Yu are required t have supprt and apprval n this applicatin frm the fllwing individuals at yur schl: yur principal, the schl nutritin/fd service manager, and the PE lead teacher. Prcess Cmplete and submit the nline applicatin by Wednesday, Nvember 7, Applicants receive ntificatin f the funding decisins n yur Fuel Up t Play 60 Dashbard within eight weeks after the Funding Deadline. Funding Guidelines and Terms If the applicatin is apprved, yur schl must cmply with all reprting requirements where yu will be asked t dcument the use and impact f the funding (See Funding Reprting sectin). Funding requests may nly be submitted by schls, nt individuals. There are tw funding applicatin deadlines during the schl year: 1. Wednesday, June 13, Wednesday, Nvember 7, 2018 Schls may submit an applicatin during mre than ne funding cycle per schl year. Applicants must select ne Healthy Eating Play and ne Physical Activity Play. All applicatins must cntain bth Healthy Eating and Physical Activity Plans. Fuel Up t Play 60 mnies and equipment/gds/services are meant t fund and stimulate activities that lead t lasting change within the schl envirnment. Schls are eligible t apply fr Fuel Up t Play 60 Funds if they have received ther Fuel Up t Play 60 mnies in previus years (frm Dairy Cuncil, NFL r ther prgram supprters); hwever: - The current applicatin must include a plan fr activities that are different frm and/r build upn past funded initiatives. - Applicatins that duplicate yur schl s funded Fuel Up t Play 60 activities frm previus years will nt be funded. - Schls must have submitted all required reprting fr previus funding awards Schls are eligible t apply even if they have received ther funding t supprt health, physical activity and wellness at their schl. Applicants receive ntificatin f the funding decisins within eight weeks after Funding Deadline. Funding will be disbursed shrtly thereafter. Funding Reprting Schls that receive funding must cmplete the fllwing reprting: - An nline Use and Impact Reprt at the end f the schl year t dcument activities, expenditures and impact. This reprt includes prviding daily meal participatin and dairy sales data fr the previus and current schl year. - If yur schl received Funds fr Fuel Up t Play 60 in the past, yur schl must have submitted all required reprting in rder t receive additinal funding. - Failure t cmply with the required reprting may prhibit yur schl frm receiving future funding. Funding Amunts and Limits Schls are encuraged t request nly the amunt f mney they will need, frm a minimum f $300 up t a maximum f $4,000. Funding shuld be used by the end f the schl year. The amunt requested t fund Physical Activity may nt exceed the amunt requested t fund Healthy Eating. Use f funds is at the discretin f the schl s Fuel Up t Play 60 Prgram Advisr(s) and/r the individual cmpleting this applicatin, wrking with the schl team (students and adults helping t plan/implement the prgram) and in cmpliance with any schl r district rules gverning use f grant funds. Funds recipient agrees t use the funding as utlined in the apprved applicatin frm. Any changes t the use f funding must first be reviewed and apprved by the lcal Dairy Cuncil. Funds recipient may be required t repay the GENYOUth Fundatin any prtin f the funding that is nt used fr the purpse utlined in the apprved applicatin frm, and/r nt used by the end f the schl year. Schls receiving funds and equipment/gds/services agree t prvide infrmatin abut use f funds and equipment/gds/services if requested by Natinal Dairy Cuncil, yur lcal Dairy Cuncil r the NFL.
