2 Team Cntract Dear Parents and Cheerleaders, First, I wuld like t welcme all new cheerleaders t ur team prgram and welcme back all veteran cheerleaders. This Team Cntract is a tl that Universal Sprts and Academics, Inc. (d/b/a Unique Sprts Academy ) has created t establish agreement between the parents and the gym. We als want t thank yu fr yur patrnage and fr the cmmitment yu are making t ur prgram. The facility is wned by my partner, Je Taglisferi and I. We will cntinue t ffer the same great level f service t ur gym family that we always have. My prfessinal relatinship with Je ges back ver 15 years wrking tgether during ur years perating under Maryland SuperStarz All-Star Cheer and Je wrking at Elite Gymnastics befre jining tgether and establishing ur wn facility. In additin, we have a staff f successful industry certified prfessinals wh have been by my side; sme ver 15 years. As Cheer Directr and wner, I have ver 20 years f experience as an all-star gym wner, cach and mentr. Over the last several years ur teams have wn numerus natinal cmpetitins and prestigius bid pprtunities including Alha Spirit Natinals in Hawaii, The All-Star Games in Las Vegas and Cheerleading Wrlds in Orland. We lk frward t cntinuing t build a successful prgram with yur child! Please read the fllwing cntract and sign the last page verifying that yu have cmpleted the fllwing: 1. Read the cntract and understand the cntents. 2. Understand that this is a seasn cmmitment frm June 2015 t April It is imprtant that this cntract be fllwed as budgets, schedules and team cmpetitins rely n all participants remaining in gd standing. 3 TEAM PHILOSOPHY The tw fundamental philsphies f the cmpetitive prgram are quality f perfrmance and pursuit f excellence. During wrkuts and cmpetitins bth themes are stressed and reinfrced. These tw philsphies help cheerleaders develp the mental, emtinal, and physical strength needed fr the high standards f perfrmance expected f them fr cmpetitin. Unique Sprts Academy s All-Star Cheerleading is gverned by the United States All Star Federatin ( USASF ). All athletes must becme members with USASF befre being eligible t cmpete in USASF sanctined events. TEAM TEACHING PHILOSOPHY The caching staff understands that cheerleading is a team sprt. The cach will give the cheerleader apprpriate guidance, nt nly t help the cheerleader t succeed in this sprt, but als in life. Self-cnfidence, psitive self-image and critical thinking will be taught t imprve the quality f perfrmance. We will respect the balance between wrking ut and taking care f ur bdies and each cheerleader is encuraged t strive fr quality and excellence at their wn pace. In this way, we hpe the cheerleader will retain the lve fr the sprt that riginally mtivated them t becme cheerleaders. 4 CHEERLEADER EXPECTATIONS HAVE A POSITIVE ATTITUDE!! Be ON TIME fr ALL practices and perfrmances. Put away yur cell phne during practice. Parents wh need t reach an athlete may call the gym. Attend all practices as scheduled. If practice is cancelled due t hlidays, weather, etc. make-up practices may r may nt be added at the discretin f the Directr. If yu are sick (nt cntagius) r injured, yu shuld attend practice and watch frm the sidelines fr any changes that may affect yu. If yur dctr recmmends yu refrain frm practice due t illness r injury, bring a written nte frm the dctr. PLEASE NOTIFY YOUR COACHES IMMEDIATELY IF AN INJURY OCCURS. If yu are running late t practice, yu must call the gym. If yu chse nt t attend practice the week befre a cmpetitin, yu may be remved frm that cmpetitin and replaced. Attend cmpetitins, perfrmances, activities, fundraisers, and cmmunity events as per the schedule determined by the Caches/Directr(s). Wear the required unifrm, hairstyle, and makeup n practice and event days. Be present t supprt fellw teammates during their perfrmances at cmpetitins. Attend scheduled Chregraphy camps/practices. Prmte friendship, supprt, encurage, and RESPECT all team members. Understand fully the cmmitment f being n the cmpetitive team fr the entire year Remain unified as a team during events, perfrmances, exhibitins, cmpetitins, etc., hme and away, in rder t prmte spirit, and psitively represent the Unique Sprts Academy All-Star Cheer Prgram. Set an example f gd behavir at all times (whether in unifrm r nt) in class, schl, hme, and at all functins, whether perfrming r nt. Champins Live Differently! Treat all athletes frm ppsing teams with respect. Unique Sprts Academy is cmmitted t prmting a healthy lifestyle fr all f its participants. Cnduct cntrary t this philsphy and/r cnduct which pses unacceptable risk and disregard fr the health, safety, and welfare f the participants may result in dismissal frm the prgram (e.g. use f illegal narctics, alchl, abusive language, etc.). PARENT EXPECTATIONS Being a parent in Unique s All-Star Cheer prgram can be an amazing experience. Help us make yur child s experience just as amazing by supprting bth yur child, and the prgram, with a psitive example fr yur child t fllw. In additin t encuraging ur expectatins f yur child, we als ask that yu agree t the fllwing: Gd sprtsmanship is mandatry at all cmpetitins, practices, and gym functins. 