6 Team Evaluatin Frm The Team evaluatin frm (accmpanying dcumentatin) is a tl t help yu identify the level f yur team. Is HIGHLY recmmended that a cach fills it ut t determine yur team s level f play. Instructins: 1) Individual Assessment Frm a. Cmplete ne individual assessment fr each athlete and r Unified Partner b. Chse a pint value the best describes the individual s skill level c. When each sectin is cmplete add each scre bx fr a ttal pint value fr that sectin d. Add up the ttal frm each sectin and divide by six fr the individuals verall rating e. 8 Flag Ftball & Cheerleading Cntinued Seasn Ending Turnament When: Saturday Nvember 17th Where: Gillette Stadium & Franklin High Schl Teams that were at Franklin High Schl in 2017 will be given preference, but are nt guaranteed, t play at Gillette in 2018 Meals N lunch is prvided. Special Olympics Massachusetts is wrking with the Patrits t ffer discunted meals at CBS Scene fr all teams as well as wrking t arrange fr cncessins at Franklin High Schl Transprtatin All prgrams and teams are respnsible fr their wn transprtatin t and frm this event. Ntes This annual event is hsted by the New England Patrits Charitable Fundatin. With this partnership, this event als serves as a New England based Flag Ftball Champinship with teams frm all ver New England participating. 9 Sccer Cntinued Seasn Ending Turnament When: Nvember 4 th Where : The Gvernrs Academy 1 Elm St, Byfield, MA Meals A bx lunch f sandwich, drink, & fruit is prvided t each participant. All lunches are dnated by The Gvernrs Academy Event Parking Once nsite Gvernrs Academy students will direct yu t the apprpriate parking spt. Please fllw the students as the direct yu int the campus and park where instructed, s we may have a safe traffic flw in and ut f the campus. Ntes The annual Sccer State Cup is hsted in partnership with The Gvernrs Academy. The Gvernrs Academy has been the hst site fr this event fr the past 30 years. Students make up the Games Management Team and meet fr 2 mnths with SOMA staff t prepare themselves fr running this event. In rder t be a member f the Games Management Team, students apply during their junir year, where they shadw a senir fr event. Shadws and event leads recruit the entire campus t vlunteer, train vlunteers, and raise funds in supprt f Special Olympics Massachusetts and the State Sccer Cup. 9 P a g e

11 Cach Educatin Criteria T Receive Certificatin 1) Be a SOMA Class A Vlunteer: a. Cmplete a Class A Vlunteer applicatin (every 3 years) b. Cmplete the nline Prtective Behavirs (every 3 years) c. Cmplete a Caching in SOMA General Orientatin (1 time nly) 2) Cmplete the SOMA Sprt Certificatin test (every 3 years) 3) Cncussin Awareness Training a. Requirement fr all S.O. caches acrss the cuntry. i. Available thrugh the CDC website ii. Available thrugh the Natinal Federatin f State High Schl Assciatins iii. If yu have already taken it thrugh yur wrk r schl submit yur cmpleted certificate t SOMA and yu are cvered 4) CPR & First Aid Certificatin a. All prgrams t have at least ne individual wh attends practices and turnaments t have this certificatin. b.
