We shall continue to pray for God s grace for each and every one of you this coming year. And, since it is the New Year, and as tradition has provided for forty-four previous years, here is your official invitation to the 45 th Annual KCIAGA tournament, to be held from Sunday, June 9, 2013 thru Friday evening, June 14, We have been busily preparing what we think is by far the very BEST TOURNAMENT that you have ever experienced and we are looking forward to a week of shared fun, golf and camaraderie with you, your family and guests. In addition to this cover letter, (pg. 1) enclosed you should find the following information: Shirt & Hat Information (pg. 2), Entry Form (pg. 3), Entry Form Instructions (pg.4) Tournament Fees, Tour and Entertainment Fees, and Apparel Fees Order Form (pg. 5), Schedule of Events (pg. 6), Entry Form Instructions with Hotel Information (pg. 7), and Tour Information Description Sheet (pg. 8). Please make sure you send in your Entry Form, Tournament Fee Form and all other forms that are applicable to the outing, with payment, by April 15, Any cancellations received after May 15, 2013, will be prorated based on costs incurred for that reservation event. *Please note that this committee is purchasing tickets to the attractions as a group, so reservations for these tours must be made by the May 1, 2013 deadline. 4 INSTRUCTIONS FOR COMPLETING ENTRY FORM 1. Complete all information as requested on the form. Entrants may be called long distance COLLECT if information is not completed or correctly supplied. 2. Signature of your Financial Secretary is not required. To verify your status as a Knight of Columbus in good standing, you must present a valid and current Membership Card with your membership number clearly displayed, at the time of registration. To be in good standing for this tournament, your dues must be paid through March 31 st of the current year. 3. Entry fee of $ includes 18 holes and riding cart for Thursday and Friday of the tournament and 3 evening social events. 4. Scramble fee of $50.00 includes 18 holes and riding cart Wednesday. 5. Ladies golf fee of $45.00 includes 18 holes and riding cart for Thursday event. 6. Wives and/or adult guest may attend all socials for a fee of $55.00 each. 7. Fee for children under 12 is $35.00, 12 and over is $55.00 and includes Wednesday, Thursday and Friday evening socials only. 8. Ladies Luncheon fee is $ TOURS! Tour fees must be included with entry forms. Please carefully review descriptions, instructions and fees for tour reservations on the enclosed information sheet. 10. PRE-FLIGHT SCORE INFORMATION: If you checked the entry blank as a firsttime entrant, or you have not played in the past five (5) years, you must provide one of the following: a) a photocopy of your home club handicap card within one month of Tournament entry deadline. b) two (2) 18-hole rounds must be played within a month of the entry deadline and the scorecards must be signed by the course professional or the assistant professional and submitted to the host council by the deadline.

8 TOUR DESCRIPTIONS TOUR #1 MONDAY...Brookwood Lunch & Shopping Spree The Brookwood community is a God- Centered educational, residential and entrepreneurial community for adults with disabilities. The Campus consists of: 8 Group homes, 2 Single-family staff homes, a residential inn, health and dental clinic, worship center, enterprise building, activities and administration building, 47 greenhouses, gift & garden center and the Café at Brookwood. You will have a touching and once in a lifetime experience after visiting Brookwood. Bus departs the KC Hall on Monday at 12:15 PM and arrives back at the KC Hall before 4:00 PM. Cost - $20.00 per person. TOUR #2 TUESDAY BAE Army Truck Plant Tour & Lunch BAE Systems is a Army Truck Production Plant for the United States Government. All trucks are carefully assembled for durability and protection for our soldiers. This large production facility covers many acres of land so that many types of army trucks can be produced at one location.