5 Parents are always welcme and encuraged t attend events, but are expected t attend as spectatrs and nt extra caches r cmmentatrs. The caches r Directr will have sle cmmunicatin with the cmpetitin fficials. Parents are nt permitted t cntact fficials directly. If yu have a cncern, please discuss it with yur cach r the Directr nly. Help us teach the imprtance f cmmitment, fllw thrugh, and dedicatin t yur children. Please help yur child be rganized and time efficient. Missed practices due t versleeping, prcrastinatin n hmewrk, a jb r nn-cheer related activity, will nt be accepted as an excused absence. Shw respect fr the caching staff, ther teams, and hsting rganizatins. There shuld be n drug r alchl cnsumptin while traveling with the team. It is ultimately a parent s respnsibility t financially supprt the activities that they chse t allw their children t participate in. Parents must understand that it is their respnsibility t fllw thrugh with the financial bligatins assciated with tuitin and ther fees. Shuld yu fall behind r fail t make the necessary payments, yur child's invlvement in the cheer prgram may be jepardized r even terminated at the Directr s discretin withut refunds r reimbursements. All transprtatin t and accmmdatins at events will be the sle respnsibility f each family. We will d ur best t get infrmatin t yu in a timely fashin; hwever, if fr any reasn a cmpetitin is cancelled r rescheduled, Unique Sprts Academy will nt be respnsible fr any travel reimbursements. Attend parent meetings. SOCIAL MEDIA N athlete r parent may pst inapprpriate messages n any scial media platfrm (Facebk, Twitter, Instagram, Fierce Bards, grup chats, etc.) abut anther prgram r individual. Vilatin f this plicy will result in dismissal frm the Prgram. ATTENDANCE POLICY Cmpetitive Cheerleading is a team sprt. Attendance at practices is crucial t the success f the team. Athletes are allwed 3 unexcused absences. Additinal unexcused absences may be grunds fr dismissal frm the prgram. All summer absences due t family vacatin/visitatins must be submitted in advance t yur Team Representative. N absences will be tlerated fr mandatry camps r events. Excused absences include, but are nt limited t: Death in the family Schl related functin that reflects a grade Cntagius illness with a dctr s nte Unexcused absences include, but are nt limited t: Jbs Sickness, cramps All-Star Cheerleading Page 5

6 Scial events Schl prjects, hmewrk, tests High schl sprts practices r events Traffic r lng distance TRAVEL TEAMS All athletes n travel teams are expected t represent the prgram with the same respect as at hme. In additin: Athletes MUST stay with a parent, guardian r designated chaperne. Parents are respnsible fr the athlete s transprtatin t/frm the event. Travel itineraries must be given t the Team Representative prir t departure. The use f drugs, tbacc, alchl and abusive language is prhibited. This behavir will result in immediate dismissal frm the prgram. All athletes are required t arrive at away cmpetitins n later than 4:00pm n Fridays. Htel Blck infrmatin will be prvided fr parents. Sme natinal cmpetitins have implemented a Stay t Play Plicy which requires all athletes, caches and spectatrs t make rm reservatins thrugh Cnnectins Husing. Teams and/r individuals that dn t abide by this plicy will be ineligible t participate. Families and caches will be REQUIRED t bk htel reservatins in this manner. We will let yu knw which events this applies t. UNIFORMS AND PRACTICE WEAR Unifrms and practice wear are required fr all athletes at an additinal cst, t be determined. A unifrm depsit f $150 and practice wear payment in full will be due at the time f fitting. Fittings will be scheduled ver the summer. CONFIDENTIALITY Unique Sprts Academy aims t prvide the best and mst unique chregraphy in the area. By jining the All-Star prgram all members and parents agree t keep all Unique Sprts Academy related material (written r intangible) cnfidential. In additin t ur fees, this agreement is an integral part f the cnsideratin received in exchange fr ur services, and includes but is nt necessarily limited t dance rutines, stunts r pyramid sequences, tumbling passes, and similar maneuvers. This is ur artistry, and it is subject t the same legal prtectins as ther frms f art. As yu knw, nbdy can legally cpy a mvie, painting, r CD withut the artist s permissin. Likewise, nbdy can cpy ur wrk withut ur permissin. Please d nt teach r share chregraphy, stunts, dance rutines, r ther material f any kind with yur schl cheer team r friends. This requirement is critical t ur success in a very cmpetitive field. Any vilatin can result in immediate dismissal frm ur prgram withut refunds r reimbursements. All-Star Cheerleading Page 6

