beckon golfers of all skill levels to traverse tree-lined fairways, dramatic elevation changes, demanding bent grass greens, natural rock outcroppings and babbling brooks.
