37 Summary The Hazard Hunt (HH) programme was well received and fully supported by Qarn Alam Steam & Engineering team. kick off meeting (HH background, objective & deliverables) was attended by PDO : QA Stem TL, Site management, Engineers, Supervisors, Operators and Technician. Contractor :EMC (STS), ODC (Galfar), Al Sarooj and Al Haditha. Contractor : Site management, HSE personal, Civil, Electrical and Mechanical Supervisors. Observation; Galfar has a weekly HSE committee sites visits (i.e. HSE inspections) that was agreed to incorporate the HH programme within the programme. No significant major observation/ or findings was noted during the site visit exercises. Overall poor housekeeping at Al Sarooj Yard and workshops was a concern followed by the Galfar Welding workshop which reflects a poor safety culture induction at the Yard and workshop. Take away: Formalising of the HH programme for QA steam team, along with a replication of Galfar agreed programme across contractor committee. Site management to close out noted observation/ findings during the visit. Keep MSE posted of any updates and/ or support needed. 37