7 TUITION AND FEE GUIDELINES Tuitin All Team parents are respnsible t pay Universal Sprts and Academics tuitin n the first day f each mnth. If payment is nt received by the 5 th f the mnth, a $35 late fee will be assessed. If tuitin is still nt received by the 10 th day f the mnth, yur child(ren) will be asked t sit ut and a replacement will learn the psitin. If yu have an utstanding balance after 30 days, yur child(ren) will nt be allwed t participate in any Unique Sprts Academy activity until all financial bligatins are current. Prrating due t severe injury r illness is up t the discretin f the Directr and must be applied fr in writing accmpanied by a dctr s nte. Prrating due t vacatin r excessive absences is nt permitted, n exceptin. Fr yur cnvenience, the gym has implemented an aut-pay prgram. Shuld yu elect t make the mnthly payments with this methd, yu will receive a 5% discunt n tuitin. This will als prevent any late payment fees. The gym als ffers a sibling discunt f 10% ff tuitin fr the secnd child and 15% ff tuitin fr the third and subsequent children. Fr a returned check there will be a $35 fee added t yur accunt. Checks will n lnger be accepted after a secnd returned check. Registratin, USASF membership, chregraphy, music and ther fees and due dates will be utlined in a payment schedule. Cmpetitin Fees These fees cver athlete entry fees, caches fees and caches travel expenses. All cmpetitin fees are NON-REFUNDABLE. FUNDRAISERS Optinal Fundraisers Fundraisers are available t Unique Sprts Academy members wh wish t participate but are nt required t maintain status as a member. Sales Fundraisers (prduct sales) Event Fundraisers Unique Sprts Academy ffers parents the pprtunity t wrk cncessin stands at RFK Stadium, DC Armry and Fedex Field. In general, 10% f prfit ges t Universal Sprts and Academics, and 90% is split amng the fundraising participants. All fundraising distributins will be at the discretin f Universal Sprts & Academics in rder t maintain its exempt status. Fundraising funds are applied t cmpetitin fees first, then tuitin and ther fees. Yes, yu can fundraise and funds will be credited t yur accunt, but under IRS law, in rder fr Universal Sprts and Academics t retain its 501(c)(3) nn-prfit, tax exempt status, parents are nt allwed t fundraise slely fr their wn athlete. It is imprtant t remember that Universal Sprts and Academics cannt benefit individual athletes. This is why a member fundraises n All-Star Cheerleading Page 7

8 behalf f Universal Sprts and Academics and nt fr themselves r any particular athlete. When an athlete r parent fundraises, the earnings belng t Universal Sprts and Academics and are distributed t Unique Sprts Academy members in accrdance with the federal laws with respect t nn-prfit, charitable rganizatins (including Internal Revenue Cde Sectin 501). PRIVATE LESSONS Private lessns may be scheduled with any Unique Sprts Academy cach fr an additinal fee. Prices are set by the gym, nt the caches. All athletes taking private lessns must be current n all financial bligatins. Private lessns will nly be held during nrmal perating hurs. At all times the nrmally scheduled classes and events in the gym take pririty ver any private lessns. PARENT MEETINGS It is strngly encuraged the parents attend these meetings t ensure a clear understanding f gym events, prcesses, and news. If pssible, these meetings will nrmally be held at multiple times fr yur cnvenience. Adequate ntice will be prvided thrugh Team Representative. COMMUNICATION If yu wuld like t speak with the Directr abut a cncern please call ahead f time t arrange a meeting with them befre r after a practice. Talking t a cach during practice time is unfair t the athletes. is the mst effective way t cmmunicate between the caches, management and gymnast families. Infrmatin like accunt invices, meet schedules and ther gym activities will be ed. Please infrm us immediately f any changes t yur address. Team Representatives will be assigned t each team. These are vlunteer liaisns between the gym wner, caches and the parents. They are respnsible fr sending and cllecting infrmatin. All-Star Cheerleading Page 8

I fully understand this dcument is a cntract between me and Unique Sprts Academy. By signing belw, I acknwledge that I will adhere t all f Unique Sprts Academy s rules and regulatins as stated in the Unique Sprts Academy All-Star Cheerleading Cntract. I als fully understand that if any f the rules are vilated by any f my family members, the result culd lead t my child s immediate remval frm the prgram. PERMISSION STATEMENT & RELEASE My child has my permissin t participate at Unique Sprts Academy. I cnfirm my child is in gd health. I am als fully aware f and appreciate the risk f participating in any activity n the premises f Unique Sprts Academy including persnal damages and lsses; accidental injuries, minr r serius, including head and neck injury; and lss f life. I hereby give my permissin fr Unique Sprts Academy fficials t call a dctr and/r paramedics fr treatment in the event f an emergency. I further agree nt t hld any Unique Sprts Academy fficial r staff member respnsible fr any pssible illness, accident r injury which may ccur in training, class r n Unique Sprts Academy premises.
